Do not have your loved one there! They are only after the the insurance money, can't keep decent staff, treat patients very poorly. Had break my friend out by ambulance they tried to stop me. Reported them to the state

My father has been at the Oaks for a week and has never had a assessment or seen and MD to lay out plan of care. They have not supplied him with towels for cleaning himself and let him stay in a dirty hospital sock with feces covering it. His wound has not been looked at by anyone other than a nurse and all that is being done is changing the bandages and not accessing a new area of concern. They staff stands around and lets the call buttons and phones ring without addressing the patients needs. The bathroom he has is so disgusting and the toilet leaks. The meals are never brought at a specific time and on the weekends breakfast is not brought until 10am. I will never have my father in this place again or recommend to anyone

I visited this facility

The staff is lazy instead of staff going to a bell when it rings they're worried about chatting it up or being on that damn phone check the phone at the door lazy's my ex-husband is a quadriplegic he lays in poop he's always crying because when he rings his Bell they don't bother he can't get up he's been dumped out of the bed on his face I would not place my bead dog there let alone a human being

I recently placed my loved one at The Oaks at Sweeten Creek. It is a hard task to place your Dad in a facility. We tried for so long to care for him and we found it too difficult. My family were a mess the day that we had to take my Dad to live at The Oaks At Sweeten Creek. But our nerves were put to rest by the wonderful staff. The Nurses and CNA's went above and beyond to make sure that my Dad was comfortable. What some would think a small thing during the assessment of my Dad we told the Nurse that he sleeps with 3 pillows. Before the assessment was over the CNA went and brought my Dad 3 Pillows for his bed. This made my Dad smile. The staff truly care about the Patient and their needs not just the diagnosis that brings them to the facility. They work really hard, the building is clean and everyone seems happy which helps me to rest at night knowing that he will be cared for.

The staff work very hard to provide care for the patients at The Oaks At Sweeten Creek. I am speaking from experience I know that when my mother was in need of care at the hospital most would not consider her because she was Medicaid. Ginger came and visited with my Mother and gave me the opportunity to tour the facility and meet with the staff to ask any questions. My Mother is now a patient and she is actually happier than she has been in a long time she attends activities she goes to the dining room for meals for more socialization. Good care is not in the color of the paint on the walls or fancy furniture it is in the true hands on care from the staff at the facility level. You should give this facility an opportunity to care for your loved one.

My mother is in this facility and is on Medicaid. I think the Oaks does what is can. It appears that there are a lot of medicaid clients here which probably dilutes the income and does not have a lot of paying clients. My mother before Medicaid was in a very expensive and beautifully decorated facility however the care was the same - just had more lipstick on the pig so to speak. I do think the staff is overwhelmed at the Oaks at times and there are some really hard working and caring caretakers.

I visited this facility

Nurses and nursing staff were chaotic and very unprofessional . I cried thinking of leaving my mom there. The PT was good but that's all I can say.

I visited this facility

Today my sister called to let me know that nurse Sally went beyond and above to tend to my sister Irene who has been at The Oaks at Sweeten Creek for about 7 years. Being that I live in San Antonio, Texas I am not able to check on her like my sister there in NC, so she gives me updates on Irene. It really touched both our hearts that Sally has so much compassion and cares about the residents there. You will know that your love one is in good hands under Sally's supervision.

My mother was a resident at The Oaks twice. Once as a rehab patient and then the second time for a couple of years as a resident. On the whole my mother received good care. As her health declined I found that she was treated well and with as much attention as possible. The nurses and aides were friendly and attentive. My mother did have an issue with her roommate who could sometimes be cruel. It took a while for this roommate to be moved but the staff tried to make sure that my mother was treated well. The Oaks always seemed to be clean and looked as homey as possible. I wish that they would update the furnishings and add more color in the public areas because it can seem a bit dreary. On the whole I think Mom was treated well in her final couple of years at The Oaks.

I am very shocked that no body is concerned for our elders. The man before us has special needs and should not be here with the lack of caregivers he was literally tied down with a bed sheet! The next things that I witnessed on my way to have coffee this morning was my friends Joe and Sarah shivering at nursing station. I asked Joe if he was ready for our morning cup of coffee, he replied , No. As he gasped for air! His oxygen line had fallen off! His head was tilted down with drool coming down his face! I then asked him if he wanted me to put his oxygen line back on his nose he said yes! I did place it back up to his nose and over his left ear! His head popped up and he said as he took in deep breath! I'm cold can you get me a blanket! I said, yes! I looked at Sarah and asked her if she wanted a blanket! She said yes! I turned to my left and seen an elderly lady in a special wheelchair with a tray on it shoved all the way up to her breast pressing her back into the vinyl straps of the back of the chair. She was crying and I did not know how to help her! I went to get my CNA to help with the blankets and the lady crying! I'm praying as I go down 100 hall! Lord help me help them! I asked the CNA's to get blankets and help the lady. They said, in a couple minutes. Then I went back to tell my friends Joe and Sarah that help was coming! Is it really coming!

My first impression of the facility was that it seemed very clean and was well maintained. Furnishings were arranged to give residents a feel of home. Staff was friendly and very professional. Like most assisted living facilities, there were too many residents for the number of staff on duty. My mother is there due to a fall causing a broken leg at another assisted living facility. The Oaks staff were much more attentive to the fall risk associated. Also, the staff went out of their way to get to know not only my mother but the rest of the family as well. The only real complaint that I have concerning the oaks is of the kitchen staff. The doctor and the family left specific instructions on types of food to avoid. The kitchen continually ignored those instructions and sent food that was listed as food to avoid. Nursing staff did a good job in removing that food, and returning with approved items
