I visited this facility

I went to look at Azura Memory Care of Kenosha. They were very polite and informative. The place was beautiful and kept up. Their staff looked to be taking care of everybody very efficiently. The rooms were very nice and efficient. My only setback with the room was that there was no TV for my husband. The individuals that were in there just walking by looked very comfortable. The grounds were nice. There were bird feeders set up for the residents to check out. They had puzzles set out.

I am a friend or relative of a current/past resident

My grandmother was a resident in 2016-2017. At first they took very good care of her and when I visited they insisted that I eat lunch with her. After a new administrator took over it went to being very lack in caring. One time when I went there they weren't even going to give her lunch because I was there. She had a mini stroke and they did nothing the dr said she was fine even though the side of her face was dropping and her bottom eyelid was inside out. On a trip to Froedert the doctor agreed with me. The on week she fell twice and wasn't taken to the hospital until the 2nd fall. When in the hospital it was determined she first broke her pelvis and the 2nd fall was a broken hip. When the surgery was done she then had to moved to hospice to die. The director was going to meet me at the hospital and never did. So very disappointed it the care they gave her. And when I did visit here all the caregivers seemed just be wanting to sit and eat. I saw one patient almost throwing up and she continued to just eat until I said something.

At Azura we work very hard to address any issues in a timely manner, but as stated in the review, this perception of Azura is more than 2 years old. Many things have changed during that time, as we are always looking for ways to improve and implementing these enhancements. In addition, HIPPA doesn’t always allow us to communicate issues to all involved in the love and care of an individual. However, whenever a family or anyone has a concern our Azura Team is always willing to listen, address the issue and move forward to ensure the care of our residents. In addition, we have an Azura Connections system that allows for anyone to call, email, fax or send in a comment of concern or praise. We wish this process had been used, instead of reporting it 2 years after the perceived issues occurred. Thank you.

I visited this facility

My review is based on the admissions process and not the facility itself. When my wife and I initially visited Azura while looking for a long-term residence for my father, I spoke with an admissions advisor at length, I explained my father's current condition. That he has short-term memory issues but has many of his long-term memories, that he is ambulatory, that he smokes, and that he has anxiety issues (afraid to shower alone, afraid of confinement). The advisor assured us that smoking was allowed outside at the facility and that the staff was trained extensively in memory care and had handled anxious patients in the past. Based on this information and our overall impression of the facility and staff, we moved forward with Azura as a possible residence for my dad. About a week later, after my father had come out to visit Azura for lunch at their invitation and had a very nice time, the advisor called to let me know that actual cigarettes would not be allowed on the property but that smoking e-cigarettes would be OK. Later that same day, the resident nurse left me a voicemail advising that smoking e-cigarettes would be a short-term measure in an effort towards smoking cessation. The next day, I was informed by phone and email by admissions that long-term e-cigarette use would be possible for my dad. The following week the lead nurse assessed my father in-person, and immediately after that I was advised that he would not be admitted to Azura because he was a) a smoker, and b) because he is not completing daily care activities at the rehab facility where he is currently residing. This seemed strange to me based on the previous conversations I had with Azura staff regarding smoking and that the entire reason I am seeking memory care for my father is that he needs reminders (and maybe a little help) to take care of himself and perform daily living activities. The staff felt my dad would not be a good fit for their facility, which leads me to wonder how they determine who is a good fit.

We apologize for your frustration and understand your need to share with others. Please let us take a moment to try to explain our thoughts as well. Azura Memory Care is very committed to the overall wellbeing of our residents and specialize in helping those with memory loss and advancing dementia. While our current policy does allow supervised smoking in designated areas, we have found that smoking, even with assistance, is not a long-term possibility for those with memory loss as they begin to forget how to properly use the cigarette or other smoking material in a safe manner. Therefore, we have found great success in helping residents transition to an e-cigarette, which initially helps with their oral and nicotine needs. The beauty of an e-cigarette is that it slowly wears down the nicotine and weans the person off of that need and eventually their oral need dissipates as well. This is much less taxing on a person than asking them to stop, which could involve serious withdrawal effects often exacerbated by the dementia disease process. This is why we encourage families to help those with memory loss who are smoking switch to e-cigarette as soon as possible. In addition, a face-to-face assessment is always part of the Azura plan of care, which is why we ask for the ability to do an in-person assessment early in the conversation with the family. This assessment gives us a more clear picture of what is actually happening with the individual and if our care would fit their needs. It is rare, but there are times when the assessment finds that we would not be a good match for the individual. If that occurs we immediately notify the family and try to offer other options to them. Again we apologize that we weren’t able to help your loved one at this time.

I am a friend or relative of a resident

Azura Memory Care of Kenosha North is a really good facility. They just opened up a new building, and my mom is in there. She was the fifth one in there, and now it’s full, so it seems to be a really good place. The staff are detail-oriented, and they really pay attention to what’s going on. The food is pretty good; sometimes when I go there, I eat lunch with her, and they serve good food. They have a variety of activities. Every morning, they do exercises. And then they also have different activities, like building crafts and some physical activities. They’re very compassionate to the residents and their families. They take good care of the people.

I visited this facility

Azura Memory Care seemed to be very good. We were anticipating that they were going to build another additional wing -- which right now should be done in a month or so -- and that the next part of it was going to be brand new. Maybe my dad will be going into that particular part. The person who took me around was very friendly, and I thought she was really good at answering questions. The facility seemed to be OK.
