My Mother was a Resident at Emerald Ridge My Mother had been discharged from hospital to her home and was declining rapidly. I spoke with Admissions Director Ginger J. on the phone who was very helpful to me to understand my options. She met with me and explained my insurance and Medicare benefits . She set up transport for my Mother to admit to Emerald Ridge . She was greeted by Ginger and the Unit Manager Jessica both of theses ladies made the transition so much easier for my Mother and our family. The staff at Emerald Ridge treated us like family they work very hard from Nursing, the therapy team was very knowledgeable . There were Therapy dogs for my Mom to love on. Unfortunately my Mom passed away the staff made her transition from earth to Heaven very comfortable and precious for us. I cant say enough about the team at Emerald Ridge I will forever be grateful to Mark the Rehab nurse and Ginger for all that you did for me.

My sister was placed here after her hospital stay and everyone has been very nice and professional. Has shown that they really care for there residents. Very friendly staff and a clean environment. Staff goes the extra mile to assist the residents and family.

The staff here are very inattentive. It takes ages for anyone to answer a call for help and meds are rarely on time. They can't get their basic tasks completed, forget about anything else your loved one might need. Anything you ask about just makes them uncooperative. Take your loved one somewhere else. This place is a nightmare!

Your "Journey Home" program promises a "post-admission meeting with ... family ... to formulate effective communications throughout the resident's stay", and "frequent communication ... to keep ... family informed of ... progress". You also promise "an invitation to a Care Navigation Meeting", and that you "will provide communication ... to keep ... your family informed of your progress". This did not happen, although some communication was provided. You also promise "an assigned Customer Service Liaison", which also did not happen. You also promise that you "will keep an inventory list of all your personal property that you bring with you". This also did not happen. In fact, upon discharge, the patient was dressed in both pants and top that were not hers. She was missing two bras, 7 1/2 pairs of socks, 2 pairs of slacks, and 5 tops. To "offset" this, she was sent home with 2 bras and a pair of panties that were not hers, the former having falsely been imprinted at your facility with her name, and the latter clearly labeled with the name of another resident. The resident's spouse was told by the facility Social Worker that he had heard several other complaints about the laundry service being provided by an outside contractor that was mandated by the corporate office, but apparently that was either not "sent up the line", or the corporate office didn't care. In addition, the local facility did not effectively monitor the service that was being provided. The resident's discharge time had been arranged the previous day for 10:00 am. Upon arrival at that time, the family member called the main facility telephone number, and was told "I'll be right out". 20 minutes later, another facility employee inquired whether I was being assisted, and said that she would inform the appropriate person of my arrival. A person soon came outside to review necessary paperwork with me, and deeply apologized, saying she had not been informed of my earlier arrival. When my wife was brought out in her wheelchair and assisted into the car, her personal belongings were also brought out. However, that did not include any clothes, nor hand/arm braces. When I confronted an employee about this, I was told "I am a CNA, and do not work in that area". In other words, "It's not my job". She did, however, at my request, ask the Social Worker to come out to talk with me. He went back inside and found the braces in her room, and some of her clothing (see above) in the laundry area. He also told me that he had not been informed of the previously agreed upon discharge time. Clearly, the discharge planning was stunningly inadequate. He also said that he would continue to search for the missing clothing, would keep me informed of progress, and would personally deliver found items to the assisted living facility to which my wife was being transferred. 48 hours later, I have not heard from him. I also asked that he forward a message to the facility administrator that I would like her to contact me. It was only roughly 30 hours later, and after a second request by me, that I received a call back. Finally, and perhaps most egregiously, the discharge paperwork indicated no skin condition areas of concern. Yet, my wife's groin area showed major inflammation, and even some skin breakdown; the crook of her contracted elbow required application of medication; and a toe on her stroke affected foot exhibited a major area of inflammation with an open wound, and based on the judgement of the home-care agency nurse at the receiving assisted living facility, was likely broken. It seems obvious that the staff at the rehab facility had not paid proper attention to the skin condition of my wife, or more damning, was aware of these issues, had not properly addressed them, and was lying on the discharge paperwork. I could not rate Emerald Ridge more than two stars overall. I was not satisfied with my wife's total experience, and cannot recommend this facility.

My mother was in Emerald Ridge for one week. Would have been less, but due to the holidays, I couldn't get her moved any quicker. She still cries when she talks about her stay there. The staff was nice, but they were extremely under-staffed and under-qualified. I was told they had no clean sheets for the beds- It was dirty and the rooms were tiny. 4 patients had to share a toilet and a sink. Beds were antiques and miserable. Furnishings, what was there- was cheap and very old. I filled out admission papers the day she left! They obviously didn't think that was very important. It was horrible. There was a help wanted sign out front, no experience required. Took 3 days to get her meds transferred from hospital. Wheelchair was horrible, would not roll straight, my mother had had a stroke and needed much attention than this facility could provide even if it had of been clean and rooms even a little more spacious. Craziness... It was my understanding that Mission Hospital bought this facility- suddenly it all made sense, most nurses at Mission were disgustingly rude, yet also overworked. They are understaffed as well.
