How do I handle Dad refusing an eye exam?

How do I convince my Father to get an eye examination? He has not had one in over two years, and believes that if he goes in for an eye examination that his driver's license will be pulled. He and my mother live in a senior center which feeds them twice a day, and have their own apartment. His license probably should be pulled, but he went through the process at DMV, and I believe they allowed him to keep his license - he says he never got a final determination from DMV, which sounds unusual. My siblings and I all are concerned about both his vision and his driving. Any ideas?
