I visited this facility

I toured Zionsville Meadows. It's an older and smaller facility. For their size and everything else, they seemed to be a pretty good facility. It seemed well kept. We only met a couple of staff, and they were friendly. The rooms were older, but they were clean and neat. They just didn't have the size that we were looking for.

I visited this facility

I didn't like that Zionsville Meadows had independent cottages that weren't well maintained. Their activity center was old and smelled musty. The landscaping was in great need of maintenance, but the lady that we talked to was very nice and very knowledgeable.

I visited this facility

I love the Zionsville Meadows, but they just don't have attached garages. They have garages, and they charge extra for the garage. I like it. I like where it is at, and I like the cleanliness. The lady that gave me the tour, Lisa, is friendly. The community is well kept. They have a lot of amenities. Most of the ones I've toured give you one or two meal a day. They don't give any, but they've got the dining room and the business dinner. My only drawback with them is that the garage is not attached to the living unit. They don't give you free meals, so I'll probably cook myself. I'm renting a garden home, which is independent living. They're great. They're large enough for me. The room is alright. It's a garden home; it's not a room.

I visited this facility

Zionsville Meadows was half a mile from my house and I liked it. It had the little independent houses and cottages and it's a real close walk to the activity center. Everything was nice and the price was right. It was a good choice. We also got the assisted living tour. The activities sounded very adequate, which the staff showed me, and I even met the activities person. It was an older facility, but adequate.

I visited this facility

We chose Zionsville Meadows because they were less costly than others that offer almost identical amenities. We toured with Lisa, and she was a very good person. She was giving us information and showing us the different places. She showed us the showroom, which was already decorated. I think she basically sold the community very well. We went for the cottages, and they were amazingly spacious. It was clean, it was nice, and it was kind of country oriented. They have a lot of land around the facilities where you are able to get outside, and they have a walking trail close by. They have activities, they have a physical-therapy room, and they can offer exercises for people who would want them. They have an emergency system, even for the garden homes. If you have trouble, they could send nursing staff from the other areas to come and help you. They offer all levels of care, but not all in one building. They have assisted living facilities, and they also have memory care. They basically offer everything in this one complex. They have a recreation room, and they have a dining room. They own quite a bit of land in the area, so they are not sandwiched in between housing. It's kind of separated off by itself, and you'd have quite a large area. You could walk around and not run into anything else. It's not new, but it offers the same amenities. We were able to get two baths; one was a walk-in shower, and the other one has a tub. It has two bedrooms, a living room, a front porch, and a patio, and a washer and dryer come with each unit. It has a small kitchen but a full-sized refrigerator.

I am/was a resident of this facility

I am currently at Zionsville Meadows. It is okay. The food is bad; I buy my own food as much as possible. The staff seemed to be great, and they seem to genuinely want to give good quality of care. However, they have a lot of agency people that sometimes are not so dedicated. That is something that they really need to address because it is taking its toll on people. I have been in five different rooms. They quarantined me initially because they quarantine everybody. Then I was having symptoms and I got quarantined again. Now I am in a shared room: two people in a room and one bathroom. You get your own TV, and you can have a refrigerator. They are just starting to open back up since everything was shut down because of Covid. I know they have bingo now, and they have a room where they play cards and board games, but they literally just started this past week, so I am not 100% sure what the games are. Visitation just opened back up this week too, and it is 8am-8pm. If you have a private room, you can have a couple more people, but if you are sharing a room, you cannot have more than 2 people. Otherwise, you have to meet down at the chapel. Because we were in quarantine for so long, I could not really tell you a lot about it. I know how to get in. I know how to go back to the hallway where my room is, but I do not really know where anything else is. Physically, it is hard for me to get around. I have a walker, but it takes a lot because I have a congestive heart failure. I visited a lot in the courtyard, and it is really nice. They have blooming plants out there and a little gazebo. I can see it from outside our window. This part is set up around the courtyard and it is nice. For the rehab skilled nursing, they take Medicaid and Medicaid waiver, but for the assisted living it is private pay only.

There are issues with insects and cleanliness. There are issues with staff being on their cell phones rather than caring for residents. The State visits this facility many times a year on resident or staff complaints. If you care about your loved ones I would advise you to look elsewhere for care......believe me.

I am/was a resident of this facility

I like Zionsville Meadows very much, and I like most of the staff. They have a yard, landscaping, a fitness center, dining room, they have PT available, there's a Delay the Disease class on a week, and bus transportation. They recently got a different dietitian, but I have not eaten there since then. The food was bad before. The facility is pretty good, and they're doing a lot of remodeling and updating. I really like the gym there. There are more activities that could be done, and I would like it be more affordable.

I am a friend or relative of a current/past resident

Zionsville Meadows has a nursing home, assisted living and independent living. My first impression is that they do a good job with taking care of the patients but the company that's running the operation there has a few things that remains to be desired. The place itself is very organized and clean. You walk into a canopied entrance, and on to the left is the rehabilitation area and to the right is the cafeteria or dining area. Then the rest starts breaking off into separate sections of each patient's place. They have daily gaming activities, interaction time, movie times in the evenings, and excursions. They also shuttle the residents to different parts of the community if they want to go to specific stores or mall. My mother-in-law is there right now. In terms of pricing, it's actually being taken care of by the different medical programs that were available. It's going through different management changes, so there are problems. The actual amenities for the clients are fantastic, but their administrator staff needs work. They train their staff; the medical staff is wonderful. The food is pretty good.

Zionsville Meadows was an older, nice, and clean community. We didn't go there because it was more expensive, and the environment felt like a nursing home. The guy who gave me the tour was very nice and informative.

I visited this facility

Zionsville was very nice. The only problem was the distance; I would have to change my doctors and veterinarians. It was very similar to the place I would be going to, but I did not think the food was as good. They had bingo, a theater, a hair salon, and a library.

I am a friend or relative of a resident

Mom is currently at Zionsville Meadows. The place is good, and they take good care of her. I feel they could use more workers and they are understaffed, but they have been adequate at this point. The food seems fine. They do have interactive stuff and crafts. They also have people encouraging them to participate in their activities. They keep the residents occupied most of the time. The rooms are nice and spacious with good showers in them. Overall, they are OK other than they need more staff.

I visited this facility

I was treated very well at Zionsville Meadows. I was given a tour. They were very friendly. They have detached housing units for independent-living. It was away from everything, but I really liked it. It has older units, but the units are larger. The community is quite attractive, and is laid out more like a housing community rather than an institutionalized building.

Everything is great getting at Zionsville Meadows, but when she got in, they’re not fixing things as they said they would before she moved in. Despite that, I would still recommend this facility.

I visited this facility

Zionsville Meadows was older, so the rooms were pretty large. They were in the process of remodelling the common areas. They had a nice dining room. The staff was very nice. I’ve heard some negative things from some people who had relatives there.

I visited this facility

Zionville Meadows had very nice staff. I could tell that they were caring and seemed to be knowledgeable. However, it was an older location, and it really needed to be updated. I was sure they tried to make it as pleasant as possible, but it just didn’t seem to have newer fixtures and needed repainting, as the doors were old and scratched. Some of the letters were coming off their sign. It definitely needed a little bit of care as far as the atmosphere goes. I thought the building needed to be updated and refurbished. I didn't eat there, but I heard a gentleman asking what was on the menu for lunch, and it didn't smell very good to me. It was chicken liver, which didn't sound too appetizing. I'm sure it was nutritious, and maybe a lot of the older residents there would have enjoyed that just fine. It just that they day I was there it didn't sound too appealing. I think they had different kinds of activities, like bingo and exercising. They also had birthday parties for the residents, looked at nature, and fed the birds.

Zionsville is less than a mile from my house so I moved mom there. I think the staff is okay, some are wonderful and I think they’re attendant to her needs. The building has a big expensive lawn near it. Mother’s room overlooked a little sitting garden. I checked on the food and she’s given three hot meals a day and she does have her choice of two main dishes every meal. She likes the food. They do have plenty of opportunities for snacks. They have activities and they keep her very much engaged. They have bus trips, Bingo, crafts and a little cooking class. They have exercise three times a week. For improvement, they can use a fresh paint, new carpeting and wall paper. They are doing a nice job but I think the building is a little dated and needs to be upgraded.

ZIonsville Meadows was significantly understaffed. The CNA turnover was enormous, and the LPNs and RNs were overworked. Hiring high-school dropouts with a few weeks of training to care for our helpless elderly isn't a good idea, and the constant warnings about theft and infection were evidence of that. I witnessed law enforcement investigators following up on drug thefts while I was there, and the place constantly stank of urine. The restrooms were not kept clean - dried feces were found on the toilet in the room my grandmother was using. The RNs tried hard, and I have to give them credit, but the place was poorly staffed and poorly run. The only bright spot was the physical therapists. For a place where I and my family were the customers, I felt like I was treated as more of an annoyance. I shouldn't have had to hover about to make sure that things were getting done. My experience there made me rethink the whole idea of having nursing homes be a for-profit business.

I am a friend or relative of resident

I was so excited when I first visited this facility, I thought I had found the best place for my grandpa. I now am ridden with guilt because he is back at the hospital after being abused for past month. I would not recommend this facility to my worst enemy. They are understaffed, the nurse practioner is inexperienced, and the PT and OT are neglectful. My grandpa both fell out of bed and was let to fall out of wheelchair on concrete when taken to doctor...and it was not documented or reported!
