I am a friend or relative of a current/past resident

I have been to visit my Father-In-law twice at this facility. The people are friendly and caring. My Father-In-Law is hard of hearing and slightly difficult and they have been doing a great job with him. Employees are everywhere. He presses the button for help and someone was there in a few minutes. My Mother went to another Rehab Facility in MA and it was the total opposite of this place. They were always "short staffed", when you press the help button they would not come for about 45 minutes. Life Care Center Of Acton is overall outstanding compared to other rehab facilities.

Let’s face it, no one enjoys watching their loved ones enter a rehab or life care facility. The story of my father is a simple one. He suffered a mild stroke which caused complete blindness in his left eye and later, partial blindness in his right. Before this issue, he was a very healthy 87 year-old. He cycled, when weather permitted, 25 miles/night up until the age of 77. He worked a full time job and still drove. Well, within a matter of seconds, the driving and working were gone and his life would be forever changed. He spent two weeks in Massachusetts General Hospital in Boston where he was diagnosed with a rare disease, GCA (Giant Cell Arteritis), which caused the apparent mini stroke. After completing several tests over a two week period, it was decided to transfer him to a rehab facility. We chose Life Care Center of Acton because it was closer to home and my elderly mother would be able to visit him. The day he left MGH, before being transported, he had a minor incident where he nearly fell when trying to make his way to the bathroom, but he quickly grabbed on to the side of his hospital bed to keep himself standing upright, but by doing so he hurt his wrist. We visited him the day he entered the Acton facility. He was roomed with a gentleman who was leaving the following morning. We were advised the that morning the roommate tested positive for Covid-19 so they immediately moved my father into a room with a single bed. Before doing so, my father had asked an employee, which he assumed to be a nurse, to please look at his wrist as he felt he may have sprained it the day before at MGH. The person sourly said, “I’m not touching you.” From this point forward, my fathers stay was nothing but an emotional rollercoaster ride. He was unable to get the PT that he was there for because he himself had contracted Covid-19. His days there were then limited to bed, and bed only. He complained to us daily, while on the phone or in person that he was not receiving the attention he needed, whether it be to get up and use the bathroom or simply walk around. Upon entering his 10th day in the facility he also mentioned he had not been bathed in 3 to 4 days. Frustrated, my sister contacted the head nurse of the facility by telephone and asked why our father had not been bathed, it was explained to her that he refused to be bathed. Because of this being an out right lie the decision was made to get him out of the facility ASAP. We were told they would not release him until he could produce a negative Covid-19 test. Two days later he was released back into our care and sent home. Upon his arrival home, we noticed he was severely dehydrated and weak, to the point he was not able to even standup to use the walker supplied by MGH. For the short period he was home it was nothing but a nightmare for all of us, especially him. At approximately the 30th hour of returning home he asked that we call 911 because he felt as though something was seriously wrong. He was correct! If we hadn’t had him rushed to the UMass Medical Hospital in Leominster when we did, he may have easily passed away. We were told by the physicians that he had contracted sepsis and his kidneys had shut down. The attending ER physician asked if we would consider authorizing a DNR (Do Not Resuscitate Order) to which we said no. When we asked what may have caused this to happen we were advised that it may have resulted from dirty conditions at the rehab facility, Covid-19 or even the possibility of crossing medications he was now taking. It maybe ironic, but all three of those things may have occurred while staying at the Life Care Center of Acton. Furthermore, he was still testing positive for Covid-19. Our father is on the mend but not without complications from this experience. We understand that he is 87 years of age and life can quickly change but we also knew how his condition was when entering the facility versus the time of his dismissal. We would NOT recommend this facility to anyone at anytime!

I visited this facility

This place is horrible. My relative stayed there and she could not walk to the bathroom due a fall. She would have to go to the bathroom in a bedpan and be dirty for an hour until they finally picked it up. They ignore patients. If the staff does something wrong they blame the patient or relatives and lie about what happened to cover up incidents. This happened multiple times. Managers try to twist your words on complaints, so they look OK and do not have file a report. Everything is a “misunderstanding”. The Food is horrible. TVs are broken. Bad care and not trustworthy. Everything there is of the lowest quality: furniture, comforters, rooms. Don’t take your relatives there. They deserve better

After taking care of my mother at home for the last decade, My wife and I came to the difficult realization that it was no longer safe for her at our home. After direct experience with 6 different PT facilities, and researching/visiting 18 different nursing homes, I came across LCC of Acton and paid them a surprise visit one rainy afternoon. I was greeted by the director of the facility who took me on a tour, and was the first person in my long tour of nursing homes to ask me what my mother wanted and needed, as well as, what she enjoyed doing, rather than just focus on her finances. Some places won't even give you a tour unless they have your loved one's financial info ahead of time, here they care about the person first. My mother has been here now for over 6 months and has really come alive and out of her shell again. They matched her to a great roommate, who has become her new best friend, and they are almost inseparable. My mother receives great care, her room and the facility are always super clean, and there is always something going on there due to a great activities director and staff. The menu here has a huge amount of variety, with snack and treats throughout the day, my mother is always raving about how great the food is here. The people here really due care about your loved one, I have been to the facility at all hours and am always greeted by a smile and friendly faces. The people here truly care about my mother, and now feel more like old friends and family. I now sleep good at night knowing my mother has a safe loving home at LLC of Acton.

I am/was a resident of this facility

I was at Life Care for rehab of a total knee replacement. The normal length of rehab is 7-10 days. I was there 22 days. From the moment the ambulance brought me to Life Care of Acton, I wanted to leave. The facility was dirty and smelled. There was dirty laundry in the hallway. The halls were filled with patients yelling and crying. They mixed the rehab patients with the nursing home residents They insisted on tests that I didn’t need. When ever I needed assistance to go to the bathroom,it could be up to 45 minutes before an aid appeared. Do not go there for rehabilitation. There is very little physical therapy

I visited this facility

Staff here are very nice but the overwhelming smell of urine in the air is appalling. I don’t know how the staff deals with it all day, or perhaps they have become desensitized to it. PLEASE Lifecare Center - fix your ventilation issues!

This facility is horrible-doesn't even deserve 1 star. My father was there for rehab and we couldn't get him out fast enough. The minute you walk by the front desk it smells awful. The Ex. Director doesn't care about anything so therefore no one else cares. As my father says every meal was a "mystery meal" except for breakfast because it was the same bacon and eggs everyday. Staff just "yes" you to death and never follow-through. Think before putting a loved one in this facility.

if you wish to have your loved one in a diaper for 12 hours and smell like a sewer, this is a place for you. if the patient cannot talk or ring for the nurse, they are left there until family comes and says something, my dog is treated better than this at the animal hospital. they should examine their conscience. do a complete search before placing your family member in this place.

My dad is a new resident of Lifecare in Acton Ma---so far so wonderful! The staff is so helpful and I am always sure that while we are not with my dad he is well taken care of The communication between the staff to each shift is top notch. I can go anytime of the day and his nurse knows exactly what is going on at the moment OR during the previous shift.

I will never again leave a loved one at this facility. I left explicit directions to be notified when my loved one had a change in her condition so I would be with her when she passed. 24/7 phone numbers were in her file and I saw her every day. I was with her the morning of her passing and was called later that evening after she passed. I wanted to be there and hold her hand and help her to cross over. Life care of Action robbed both if us of that most precious few moments we had left. My heart will forever hurt because I left her in their care.

My aunt has been a patient at Life Care Center of Acton for about three years. Once her husband passed away, she gave up in general on caring about daily living on her own and went down hill to the point that a nursing home was required. She spends most of her day in bed or in a reclining wheelchair, she feeds herself only scantily, communicates with visiting loved ones occasionally and with difficulty. She does not appear to be able to walk or even stand up. In spite of all this, her grooming, dress, and hygiene have not deteriorated. The general appearance and hygiene at the facility are very good. The staff seems to give her personalized, friendly attention when we visit and seems to respect our requests when we visit. Relative to other nursing homes I have visited, Life Care Center of Acton is superior.

I am a friend or relative of a resident

I was looking for a place for my husband, but he passed away. He was in a hospice care for maybe a couple of weeks before he passed. The place is Life Care Center of Acton. In my opinion, it is an excellent facility. He was in a rehabilitation wing because when he went from the hospital, he actually made some progress as far as physical ability is concerned. He was working with a physical therapist, and he really did fairly well. Unfortunately, he chose to get up in the night to get to the bathroom and slipped and fell causing him to break his femur. The good news was they were able to screw the bones back together. I don't know what he wants, but he stopped drinking and stopped taking medication, so he proceeded to go downhill, but he was efficiently cared for by the personnel of Life Care the people at the hospice. They couldn't have been nicer. They are absolutely wonderful. They did everything they possibly could for my husband.

The staff and facility were lovely. My family member greatly enjoyed the weekly piano player who came in to play American music. The only complaint was that she was placed with an increasingly disoriented roommate who would frequently have episodes of paranoia which were incredibly disruptive and frightening to my family member. We requested a room change and the suggestion was met with a lot of resistance.
