My grandmother was recently sent here for rehab. And what an absolutely horrible experience she had. I am appalled by the “care” they give to patients there and I say the term care lightly. My grandmother text me multiple times that she called for help to get cleaned up and no one came for hours or would go in and turn the light off and not do anything to help her. So many times she would text me and I would go up there and get her cleaned up and just about the time I was done here comes the CNA. The excuses I was given are that they are short staffed. Only to find the nurse at the end of the hall watching tv and not helping what so ever. This is completely unacceptable and I fear for the people that don’t have family that can come and help them. The DON at the facility was requested to come change my grandmothers PICC line dressing after it was found with a folded up piece of gauze over the top of the dressing that was no longer intact. Off the jump he broke his sterile field when his gloves broke. Do you think he went and got new gloves? Nope. Continued on changing the dressing. I spoke with the DON about the events of the prior night and was told that the door to the end of the hallway would be closed. Next day…..same exact thing. I had my grandmother moved to an inpatient rehab that day after being told by Taylor the social worker that “it could take several days” she was kindly informed that it would be happening today. And it did. This facility is an absolute disgrace and in my professional opinion needs a very thorough visit from the state. I’ve been a nurse for 18 years and have never seen the amount of neglect of patients than I saw at this facility. I wouldn’t send my dog here. This facility seems to be extremely poorly run and needs to be swept from administration down.

I'm going to start with if you have a loved one that was recommended to Green oaks nursing home and rehab don't place them there ,and if you have a loved one there get them out ASAP!!! I'm going to start with my story now I have a loved one that was recommended to Green oaks nursing home and rehab someone was with them during visiting hours and started noticing that they do not go by protocol they don't change their gloves after picking up trash cans and going back to our loved one when the IV is done they leave it hooked up to them when we call them out many times to unhook the IV because it can cause sepsis to a loved one, no phones in the rooms if your loved ones my loved one had his cell phone for a few days before they did a covid-19 lockdown when placed there he was in good condition try to call him for days and could not get a hold of him about 4 days after lockdown someone did get a hold of our loved one and was told get me the hell out of here I'm not good I'm bad 911 was called to remove our loved one from Green oaks nursing home and rehab and taking back to the hospital where he was really bad shape I could go on more but if what I've said about Green oaks nursing home and rehab does not stop you and you'll see for yourself.Please don't place your loved ones here the staff at Green oaks nursing and rehab come across as they are only there for a paycheck not cuz they love their jobs!!

My mom has been here for almost a week and we’re about to get her out ASAP. We never planned for her to be there long term, maybe a month, as we are moving out of state. We’ve been caring for my mom since December 2018 complications following a lung transplant. Over last couple of years she has spent 14 consecutive months in hospitals, we cared for her at home for almost a year and decided to put her in a facility because she needed so much and we were playing every role (nurse, tech, pt, ot, speech, house keeping). Our goal was for her to improve a little so home care would be possible. Unfortunately, that has not been the case. She declined a lot in the first facility so we just moved her to this one. She got there on Wednesday (should’ve had dialysis Thursday) and we had the Plan of care meeting (via the phone) on Friday. We made sure to go over everything she had going on and asked if they had dialysis setup. At first they had no idea. Which was concerning bc the meeting was about her so you should know. But they checked and said they had her for T/Th/Sat. The next day (Saturday) my brother goes to the dialysis center to lay eyes on her (we don’t get to see her often bc of covid restrictions) and she never showed. Called them and the nurse’s excuse was she only works weekends. Total bs. I don’t care if you work once a week. On that one day you should get caught up and see what’s going on. When she looked they had my mom scheduled for dialysis M/W/F and T/Th/Sat but she didn’t catch that or ask for clarification. They couldn’t get her to dialysis that day but said they would on Tuesday. We decided to call the dialysis center to see if she was there on Thursday and she wasn’t. So she has now missed 2 treatments. WTF?! We told them to send her to the hospital. Trying to call them is a gamble. Today I spent almost 30 minutes on hold before hanging up and my brother has experienced the same. During the plan of care meeting they seemed to be in a rush and not really trying to understand our concerns. We understand that it’s hit or miss with any skilled nursing facility and then COVID has made their work more complicated but ppl still need to do their job I don’t recommend sending your loved one here

The people were nice at Senior Care of Green Oaks. However, there was only one nurse to each pod and there was no aide, which is why people were sitting in their own urine. They were rolling around in wheelchairs with pee dripping down out of their seats. When you walk in the place there's no way to mask the smell of urine. It was so strong. I did a quick tour and then walked out. There was no way I would put my mother in there. They didn't have enough staff members to care for their patients. The only staff I dealt with was their marketing person. She was nice, but they're understaffed and the place was very dirty.

The staff here is totally professional, competent and attentive. I appreciate everything they provide for my parents on a daily basis. They respond in a timely way and they are sincere about helping my mom and dad. I am extremely impressed and grateful for the physical therapy they provide. An extra bonus is the food is delicious!

Our basic problem with Senior Care of Green Oaks was the response from the staff. My wife can't use the toilet, so she needed diaper change, and there wasn't enough staff to be responsive to her needs. The shared room she had was adequate. She had physical therapy. She had her meals in bed all the time, and the food wasn't bad. They had some good religious services, movies, and Bingo.
