How do I find nursing home care for a 55-year-old?

My brother had been staying in a nursing home. He just had another stroke and we would like to find a better place for him that would meet his needs. My brother also has diabetes and his first stroke left him paralyzed on his left side. He also needs a wheelchair. We do not know the extent of the damage with this recent stroke but so far he can not speak. My brother is only 55-years-old and I can only find nursing homes for elderly people. Can you assist in providing options or places we can contact?
