My sister in law was here for 2 weeks in Nov 2023 room cleaned once she push the nurse button someone might show up 3 hours later place stink bad down one hallway I wouldn't recommend any one go here only rated 1 star cause it makes you just to post the review

I do not even want to give this place 1 star. My friend was in the Rehabilitation side of this place. They had 1 CNA for 12 patients. My friend could not get to the bathroom on her own and by the time they got to her she had already wet herself. I just found out that they were not even giving her her meds right. When she left, they did not send her meds with her. They said we could pick it up and take it back to Maricopa. If you need help this is not the place. I am surprised that anyone gets better there.

My rated is limited to my phone conversations with staff since I live out of the state but my sister was there for about a month. My conversations with the wound care nurse and other nurses indicated that she continued to improve in her wound healing which was the primary reason for her care there. She was given the necessary medical equipment, special mattress and regular wound cleaning. I also had many productive conversations with Theresa the case manager about the discharge plan which enabled my sister to go home with all the necessary medical equipment and home care she was entitled to with her insurance. I appreciated the prompt return of phone calls and the detailed information provided as well as listening to my suggestions on what she could use at home

This place is absolutely disgusting. My grandma had to stay here for about a month. She fell and broke her hip, and refused surgery. She was a frail, 98 year old woman. Chandler Post Acute did NOT take care of her. Someone from our family visited daily to check on her. She was not clean, fell multiple times here because she was not monitored, the worthless people who “cared” for her lost her dentures, I had to fight to get the place to replace them and they never did replace them before she died. They tried to transport her to a doctors appointment against our orders. I had to call and force the van to come back. When she arrived, I found her upright in a wheelchair, screaming and dry heaving from the pain. This place needs to be shut down and every high level staff member needs to lose their ability to work in a caregiving position.

A friend's spouse is a patient in for post stroke care. Upon visiting, he is in soiled bedding....has food on him from a previous meal... and has fallen out of bed before. If this place is understaffed, then they need to get their sh*t together. If he is being neglected, then other patients are as well. I would strongly advise AGAINST leaving a loved one here for any kind of care. If you have someone in this facility, plan on doing their care yourself.

ATTENTION! If you have a family member at this location and you care about their health and well-being please go check on them! This place is under staffed and leaves residents and patients laying in their own waste. Tips if you must use this facility: Do not allow your loved one to be dropped off on a Friday for rehabilitation; P.T. will not work with them over the weekend which can potentially cause a health set back. Check on them daily. If bedridden plan on changing and cleaning them yourself. File complaints online to protect your loved one(s).

My mom was admitted into rehab, the room she was admitted to was a shared room, the wall had damaged dry wall, the visiting chair was so dirty I would not sit in it. The cleanliness of her room over all was very discouraging. Their lack of communication when it came to lock down due to COVID 19 case (s) went to visit and was told they were locked down. They never communicated when the lock down was over. When my mom was released she smelled, and was dirty.. (this was the second time we noticed her hygiene was not being cared for) I did call and voiced my concern with the case worker. Who was going to contact nursing staff... Medicare really needs to take a look at this place... I would not recommend this place. I cannot rate on activities...

My dad begged us to get him out of there and we did on the 2nd visit when he told us more horrible things he was enduring there. 1. Rooms are not private and he was rooming with someone immobile. Feces everywhere. He wasnt sure if the roomate was sick or what, but said poop was all over the bed and floor and this happened several times a day. 2. They have no bedside manner do not explain medication and dont care that my cancer ridden father denied his cancer meds... They said they cant force someome to take it. He just wanted to know what he was taking! We got a lost went over it with him and made sure he had his cancer meds first day. Good thing we visited... 3. When givimg medicine he asked for more than a 2oz cup of water for the 8 pills. Nurse told him to take them all at once. He had to ask again for more water. 4. He was only there for PT to strengthen his muscles. Hospital recommended acute rehabilitation because he would get hours of therapy a day. First day was a quick eval. I then called and talked to physical therapist who told me he would get out in 4 days. I asked him whether he was scheduled for therapy over the weekend and he said no. I complained and they said theyd put him on which we found out was an additional 10min each day. 5. Food was always cold. He said it was inedible. We were told he ate 75 pct of his meal. As soon as we said he never eats that much ever it changed to 50 to 75pct. 6. During covid u have to schedule visits. Good luck getting in! I think this works to their adavnatge keeping patients there. Our next visit we signed papers to take him home early. 7. Anytime u want an update you habe to ask, wait til your sick of waiting, ask again and eventually someone shows. Average of 45 min each time. 8. They never asked my dad if he was in pain. Ever! They dont care and he knew it. He had just been admitted to the ER due to an enlarged cancerous liver. They should have been monitoring his pain and giving him Tylenol or other pain meds he was prescribed. They told us they only gibe if asked. They are not around to ask!!!

My grandmother was admitted their covid free for rehab. She got Covid from this facility. My parents had to ask to transfer her to a hospital for better care. My grandmother died today because of this place!!!! DON’T GO THERE!!!!

Post acute care?? Hardly. We were very disappointed in most aspects of care. Staff are not cross trained, short staffed... food was horrible... cold and unappealing. Patient was mostly bed ridden so unable to assess the activities provided. Saw very few patients participating in the day rooms.. The main interactions were with physical therapy.They did what they could with limited space and staff. Family felt they must be there every day in order to be sure of quality care. Would not recommend.

The nurses refused to call 911 when my mother was having a serious stroke. My mother could have died. All my other complaints aren't nearly as serious. Dirty bathrooms, mildew in showers, grumpy overworked nurses. Still waiting to hear back about items missing when they packed my moms stuff up. If your loved one can't care for themselves, I wouldn't recommend Chandler Post Acute.

Nasty, disgusting, and gross! We took my mother-in-law (82) here after being at Chandler Regional Hospital, but after 3 1/2 days, we pulled her out! The facility is old. The windows are dirty and there is terrible stench of urine throughout the building. The bed, television, and equipment are old and falling apart. It's filthy in there! The staff members are nice people, but the few that are there are very exhausted and spread very thin. My mother-in-law needed could not get to the bathroom on her own, yet they wouldn't check on her and would rarely help her. Physical therapy (or "rehabilitation") just doesn't happen. We rarely saw the therapy room being utilized and she only received it once in the few days she was there. The food was disgusting! After witnessing the first couple of meals, we began bringing her meals from home or elsewhere. The final straw was when my husband stopped by only to find her colostomy bag about to burst. He called for assistance, but nobody came. After another 20 minutes, he went for help again and insisted someone come immediately. By the time they got back to her room, it was too late. The caretakers then gave his mom her first shower since being there. Once they were done, he packed her bag and wheeled her out. We never heard from anyone. Nobody called to ask 'what happened' or 'why" he had removed his mom from the facility; however, 2-3 days after he pulled her out, he did get a call from the admissions who was calling to check her in. LOL! She had no idea his mom had even left. What a racquet! I would not wish a stay in the place on ANYONE! I cannot believe this is legal! I honestly cannot imagine anyplace worse! Stay away or you'll be sorry!!!

Absolutely awful staff, communication, facility overall. I literally cannot stress enough how defeated I feel after dealing with this facility for 2 weeks. Please be very cautious and careful if you decide this is the place for you or someone you love. It is beautiful from the outside and you may even get a lovely tour of the recreational areas and fish tanks. Be warned, they will lie about what they can and cannot do. They will lie about treatment or care given, even down to meals. My mother had to ask several times for the other patient in her room to be given a meal and 3 hours later she was finally fed. I am heartbroken for people that have no voice or advocates.

My 86 yr old Mother was there recovering from a hip fracture. She was there for two weeks. She was never once seen by a fully licensed physician, but was seen twice by Physician's Assistants. Her repeated requests of pain in her foot were ignored, and she left the facility with a huge pressure sore on her foot that will require treatment at a wound center. She reported the facility was VERY understaffed at night and she was concerned more than once about the safety of the facility since she would be unable to rescue herself in an emergency. She reported long waits for pain medication and for being assisted to the bathroom. The wrong paperwork was turned in causing Medicare to cut her benefits off sooner than should have been. She then received an invoice from the Center for days she was not there. My mother reported that the majority of the staff were very nice and kind to her-- they were simply so understaffed they could not adequately care for her. Only one person working there was rude to her. The PT staff was good. Even though she should have stayed longer, she was relieved to leave. The lobby smells wonderful, but as soon as you walk around the corner to the patient hallways-- you can really smell the urine.

Not recommending this place to anyone. Nurses take 45 mins - 1 hour to get to you. It is very rare they come in 15 minutes. When you go ask for help, they give you nasty faces. So, be prepared to do almost everything for your family member. Just bring them to some other places.

NOT RECOMMEND for your family members. When you call for assistance, they will take 45 mins to an hour. Very rare that they come to you in less than 15 minutes. So, be prepared to do almost everything for your family members if you take them there. If you are not up for it, take them somewhere else.

Awful, awful, awful! This is where to go if you don't mind waiting 1-2 hours to use the bathroom or if pain meds due every 4 hours are given every 6-7. And on weekends it becomes a virtual prison. Given the lack of care we wre ready to bring my mom home and care for her ourselves, but since therapists, case workers and doctors don't work weekends there is no one to sign paperwork. Normally I would say who cares, but this also means no Rxt to take home, no help arranging for at home physical therapy, and it was hinted that it would cause a problem with Medicare. Again, I say who cares, we can work it out ourselves, but my overly polite mom was willing to let them have another chance. I hate that her health is in jeopardy because the facility is understaffed and the Dr. is unreachable outside of office hours.

My father was in this facility for 2 months. It was awful. I would have to wash him up, help him brush his teeth, wash his face. There were days that he wasn’t even offered his toothbrush to brush his teeth. They didn’t like giving him a fully fledged shower, they always wanted to give him a “bed bath” until I started insisting that he be bathed in the shower. Most of the time, I was changing his bed linens because if I didn’t, he would be sleeping on days old linens. He would push the call button and it would take forever for them to come to him. His medications were changed without notifying me. At one point, he had a catheter in and it was leaking. He was sitting in his wheel chair and there was a puddle under the chair. I immediately went down to the nursing station to alert them and the very first response from the charge nurse behind the desk was: “did you push the call button?” I couldn’t believe it. So, I went back to his room and pushed the call button. A while later a CNA came in and though she wasn’t assigned to take care of him, she did put a pile of wash cloths on the puddle to help to soak it up. When his regularly assigned CNA came in, I pointed the problem out to her. She said, “Oh, well, I guess I’d better call maintenance.” She then took my dad for his shower (after I insisted he be bathed in the shower) and I waited for maintenance to come to clean up the mess on the floor. They never did……I ended up cleaning it up, on my hands & knees myself. OSHA, where are you when we need you??? After I finished, I was sitting in his room waiting for him to come back from his shower. The charge nurse that was on duty happened to walk by and stopped and asked me if I was ok. I proceeded to tell her about all of this and her response to me was: “well, you know you can have home health care at home and he can be taken care of that way.” Among other things, the food was terrible.

My Mom was here after back surgery. I work down the road and would go to visit. They were never on top of the pain meds and always took at least 20mins to 50mins to respond to a light on. Most of the time I had to walk around to find help. I would listen to the people in the other rooms yelling about how know one responses to them when they need them. Everytime I would ask for help, they would have to go get some else. I appeared to me that it was not there job. The poor lady in another room was yelling for 10mins that she need to use to restroom. I ended up going to find her help. There was a lady standing 6ft from her door listening to her but that was not her to help her use the restroom. They left Mom in the back brace (she only wears when up right, and not for long periods of time) for 4 hrs. When she complained of pain they just kept saying the therapist would be in soon. Her back incesion got infected. My father started telling them he thought it looked infected on Sunday. I went in on Wed morning and started yelling to get something done about it. They said they would get the onsite Dr to come look at it. They should have done when my father was complaining. By 4pm Wed she was being transported to the hospital. She also had a bleeding ulcer and had been having issues for 2days. When dad questioned the staff about the black running stool, they said it was normal because they eat old people food. The Dr at the hospital couldn't believe they had ignored these things. This place is way under staffed, they are all too busy to help with anything, and I don't think they even know what they are suppose to be doing. We are now searching for a new place for my mother. The staff was nice, the place looks nice, but I would even have them watch my dog here while I went on trip. I feel bad for the residents here. This place needs new management.

I visited this facility

This is a large skilled,ltc, and rehab facility. It is very clean and does not smell inside. The building and grounds are well maintained. There are trees, shrubs, greenery, and flowers. There is a larhe patio and courtyard area. They have it divided into two stations. One LYC abd one skilled and rehab. Most rooms arensemi-private with a couple of private. They do laundry and housekeeping. They have three meals a day and a snack. The food is ok. They are on a rotating menu without lots of variety. They have a house doctor. There are Rnsand Lpns 24/7. All caregivers are licensed CNAS. They have a rehab team and activities director. There nis a beauty salon. They transport to appointments. The staff seems nice and caring. They do lots of activities as bingo, games, birthday parties, celebrations, exercises, music, and more. They go shopping and out to other events. This seems a good and clean facility to me.
