I visited this facility

My brother went into it after a stroke, in 2 weeks he got a terrible bedsore on his buttock the size of a 50 cent piece, due to his immobility, they never turned or rolled him over. They gave him a food tray n walked out knowing f he was unable to feed himself. Also he ended up with pneumonia n had to be hospitalized and had to have his sore debre, they left him in a wheel chair at 7am without a call button , he couldn't move, n it wasnt till I got there at 6pm n insisted he be put back in bed before they did it unhappily. If u wanna die, go there. Very small room with 2 beds.

My Dad was here during covid. I was NOT allowed to see him the entire time he was here. He was not happy. He was here approximately 9 months. Said he was left in a dirty diaper all day even after asking for help. Left in bed all day as he couldn't walk. Or reposition himself. Eventually got him moved to another facility within a week of being in the new facility they had him sent to er concerned about his bowl movements. Turns out he had colon cancer. He only lived 3 weeks afterwards. Beaver valley nursing home had to notice the change in his stools, white and appetite. They neglected my father and cost him his life. DO NOT TRUST THEM.... they care about the $$$ not their patients.

My mother-in-law is in Beaver Valley Health and Rehabilitation Center. So far, it's been excellent. She's only been there three days. They're caring people. Communication is really good. I like the idea they're going to have a meeting of all the caregivers for her so that we can have a plan. Her room is clean. Rehab is good. She doesn't go out for eating. They have good protocols when it comes to cleanliness and COVID because the camera they use to identify you is a no-touch screen. I like that its hands-free with COVID going on. The precautions are good for cleanliness and sanitizing.

Please do not take your loved one here They didn't do anything with my husband Left him in a dirty diaper which when he came in never wore one but when I asked why they said they were short staffed never gave him a shower because little do they know I marked his arm with a pencil which would come off in a shower didn't brush his teeth The rooms were dirty didn't even have a bathroom in it there was a bathroom my husband wanted to go in I looked in that so called bathroom it was dirty and basically they used it for storage The staff treated him terrible My daughter came in the night I didn't come in she was out in the hallway waiting for the nurse to change his diaper but was rude to him made him hold his pee little did she know my daughter was out in the hallway and heard everything she said to my husband basically provoking him and made him hold is pee until he said it out loud Well that didn't go to well with our daughter When she told me I immediately told them to have him ready I was coming to get him When we went to get him he was in a dirty diaper that a person from housekeeping changed him When he got home he took a shower and there was so much scrap coming out of his butt into the tub it was disgusting I wouldn't bring my dog there The kennels I bring my dog too is so clean it's a shame

My dad was in Beaver Valley Health and Rehabilitation Center. I wouldn't give it a very good rating. We were not happy with it at all. I think the physical therapy and the occupational therapy were good. However, things went missing, like many of his clothes. We just were not happy with it at all. The staff was OK. They weren't great, and they weren't good. They had a Christmas singalong, which is all I saw that he went to. The food was brought to him, and it was horrible. It's one of the things my dad remembers: how horrible the food was. I would suggest that there would be more communication with the family. We found out when he came home that he wasn't getting medicine that he should have been getting, and there was medicine given that shouldn't have been given.

Do NOT put your loved one in this facility!!! It’s dirty, the food is inedible! PT,OT and some of the nurses and CNAs are great. Most are not. You will call for a nurse and they may get around to see you in 1/2 hour or so. Or probably not at all. The rooms are disgusting. Furniture is broken. Guardian Healthcare should be ashamed of themselves. I wouldn’t put my worst enemy in this place. It is an awful place. RUN!!!!

Do NOT send your loved ones here. They will be ignored, and put into the same dirty clothes each day. We visited our mom every day and she was wearing the same clothes four days in a row. The other three, it was the same outfit. We brought her clean clothes and we laundered her clothes. So why was she dressed in dirty clothes each day? A trip to the doctor's office and she's in dirty clothes. What an embarrassment. And she's told they're clean clothes. They've lost the clean clothes we've brought her, too.

Was there for rehab. The staff is very good and do their best to please you. The PT people work with me to get me walking again. They were very good. I recommend highly.

This place let my friend go with out eating and no shower for days,was in rehab side of the facility,medicine is not given regularly like they are suppose too,nurses don’t Treat the residents right and they don’t even come see them on a routine check,the patient was so upset when picked up cause of it so I know there is something not right they need to be checked out with the social services

I agree with the other reviews that said they treat you like family. Deb(CNA), Brenda, ( nurse), and all the staff took care of my Mom ( their patient/ resident) on hospice( no regular food or activities) as well as my Dad( a diabetic). The nurse took his blood sugar and asked an aide to get him something from dietary. They treat everyone as if they are their own parent or family member. I know people say that, but I am a CNA in Anderson, Indiana and want to come work here or steal them to work with me!

This place is THE BEST rehab around. My 96 year old mother fell and fractured her pelvis and basically could not move. The fantastic care and great therapists there got her up and walking in 3 weeks. Between the great nursing staff and therapists both physical and occupational they did an outstanding job. They all worked as a team and treated my mother like family. Any questions I would ask they were always there to answer and never minded taking the time to make sure we understood everything. Over the years we have experienced their tremendous treatment with my wife's mother and father and her two uncles and aunts. We can't thank them enough . Sincerely, John Santillo

My mother is there for rehab & physical therapy. The nursing staff is not helpful when calling in to check on someone. They do not seem to know there patients very well. When asking how her PT was going- the nurse said she did not know. I asked if she received report on my mother & she stated - not about her PT. As a very seasoned nurse- I find this not acceptable. PT is the reason she is there & the nursing staff should be aware of how she is doing to know if she is improving- the reason she is there.

My mother had been a rehab patient twice before, but the third time resulted in long term care. The reason we kept coming back was the staff. At first, my father was their daily and my sister was there in the evening, with my visits twice a week to give them a break. My father passed, and my sister kept it up, with me keeping a twice a week presence.(I was still working in Butler,Pa.). After my sister passed, more of the visits fell to me,and I tried to keep up. Unfortunately, I live 1.5 HOURS away and couldn't keep up. I never had to worry! All the CNA's, LPN's and RN's, along with support personnel took care of her. I never had to worry!! Sure, the food was always an issue, but her room was clean, they prodded her to attend activities which she enjoyed, the personnel engaged her in conversation, and just held her hand as she was passing! What more can you ask of an "adopted"family, when they become closer than your real family? Employees are taught you can't get attached to the residents, but REALLY? In a lot of cases, employees are all these residents have. These employees are my extended family now. I will continue to visit here to see my "brothers and sisters". I love this place!

The only thing my ex husband was ever right about was the quality of care of this facility. Most of the nursing staff are excellent. The food? The rehab department is excellent. They are very knowledgeable and some of the nicest people I've ever met. They brought me to where I needed to be physically and I will be forever grateful.

My mother was a patient at Beaver Valley Health and Rehab for a total knee replacement. The care she received was wonderful. The staff was very attentive to her needs. Therapy was excellent. The building was clean. My mom did have some complaints about the food on a few occasions, but felt it was pretty good most of the time. I would recommend them to anyone.

I was a patient at Beaver Valley Health and rehab for about a month. I went for physical therapy as well as some medical issues. Everyone treated me very well and I honestly have no complaints. The staff made every effort to accommodate all of my needs. They even gave me a card on my birthday and allowed me and my family to use their conference room so my family could celebrate my birthday with me. I will definitely go back again when I have my other knee replacement. It makes me sad to read all the negative comments from others. No place is perfect but Beaver Valley comes pretty close.

I have a family member in Beaver Valley Health & Rehab Center. We are so impressed with the level of care she has received from all of the staff members as well as how clean the building is. The first thing we noticed on our tour was how friendly the staff was and the lack of the typical smells you encounter when you first enter some homes. Our loved one was at another facility and after visiting Beaver Valley Health & Rehab Center, we opted to relocate her there. She has been with them for about 6 months now, and we have never regretted our decision to place her there. We have recommended Beaver Valley to many of friends who have stayed with them for rehabilitation after surgery. They loved the therapy department, and agreed the building is very clean and the staff are all amazing!

I visited this facility

If you care for your loved one never, ever send them to this facility. When I arrived they had nats and flies in my semi private room. Refused to get me a walker, which I had to have due to a broken back. Had to ask for a pillow and when I asked for a blanket they stated they didn't have any blankets.!! I was given a Christmas throw that somebody had left there. Food was sickening and I demanded the dr release me the next day. He did. No phone reception at all on one side of the building. No house phone either. Very frightening.

I visited this facility

I took my husband here last night for rehab from a double knee replacement. They would not help getting him out of the car nor into the bed. One worker rammed him into a wall in the wheelchair. They don't seem to have walkers, had to borrow one from one of the patients.. All we heard was "the union does not allow us to do that" - how is it rehab then? The place smelled badly and the room / bed my husband was presented with did not appear clean. The staff seemed nice enough except for not being "allowed" to do anything but the supervisor definitely had a poor attitude. My husband says the staff was smoking inside, in a room off the hall. The kennel for my dogs is nicer than this place.. Does 1 star just mean it exists?

This place is horrible do not send your loved one here.The care is terrible and the place is dirty and smells like urine. My father was here after having a stroke he was unable to stand and get out of bed.The rehab people would come in and work a little with him but never got him up to work with him.He was on oxygen and every time I would go in to see him it was out of his nose and he could not breath well without it.We were there everyday.His hair was dirty and they were to order dry shampoo after 3 or 4 days of them never getting it for him we got it ourselves.We noticed one day that his left arm was swollen and hurt him badly so I got a nurse to look at it .She said that this happens with stroke victims and I did not know any better.A week later we got a call saying they were sending him via ambulance to med center ....5 hour later he passed away due to having a terrible infection in his bloodstream that was through out his body he also had a severe uti infection.Please do not send your loved ones to this place.So much more happened but it would take me forever to give all the details.

I visited this facility

Beaver Valley does a lot of rehab. It's average but fairly clean. Most of the rooms are doubles, not apartment-type. It's suitable.

My mom was in Beaver Valley Nursing and Rehab. It was very good. The staff was very good because they went above and beyond in helping her. I had not tried the food, but it looked a little better than the other place she was in. She was in a shared room, and it was adequate. I was satisfied with it. It was very clean, and the staff was very nice.

There are very FEW of teh staff whtuaally seem to care! My grandma was sent there by her doctor after being diagnosed with cancer & half the time, they "forget" to bring her any meals! They don't know how to empty her bag (a colostomy). They leave her in her wheelchair ALL DAY LONG & don't bother checking on her. I don't think I'd recomend this place to my worst enemy! It's a shame to think of how unorganized or uncaring these people seem to be! (Minus the very very FEW who actually pay attention!)

We have found most staff to be caring and interested. Several staff members have been outstanding! There is a missing link: continuity between shifts and coordination of care could be more thorough. An orientation program with new clients and family would be helpful. A client mentor or an advocate would help eliminate many questions.

I visited this facility

I toured the facility which was nice; however the Social Worker was rather rude which suprised me. Since I myself am a Social Worker I found this disheartening. I would not recommend this facility to anyone nor would say anything nasty. The entire time I was there I felt as though I was being talked down to and treated as if I were clueless. Overall the building was clean and neat. The patients seemed well cared for.
