Use a Written Record to Assess Your Parent's Dementia

If you suspect your parent has Alzheimer's, start an assessment diary. Privately track changes you notice, situations that trigger problems or upset, your parent's reactions to such situations, and how you dealt with them. This information can help you and your parent's doctor, if and when you consult one, to evaluate your parent's condition.

Make this assessment diary as detailed as possible, noting not only the changes that occur but also the time of day, duration, etc. The more information you provide your loved one's healthcare provider, the better.
