Would DEFINITELY NOT recommend!!! Very short staffed. Patient had a bed wound that was neglected regardless of my request to treat the wound. Weekends are terrible because there is not enough staff to meet the needs of the patients. The food is very spicey…not suitable for some of the patients. The facility is very attractive, but I would NEVER recommend for your loved one. The administrative staff appear to be very nice, however this facility is does not meet the needs of the patient. 1/4/2024

Poor care and basic needs were not taken care of such as bathing, replacing sheets after mess,etc. Social worker is not helpful. Dr and physical rehab staff did not assess enough. My dad was discharged and fell and broke hip that night.

I am/was a resident of this facility

Everyone was attentive and caring. Though busy they would take time to talk with me. ( I have the gift of gab) Meredith made it all work for me and the team made it like home. Tequila, Kofi, NG,Cordell, Jody, Hazel and her team, the lady who delivered my meds and many others helped me get stronger. From the kitchen to the cleaning crew they are top notch. I would highly recommend this facility. I had the best experience. The rooms were cleaned daily and the food was good and hot. According to your diet it was good. Therapy team was excellent. They really pushed me. I had to put forth the effort. Again I had a great experience and highly recommend this facility. Bring your A game because they are the A-Team.

TERRIBLE PLACE! My mom was not well-cared for, in fact more like neglected! A couple decent staff, but a staff sighting is RARE. Mom was literally told to wet herself, NOT bathed until we complained and even then only one each week. Tried to keep her indefinitely - wouldn't give us a projected release date. Promised care team meetings - NEVER had a single one during her one month stay. Only allowed dinner in a dining room, rest of the time had to be served in their room. Had to insist on mom's release, they seemed to be very happy burning her fully paid days. Tried to move her to another facility, but Belmont doctor denied. Not honest! When finally released, they sent her home with an entirely new diagnosis on her discharge - Cancer! Which she has never had and still doesn't have. RUN! DO NOT PUT ANYONE YOU LOVE IN THIS PLACE! There is so much more that was wrong here, but hopefully this helps you move on down your list and avoid putting your loved one in this danger!

The experience went from lovely, which is what the short term reviews are about, to so dismal. I have a family member that volunteers at a small nursing/rehab facility and it has many daily activities. However, our family member who came from an Allen, Tx assisted living, walked tine with a walker. We even took her to eat weekly...however, when moved to the 'fancy' looking Belmont, she was sat in a wheelchair and even a Catheter. It alone was painful but not exercising her legs to walk like at the assisted living, leaded to her going through excruciating enimas when she never had ANY problem prior. I only learned how bad she was suffering when I was decorating her room for Christmas...She was napping in her bed and started crying out " No more, stop, you are hurting me! Stop hurting my butt" . I called my sister, an MD immediately, think she was being assaulted but my sister said she has cried about this to her children, including the one that was the RP, before the savings ran out. She endured this for 3 years and I was sick about it every day until she died... The 'nice ' workout facility was never utilized by actual residents with staff. The daily activities was only a church service and one bingo a week. There was nothing for her to do but stare at that laptop sized TV , which got smaller when the money ran out, and reminded me of an episode of 'the Twilight Zone. I am still troubled but once the savings runs out, it becomes impossible to move to a better facility. --Please think hard about this. Nice lunches do not make a life!

My wife was there for 3 weeks. Very short staffed. Except for one aid, most of the staff acted like they were doing you a favor to help. Food was not good. When my wife was discharged we found a half dollar size pressure wound on her heal which had developed gangrene. DON'T GO THERE. RUN.

I visited this facility

My brother sat in pain for 3.5 hours waiting for the nurse to give him his pain meds. There was not anyone at the nurses station. When I finally found someone all they had to say is they were left with mess from the earlier shift. There was one person screaming for help. I wouldn't board my dog at this place.

I felt I needed to write a review, because I know that many times reviews on nursing homes are only done when there are problems and no nursing home is perfect. However, the Belmont is truly a wonderful place and not the first facility my loved one has been at. The responsiveness at this facility is commendable. The staff treats my son and myself like family. It is a really pretty place and it is quiet, nothing to dislike at all.

I am/was a resident of this facility

This place is horrible. I was not treated with respect or dignity. Nobody bothered to familiarize themselves with my case, nobody checked on me regularly. And despite me informing them about my food sensitivities I still got things I couldn’t eat. The nurses would act like helping me was a burden even though it’s their job. I was transferred to a new facility after two days.

UPDATE TO MY PREVIOUS REVIEW: I've raised my rating to 3 stars where it will have little effect on their average, positive or negative. The doctor did visit and the medication problem was rectified. However, I'm not sure if it would have happened without our call to the after-hours service. They said they visited last week and that their representative (a nurse-practitioner or physician assistant, I'm not clear which) also came by. However, if that happened (and I'm not sure how it could have without our awareness), they didn't engage any family members or discuss the medication issue with anyone. So, those alleged visits wouldn't meet my expectations anyway. The other staff continue to be GREAT. I am optimistic that from this point forward there won't be any more serious problems. I think this was a failure in the in-take process exacerbated by poor communication. Now that relationships are established, things should go more smoothly. I have tried to be factual and fair in both my reviews. Even though there were some issues, taking everything into account, I would recommend this facility (at the time I am writing this, anyway).

Our mother was at The Belmont in Allen, Texas from Dec. 31, 2019 until her passing on January 22, 2020. When it became clear that she needed a skilled nursing facility we did extensive research to find the best option. This was very difficult because so many reviews of these facilities were filled with negative comments. After physically checking out two, we chose The Belmont. We could not have been more pleased with this facility. At age 94 and in failing health, we did not truly expect recovery and we just wanted Mom to be as comfortable as possible and get the best care she could. The nurses and aides at The Belmont were outstanding. We watched interactions, particularly with the aides, as they dressed, changed, bathed and fed Mom. They were gentle, kind, compassionate and patient. While difficult to watch Mom’s decline, we were comforted in knowing that when we weren’t able to be with her, her needs were being met. The facility is well kept and the administrative staff is responsive and provided whatever information we needed to make the right decisions. Some of the most helpful staff that we encountered were Gloretta, Debra, Thomas, Ashley, Maggie, Angela, and Ngozi

Run! Don't give them your money, it will go downhill once you run out. Smaller room, even smaller TV, no service from 6pm until 6am......

We spent three days in this facility before discharging my family member. We were moved to three different rooms. The first room did not have a working tv. The second room did not have a working air conditioner. My family member slept in a room that was 82 degrees with no fan. This room reeked of feces and urine. The third room shared a bathroom. We were told the man who shared the bathroom was non- ambulatory and would not be an issue. We found his diapers piled up on the floor. We repeatedly requested they be taken out of the restroom. The third day they called to find out if we took our family member out of the facility. They lost our family member. When we arrived, we learned they found our family member. We also discovered the non- ambulatory man in our family member’s bed, screaming for us to get out. This man assaulted two people in our family. The staff did nothing other than to ask us to file charges with the police. I also discovered my family member had not been given proper medications. He was not initially picked up at the correct hospital even though they had the correct orders. Our family member missed dinner. When we arrived they did not have food for our family member to have for dinner despite the fact they went to the wrong hospital and delayed their pick up. After being in this facility for three days, our family member regressed to the degree we took them directly to an ER. They were immediately readmitted to the hospital for 12 days. I attempted to discharge after the first night sleeping in a room of 82 degrees without their proper medications. The Belmont forgot to order them. They refused to discharge my family member. Three days later my family member was in a hospital with pneumonia and exasperated symptoms of the illness that placed them at the Belmont. My family is living in their home today without skilled nursing.

This is one of the nicest and cleanest buildings I have been to and the head Maintenance Man Jody White not only is concerned about the safety, security and upkeep of the building, he takes time to listen to the residents needs, request and concerns then work to satisfy each one in a expedient & professional manner..

I am/was a resident of this facility

Therapy was fantastic. I love to do therapy. Was in a motorcycle versus car accident and was not in a great condition. Went to The Belmont for the intensive therapy and nursing care. Now I can say that I am walking and driving again. A great turnaround from this tragic accident and I just want to give the great kudos to the therapy team. I wasn't even sure if I was going to walk again. Thank you.

Horrible, Horrible, Horrible - Giving them a 1 star is being generous... My father was there for rehab after a hip replacement during the month of Dec. 2016. I am sure there are quality nurses there but not in the wing my father resided in. All I can comment on are the morning and afternoon LPN's. They were inattentive, uncompassionate, understaffed, unpleasant and rude. The LPN's would ignore you at the front desk..it took me one time for that to happen - after that, I walked up and interrupted whatever they were doing just to have my father attend too. My father had to go to the bathroom and not only did he have a catheter- he was in a walker. He needed assistance and this one particular LPN...Honestly, I never saw someone move so quickly to get out of the room not to assist him. I had to assist my father with maneuvering his catheter through his pant leg just so he could use the restroom. This same LPN was told about feces on the carpet that needed to be cleaned - 2 days later nothing and I had to report to the Director of Nursing for it to be rectified. The only thing I can say positive was the Director of Nursing who tried his best to accommodate but he is also the one that allows this poor treatment to continue instead of firing the LPN's that are worthless...I IMPLORE you to not send your loved one to this facility and to make matters worse... not only did I check reviews (which checked out better than the other place we could have taken him) I was told by the Social Worker at Medical City Dallas that this place was good...

My mother stayed here for therapy i was hesitant that she would not be able to stay at home by herself after her fall but they did a fantastic job. Mom is now at home even though we had to hire someone to assist her at home for a few hours she's able to be in the comfort of her own home...Thanks Belmont for all the wonderful care she received. .

The Belmont did a fantastic job with my dad who was there for several weeks after a hospital stay for heart failure. He was very stubborn, especially when it came to therapy, but they never gave up and he ended up participating with much coaxing and several tries, but he did it nonetheless. I very much appreciate the repeated efforts. All in all, everyone was great from the ED/ Matt, DON/ Michael, UM/ Ashlee and all of his nurses on the 800 hall, especially Joyce on day shift. She was phenomenal.

My grandfather was a patient at the Belmont for 2 weeks. for rehab and my family and I were very pleased with his care. His nurses, cnas and therapist were very patient and kind. Should he ever need rehab again we will definately return.

My mother in law was released on 3/20/16 from this facility after a 17 day stay. She had a variety of medical issues and concerns and needed rehab after surgery. Unfortunately, I have nothing positive to say about the facility. The kitchen messed up her food daily. The social worker advised various members of our family that she would handle everything necessary to make an easy transition for her once she left the facility. 10 days in the doctor never came to check on her after she was not responding to various medicines they were providing her. Finally she heard he was in the facility, had to go track him down and then was treated poorly. He was short with her as if he had no time for her. As soon as he saw her, he told her she was obviously not improving because of her diet. He changed her diet and of course she improved with the combination of medicine and diet in a matter of days. She is diabetic, and had special dietary needs. They still provided her with sugary desserts which in turn led to high blood sugar readings. On 3/19 (Friday) at 4:45 PM my husband received a call from the social worker saying she was being discharged 3/20/16 (Sunday) and all arrangements needed to be made. He told her that was not enough time to get the items they required for discharge since he was under the impression she was handling that, and her response was you better call the insurance and she is still being discharged on 3/20/16 by 1 to 2 PM. She complained to well she's been complaining about food and it's not our fault that she hasn't been getting better. The part time assistance she found, did not even take our mother's insurance. So they would not be able to provide us with help. She needs a walker because she has difficulty walking without assistance. So at the end of the day on Friday, with no notice and no one open to get these things taken care of, we were left to figure out the things she promised to handle . By 5:30 on 3/20/16 (her discharge day), they still had not done her final blood work to ensure that she was ok to go. As a result, she was not discharged until 10 PM on 3/20/16. This facility appeared to be a nice place for her to be, but aside from her positive experience with a few nurses there is nothing better we have to say about this place.

Run! My Grandmother was only in the hospital for almost 2 days. She has been in an assisted living home in Allen where she had a phone, private shower in her room,etc. She is not in need of major care still, however, Myself and family has tried to make a call to her....they try to get the only nurse on duty to take their cell to the person....trying to get in touch with a staff is highly unlikely (...because the person is busy doing what? I have yet to figure that out since this bldg is so small.) ***Do NOT let the decor fool you into putting down that front fee and needing to change later...it will never be worth any amount of savings and they are not cheap!

I visited this facility

I admitted my mom after a knee replacement to The Belmont. I was wowed by the level of service that I received. The staff was very caring and the rehab was outstanding. I was floored by how well my mom did during her stay at The Belmont. I usually don't write down any comments for anyone, but this time, I feel that everyone should know that my mom is back to her home because the staff at The Belmont did a superb job. I want to thank Juanita, Franklin, Deborah, Sam, Bertrand, for all you did for mom. I'm sorry for everyone else I left out because everyone deserves a standing ovation. If you loved one needs any type of care after being at the hospital, I highly recommend this place.

My wife was placed a The Belmont at Twin Creek a month and a half ago from the hospital. I had no idea what I was doing or what places to take my wife. When I toured the facility they helped and listened to what I was needing for my wife. They were so comforting and compassionate with me and with my wife. The therapy department was so great that even when my wife didn't want to do any therapy they motivated her the keep on. All the nurses here were so great and helpful. My wife really loved how her nurses and the rest of the team treated her with smiles and kindness.

My mother was at the Belmont for last three months due to a stroke. She came in unable to walk use her left side, slurred speech, and having hard time swallowing. She has done amazingly now since being placed at the Belmont at Twin Creeks. The staff from nursing, nurse aides, and dietary have been truly a God send. They have been so patient and kind with my dear mother. I couldn't have asked for better care during this time of hardship for my family. I would definitely recommend this wonderful place to anyone that needed care.

If I could less than one star I would. The care my mother received at this facility was horrendous. She was there for 2 weeks after a fall. She had a broken pelvis and dementia. Between myself and my brother we would visit her 3 times a day and many, many times we would find her naked in the bed covered in her own feces. The staff was very rough with her when they cleaned her up. When she arrived at The Belmont she was in pain but interactive. The last 6 days she was there I kept telling the staff there was something else wrong with her because she was acting so crazy but they ignored me. I finally got a phone call saying they had done a blood work up on her and the results said she was in critical condition. They transported her to the hospital where we were told that she septic. She died 2 days later. She had no infection or UTI when she arrived at The Belmont. Her injuries were none surgical. So the infection that killed her set in at The Belmont. I guess never being cleaned properly will do that. The afternoon of her last day there I went in to check on her and found her hand covered in dried feces. It was under her fingernails and crusted on her cuticles. I tried to clean her with a wet wipe but when I asked the charge nurse for a pan of warm water to soak her hand in because the wipe wasn't working she assured me that an aid would be right in to clean my mother's hand. I left as I was late for work. When we saw my mother 4 hours later at the hospital I checked her hand and it was the same. They had not cleaned it at all. No wonder she developed a deadly infection. If you love your family member PLEASE do not put them in The Belmont. What happened to my mother and what I saw happening to other patients will haunt me forever.

My mother is presently at Belmont and I am completed dissapointed in the care they provide. The nurses are rude and many times forget to check on the patient. Many of the people that bathe her are rough. She wore the same clothes for two days and even slept in them. I am sure that she did not have a shower for a week, and they have no records showing when she received a shower. When I was concerned about her coughing, the nurses blew me off and it wasn't until the rehab staff brought it to their attention that they actually listened. The rehab staff are wonderful and have been a great advocate for her. I hope to get her out of that place soon.

First impressions were good with this resort style facility, (except for the smell of pee as you round the corner towards the rooms) but we quickly realized that our mom was not improving but actually declining from the care she received in this place. We visited twice a day during her week long stay for two to four hours at a time and quickly realized the incompetence of care to not only our mother, but to many others who were yelling for help, with one falling from her chair of which I called to the attention of the staff. Other issues we encountered with my mom was a forgotten meal (not on her sodium free diet) delivered at 845 pm upon our request, no dr. visit for five days and only upon request the sixth day, up to an hour and twenty minutes to answer call, no dietary consultation as promised for my mom who came malnourished, noisy call buzzers and chatty laughing staff who hovered around nursing station and one worker who discussed some inappropriate events of her evening the night before to another staff member. She also came to work with a fever and then came in contact with my mother who had a very large blood blister that burst and left the contaminated towel at my mom's feet by the wheelchair. While this rehab has a good first appearance and a wonderful and compassionate chaplain on duty, it is not a place to take a loved one and trust they will be properly cared for. We left by ambulance back to the hospital to remove her from this terrifying experience.

Because I couldn't help save my dad hopefully this review will help save someone. One nurse on duty can't do everything. My dad was found on the floor in his rm unresponsive so I was told. He was Sent to a hospital across the street with tubes everywhere and died. I went to Belmont to retrieve his things and found a wk worst of soiled clothes and a handwritten note my dad wrote before he die. On the note he was trying to get someone to help him. This facility was well decorated but was not properly staffed at night. My experience was horrible. Now it's a year later since my dad died and I feel compelled to warn people to PLEASE do their research before you chose a rehab for your love ones. My dad fought for his life. My mother came here for rehab after a week in the hospital for pnumonia. Her first night there, she fell out of bed, laid in her own waste for hours before anyone checked on her. The staff was a mixed bag of super attentive, kind, to mean spirited and angry. The medical care was also lacking and the food was horrible. they lined up the residents in the hallway like cattle to feed when it came time for medicaitons and treatment. She was moved after a week, horrible feeling about that place.

We were fooled by the beauty of this place and the number of managers who said they cared. Our intent was to place my mother-in-law there for rehab and stay as a resident for ongoing care. We were there every day and then again every night. Our observations and how she was treated led us to get her out of there as fast as possible after about 3 weeks. Mostly non-caring techs, not being fed unless we were there to do so, not cleaning leading to two UTIs eventually requiring a hospital stay, and much more led us to not bring her back. Food was left in her room at lunch and we would find that evening upon arrival--no one helped her eat. When in dining at evening, the techs would talk and commune while the residents would just stare at the food --and they did not care that we were there feeding our mother. A late night call from night nurse saying she was too much trouble and we needed to come up and stay (not enough people to take care of all the people there) when they knew all issues she had when admitted. We found her with major bruises on her body several times and the nurses said she fell (when she can not walk?).We found once her in the room reclining chair where they had placed her while changing sheets but was left there for hours until we came that evening. The afternoon shift had not checked on her or just left her there uncovered, cold, and unattended. Complaints fell on deaf ears -- "not my shift or not my job" This place is top heavy in management to sell you on placing there buy little follow up. Two meetings to determine care were cancelled by them and never followed up to re-schedule even after our trying to do so.

Do not be fooled by the "looks"!!!very beautiful but looks DO NOT mean good nursing care! Call lights can stay on for 30 +min without being answered. Try calling the floor nurses cell phone and you get"this number has not set up a mail box!" The great therapies cannot make up for the insensitive nursing personnel . I have been in the nursing profession for over 45 years,this is not how it's to be done.

I visited this facility

Very nice looking but don’t judge a book by its cover. My 93yr old family member was here for rehab. Like many skilled nursing facilities, the care is lacking. Staff is bottom of the barrel. One area of the facility smelled like pee and or poop every time I was there. Some of the staff would tell me anything so I would leave them alone. When you press the call button someone would come but then tell you they had to finish something and would be right back and this would sometimes take 30-45 minutes. My family member was on a puree diet so not sure about the food. Never was contacted by a Dr. They had my family member wearing the thinnest diapers I have seen. After a couple of hours the diaper would leak onto the bed. When he first got in there they were changing his bedding 2/day. I asked for a barrier pad and the nurse said they don't have any which was BS since my private caretaker scrounged around and found some. No bed alarms either which is a bad situation considering the substandard level of care here. Within a week my family member caught a virus there and ended up back in the hospital. If you have had to deal with these facilities then you know that none are perfect and the entire industry is overrun with poor caretakers. Regardless, most want the best for their family. Don't be fooled by pretty places. Walk in these places unescorted and look around. As rude as it sounds, look into as many rooms as possible. Talk to some of the residents and their families if possible.

Please do not put anyone you care about in this facility. There are a few of the nursing staff that are god and nice but most of the administration is not. My sister went there for rehab for a badly broken humerus and was supposed to be there for several weeks in therapy and then possibly staying there long term. They basically threw her out before two weeks was up with only the excuse of it wasn't a good fit. We were called less than 72 hours from the time they were dismissing her and told us to find another place. The building are very misleading because they are beautifully furnished and kept up. Unfortunately the rest of the service is terrible.

I visited this facility

Wife has been in The Belmonth at Twin Creeks for two days now. It has been great so far. The staff was nice. All her needs at this point had been taken care of in a timely manner.

When our mom first came to the Belmont she was comatose from a massive stroke that had left her unable to talk, walk, move and even swallow. With much time and attention from the Belmont's nursing staff and therapy department, mom slowly began to recover. First, the therapy department taught her how to move different parts of her body. In time, mom began to sit, stand and even walk. The woman who could not utter a sound was able to understand and respond. She soon was able to form words. Before long, she was once again able to have complete conversations. Thoughts that took forty five minutes for her to finish, now came out in normal conversation. We could not have done this on our own. Our mom came to the Belmont as a vegetable-but literally walked out on her own power. Mom has been able to regain much of her self sufficiency and dignity thanks to the time and attention from an incredibly gifted and professional staff. Thank you, to each and every caring staff person who nurtured, cared and literally loved my mom from death to life. What a gift you gave our family. You all - are priceless. I had a friend who went to the Belmont for her hip. This place was great. Great care and caring staff. Therapy got my friend strong and mobile. What was great is that she saw the same therapist every day. She loved the food too.

I had a friend who did their therapy here and during my visits I was happy to see such personable and caring staff that surrounded her. everywhere you walked someone always greeted you. She had a knee replacement and she said that her therapist was great. The motivated her to get to her goals. We are very thankful to The Belmont.

I visited this facility

My mom was recently in Belmont after having her knee fixed Her care was great staff attentive and therapy was amazing. she is walking without pain and able to get around without any problems. Our family really appreciates all she received while a patient there.

The workers here at this facility neglected, ignored and were unprofessional in taking care of my mother. They did not take care of her. They did not give her towels, medicines on time, food warm or help her to get out of bed or to shower. This place does not care for people. Don't go there!!!

I visited this facility

The building was very nice but the employees did not seem to take their job responsibly. The fact that the people staying here were relying on these workers to take care of them did not seem to matter. My mother was very ill and needed help getting proper nutritional and rehabilitative needs. Her mental state was also failing which no one seemed to take any real action to help. She stayed over night once and the next night she began acting crazy because at the time she was coming off of the drug Lyrica and they failed to recognize any of the symptoms she was displaying. She had no idea what was going on and was hallucinating and the workers did nothing to calm her down. We ended up having to pull her out of their care and bring her home because of their inability to do anything to help her. The workers seemed nice enough but did not seem actually qualified to be taking care of ill people.

My mom stayed at this facility for a week for rehab. She has been to several other facilities in the area over the past 10 years. This is one of the worst one we have been to. The place looks nice, but don't be fooled by it. The staff is very incompetent and will not take care of their patients. There were cases that my mom would have to wait for almost an hour to get any attention after riniging the bell for help. in another case we had to wait for almost 5 hours to get someone to give my mom sponge bath after we asked for it. They asked us to be there to help them with this, but the nurse kept disappering and would come back and say it would be another 15 minutes, and then she would be gone for 2 hours again.

My mother stayed here late last summer and into the Fall. The facility is beautiful and most of the staff were professional and personable. A few could have used some tips from their co-workers. Overall, it was a good experience for both me and my mother. I would recommend it to others, especially after reading some reviews about other facilities in the area.
