I am a friend or relative of a current/past resident

Before Covid, my dad and sister kept my grandma's room clean and free of bugs. As of March, when family could no longer go in, her room became roach infested. We moved her out of the facility and went into her room to empty it out and there were roaches everywhere. Her room had not been cleaned. Anytime we tried to call and ask the head of the place questions about things that had happened her response was, "well, I wasn't there this weekend." I am pretty sure if you are in charge of a facility you better know what is going on at all times. She fell and was left on the floor for a long time. They did not notify the family that she fell, we found out from my grandma. We put her here three years ago because of the cost and it seemed like a decent place but it changed management and since then went down hill. I do not recommend anyone put there loved one here!

We chose here for my father more because of the cost. The people seem to care about the residents, but they don't do a whole lot activity-wise. He's been diagnosed with dementia, and they take him out to the common area, and they play bingo and do some word games. He's in the locked side. The menu looked like school lunches. It had meat, vegetables, lasagna, salad, and pancakes, sausage and eggs for breakfast. They do three meals and one or two snacks a day. The people are extremely nice. I just wish the facilities are a little nicer. They provide the bed, dresser, chair, and pretty much everything that he needs. It's not a bad building, but it's not as attractive as the others. It doesn't smell bad, and they seem to keep it fairly clean. He's in diapers now, and they're keeping him clean. We've been very pleased and happy with what we've seen.

My aunt was put here. The place smells, the staff was rude to me, my grandfather and the residents of the facility.

I visited this facility

My mother is in here, the facilities are nice, and the food is excellent, but they do not have enough qualified people to care for the residents. It had a real nice entrance and living area, and it had a pretty cafeteria. She shared a room with another lady. They took care of the laundry, and they had TV at both ends of the facility, but I didn't see a whole lot of other activities.

My aunt is in here. She is doing good. The staff is pretty good. They’re doing an excellent job. My aunt has her own room. It’s a private room. It’s very clean, and they’re very courteous. They have done an excellent job. We visited five communities, but this is the only that meets my aunt’s criteria and offers private rooms.

I visited this facility

My father was here, and it was not good at all. The staff was not watching their residents at all. Every time I go there, they were sitting and watching TV. My dad fell three times, and he was only there for three weeks. The place itself was nice but not the care.

I visit my 91 y.o. Grandmother there. She is always clean & well cared for. She and the other residents seem happy and the staff is friendly and always willing to be of service. They do a nice job of trying to provide a variety of activities for the residents. My Uncle seems very pleased with how his Mother is being cared for.

The man who toured us around Southern Oaks is very knowledgeable. They do have a calendar with activities on it. I would recommend this place.

I visited this facility

Residents are not cared for; ones needing assistance are housed in unit without assistance. Food is very limited. Two family members visited and were very disappointed. Manager is rude.

I am a friend or relative of resident

I have been coming to this facility as a visitor for over five years. THEN: It was affordable, like a big family atmosphere with good food and fun; an older facility but nicely maintained and (mostly) good caregivers. NOW: Recently changed ownership. Building maintenance, condition, staff morale, staff/resident ratio, food, resident socialization, all is steadily declining. It appears that staffing ratio has been thinned or cut back, affecting resident care. The administration has turned over twice in the last 24 months and many of the staff resigned. Turnover is non-stop and training/professionalism are non-existant. Building maintenance on the inside is sorely lacking and an “accident waiting to happen.” They rarely (I've never seen them) go on outings. The residents that are facility-dependent for outings rarely leave. If your family member is in one of the locked memory care units, they rarely, if ever, are taken outside for fresh air and sunlight. Please tour this facility with both eyes open. Many of the residents lose their “light” after being moved here. I have watched it happen. DHEC rules are loose and vague and apply to a facility overall, not a particular unit or area so don't take comfort there.

I visited this facility

I was visiting this facility with a friend of mine and while we were there visiting, we heard yelling going on in a office in front of the dinning room and heard a oriental woman yelling at a worker and found out she was the resident care coordinator. We thought that this was very unprofessional behavior for a person in that position. We had asked her if she could calm down, and told us to mind our own business. Since that time my friend moved his mother out of that facility and placed her somewhere else. For anyone looking to move their family member in this facility, I would not suggest this facility to anyone. We Later learned that the new Management that took over raised the prices and are not friendly at all. This facility needs to be closed.

I am a friend or relative of resident

I visited this facility with a freind of mine when he visited his mother and I was very impressed with the way the care givers interacted with everyone. When he went to pay his mothers rent to the office I met the management and they were very freindly and invited me to have lunch with him and his mother and the food was very good. I could not beleive just being their for a couple of hours with them the warm and freindly feeling I felt and how peacefull the facility is. I even had a change to talk with the Business Manager about my dad getting ready to need a place and that he could be eligible for VA benifits since he is a vet. I have been in other assisted liing and I wish People would understand that fancy and pretty is not always good. The care is the most important thing that should matter. I highly recomment this facilty.

a friend of mines grandfather lived at southern oaks but since passed while there he was taken very good care of and if not for the business manager wouldnt of got his va benifits this is a very good home and would highly recommend this assisted living the staff are very friendly and its a great atmosphere

2 years ago when my mother became a resident, southern oaks was so much nicer then it is now. Their prices were so affordable and the staff nice. Unfortunately the atmosphere has very much disintegrated and mom's rate was raised 4 times.The food is not very good .There are some wonderful care givers there, but also quite a bit of Drama due to nepotism. The residents are visibly bored. So sad, it was such a neat place in years past! The dining room is dreary and the employees eat there. The overall appearance is outdated.
