I visited this facility

The Symphony at Centerville was amazing. It was wonderful. The tour was excellent. They showed us several different rooms, like the dining room, the kitchen area, the facilities for exercise, and the outside grounds. The young man who talked to us was wonderful and very knowledgeable. He knew the patients, talked to the patients as we walked through the facility, and helped them. He helped one lady fix her telephone while we were there. He knew exactly what we needed to discuss about the finances. He showed us everything upfront. I've had a call from the manager of the site, and she was a very nice lady. The only drawback for us was the cost. My parents just couldn't afford it. The facilities were extremely clean and well kept. The patients were active. I was very impressed with the staff and the facility. My dad, who's 91, said he really liked it. It made him feel comfortable. I met several of the nursing staff. They have stations in each area of the home. It's kind of like a big circle, so each wing has a nursing staff. They were all very nice. We saw the menus, and they were listed on the wall so the patients could see them. There was a wide variety. They also showed us the activity list for the calendar, and all the things that were available for them to use. They also had two kitchens and two dining rooms, so it wasn't crowded. The rooms were spacious and clean.

The rooms are spacious and bright. The food choices and activities are well balanced. Something for everyone. Our tour, Shawn, was knowledgeable and respectful of my dad's thoughts. If you don't have to worry about dollar signs, this is the place to be. It's a private facility, which means they don't deal with Medicaid. If my parents could have afforded this we would have applied that day. Unfortunately, it's out of our budget. Other than that, I loved the facility and the staff.

I visited this facility

There were one or two things that I didn't feel my cousin would be comfortable with at Symphony at Centerville. She had to share a room, and for our money to stretch, the only room available was with an individual whose name was the same as her sister, and her sister's medical condition. That would make her uncomfortable, but the facility was very nice. The staff was also very nice, but it was a little pricey. It was beautiful. Their facilities were on one floor and it was spread out. You could go into this gorgeous courtyard, too. It was like a new facility.

I am a friend or relative of a current/past resident

Our mother moved in about a month ago and we are overall pleased with everything. The food is good most days from what I can tell, mother is a picky eater and even when she does eat it’s not much. The activities director Rea is AMAZING, the things she does with the patients leaves us in awe. The aides are nice and treat our mother and us with respect when we visit, they do seem to be short staffed a lot which is the only issue my family has with the facility. Would recommend to anyone looking for an all memory facility.

I visited this facility

I toured Symphony at Centerville at the beginning of my search. I found out that they're a memory care-only facility during my tour. The gentleman who assisted me was extremely helpful and provided me with information that I otherwise wouldn't have been able to find or get my hands on. He was able to help me redirect my search. They had flexibility with the pricing they could have worked with us. I have not been to any facility that would work with us on pricing. It was a brand new place. They had a gym and a salon.

I visited this facility

Symphony at Centerville was OK. It would not be my first pick, but it wouldn't be the last. Out of all the places that we saw, it was just kind of in the middle of the road. You would not be dissatisfied overall. The price was a little better than the other place, so you are expecting maybe a little bit less in return or availability. If I was to put my mother in there, I would be OK with it. The rooms were nice and were very similar to the others. Most of the rooms are furnished by the occupant. Most of the rooms we saw were empty, but they were more than adequate and well-kept. There was a long list of activities. On certain nights, they would take them out to eat or go to the Family Dollar and let them shop. They had internal activities where maybe live entertainment would come in. Somebody will play the piano, the saxophone, the violin, or sing. They had a courtyard, and they would grill out and depending on the time of the year. There were different things that were available to do. The facility was nice. We saw two rooms, the dining area, and the courtyard. We really didn't go to too many places there.

I visited this facility

My wife will be moving into the Symphony at Centerville. It seemed like it would be a better fit for us all the way around. The location was close to where we live. It was a nice and clean facility. The staff was very knowledgeable and answered all our questions. They have an outdoor area to walk around in, and it's all secure. They have a number of activities. They even take road trips with some of them. They have everything you can imagine.

My mom has moved into Symphony at Centerville. I like it because the entire facility is memory care. It's all on one level and circles around, so it causes less confusion. Mom's able to find her way to everything. There are about 6 to 7 activities a day available to join in, and they also had community outings. They have an exercise every morning, different things like trivia and bingo. They bring in outside musicians, and they have a social hour with different types of music each day. Everyone is very friendly. From the director to the cleaning people, they know all the residents' names and reach out to them whenever they see them.

I visited this facility

I went to Symphony at Centerville. The director was very attentive. She called several times to ask if I had more questions. They definitely have a really good chef, and it looked like their food is really good. You pay a fee for your room, and then you pay a certain amount when the person has more needs. If they're incontinent or if they have behavioral issues, that really adds up with the price. It's hard to know what it's going to cost until the person goes there, then they do an evaluation. They tell you how they're going to charge you for the different levels of care. It's hard for budgeting to figure it out. The facility was very nice. The apartments looked good, and everything was clean. It was good. They said they had just had a luau, and they do monthly birthday parties. The luau was kind of like a monthly family event, so it brings the family members in. I thought that was really good. They can request certain food if they want. The example was they have a resident that wants salmon every day. They fix salmon every day for this man. I thought that was nice to know that you're getting some good quality food and getting what you want. The food seemed a little bit more individualized. I only interacted with two staff people there, and they were very good.

Our client moved into Symphony at Centerville. The move has been very good. The admissions director is very professional, easy to work with, and very efficient. I gave options to the family, and the family chose them. I had a very positive experience working with them. They're very thorough and very good at what they do. They made his transition very easy.

My dad moved into Symphony at Centerville. The whole place is memory care. So far, it's been good, and it's only been 24 days. I just wish they would reach out to me more. They told me different stuff that's happening, but they didn't reach out when dad needed new prescriptions, and I had to call them. The staff is pretty good though. I didn't get to see his room until I had a window visit with my dad on Christmas Eve, and it looked very nice, very big, and roomy. I just got a picture from my sister that they'd taken a bus and saw Christmas lights, and they'd done that once before he moved in. I thought that was great, and it looked like he was enjoying it. From what I'd seen as far as I was able to get in there, from signing paperwork and stuff, it looked beautiful.

This place is such a blessing!! My dear mother was caring for my dad and she could not keep up with him. She was exhausted, not sleeping and i was worried about her more than him! We did a virtual tour and i loved what i saw! The staff was awesome! Carrie, Dani and Heather made the move in so easy and we visit dad regularly with FaceTime! Mom finally can get the rest she needs and dad is happy and healthy! We are so very please with every aspect of the community and feel it is a blessing to have dad in such a loving environment.

My father is now a resident of Symphony at Centerville. The place is gorgeous, it's beautiful and perfect for anyone who is suffering from dementia. The staff is perfect and the place is specially-designed for memory care. I have never seen them take a tone with my dad, never abusive, and instead they've been lovely and caring. The staff is amazing. Their chef makes the best cheeseburgers I have ever had. They have an enclosed courtyard with a garden so he was able to go outside, and they have a covered porch with locks on it. They also provide yoga, shows that pertain to their community, and they have people come in and play music. My mom even wants to live there. It's a very clean place. His room there is also spacious.

I am a friend or relative of a current/past resident

I am a disgruntled family member of a former resident. We feel my husband's six months at Symphony was damaging to his fragile self esteem and it increased his anxiety. His so-called “private” room did not feel private. Aides entered without knocking (until my complaints helped train them). A nurse came into his room one night while he was sleeping and cleaned between his toes! (She documented this.) Another time, an Alzheimer's resident climbed into his bed with him and wouldn't get out of the bed. (A nurse confirmed this to me.) Uninvited memory care patients wandered in and out of his room several times. (I witnessed this.) My husband's memory problems come from traumatic brain injuries, not Alzheimer's. He cannot always articulate that things upset him. Staff repeatedly put dead hearing aids in his ears although the devices blocked what hearing he does have. I repeatedly asked them to change the batteries every two days but they rarely did. Hubby would later pull out the hearing aids and then workers put them back in. The misunderstanding would have been cleared up if they had asked him if the hearing aids worked, or if they had checked for the beep, but they did not. Hubby began to feel disrespected. I feel their handling of his situation escalated his reactions. Staff continued putting his dead hearing aids in and he kept ripping them out. As he grew more frustrated, they asked me what they should do. I said the obvious: “Ask him???” As an assisted living facility, Symphony is not listed at the Medicare.gov website where ratings are assigned to Medicare eligible facilities, and government officials provide reports about the treatment of this fragile population. I now realize how important it is to choose a facility where records of infractions are documented in this way.

I visited this facility

Very disappointed, I was looking for a facility for my aunt and uncle both in their 90s and heard great things about Symphony, good food, stellar activities and amazing care staff, however I didn’t get to experience any of that due to the very rude front desk girl. I was driving by on my lunch break and thought I would pop in, the lobby was beautiful I was immediately impressed by what I saw, as I walked in the concierge didn’t even acknowledge my presence, let alone look up from the binder she was looking in. After standing around looking at the activity calendar I walked over and asked if it was possible for some literature as I didn’t have time for a full tour, she very rudely replied with barely looking up from her binder that she didn’t have any pricing information that she was just the scheduler, didn’t even offer to take my information. Very unfortunate because of the great things I have heard but if that is the buildings first impression or representation I wouldn’t want my family living there anyways.

We’ve been increasingly pleased with the care my mom is receiving at Symphony. The staff at all levels seems to genuinely care about her well-being, and the communication has been good as we’ve had to make some adjustments to help her settle in the first two months. This is a bright, beautiful, newer facility with engaging activities, good food, and an overall nice atmosphere that is better than more traditional nursing homes we’re familiar with.

I visited this facility

I can honestly say, this community is wonderful. So many heartfelt moments have been occurred in such short time. I know the staff strives daily to continue to please residents and families, staff is always smiling and patient. I am so lucky to have such wonderful staff to take care of my mother. The residents are always doing some activity, nursing team always updates and answers all my questions about my mother and it really helps this transition easier. Also it is very clean, and the food is wonderful. I can not say it enough, this place is truly a blessing!

I visited this facility

Symphony is really new. There were not many residents living there at the moment, but it was very nice. It was gorgeous, perfect, and I liked the layout. It was large and spacious. There was a mix up with the tour, and one of the nurses took us on the tour. She was very helpful, she answered what questions she could, but the only thing I didn't like was that time there was no activities director. They were working on finding one, but to me, that was a major flaw for the residents.

Symphony at Centerville is a new community of only six months. It's half full, and the environment is clean. I'm very pleased with it so far. They have two very nice dining facilities. The food is tasty, cooked well, and looked satisfactory.

This is a very nice memory care facility. The staff is caring and considerate to the need of both resident and family. The staff pulls together and works as a team. I recommend u see the place before looking at any reviews that seems negative. When dealing with memory care residents u have to have a special heart. We also need to understand that there are multiple residents and they all have needs. Also if people are leaving has any one ever thought that it's staff that needed to leave. They are all wonderful people and they work together

My mother is there. It was great in the beginning. Since my mother moved in late April 2 activity directors have quit, a number of aids have resigned as well as nurses. They have been without activity director since July. The rooms have been dirty and the aids and nurses seem to have difficulty handling the residents with memory care issues and become short with them, correct them and tell them they are lying. I would strong recommend calling the Ombudsman and Health Dept to see what calls they have received before making your decision.

I visited this facility

Symphony at Centerville was brand new. The people were really great. I had a nice tour of the place, and the director and the person in charge of marketing were very informative. The rooms were a good size, spacious, open, and had a lot of windows. It had an outside garden area which was really nice, and it's all on one level. The day that I was there, they were taking their residents to an outing in Cincinnati on a bus. The director was excellent and definitely knew her place.
