Does Medicaid transfer from state to state?

If someone has Medicaid and formerly lived in Michigan, is it valid in another state? Are there any resources for more help on getting assisted living for an elderly person exhibiting signs of dementia and or schizophrenia? She will not seek medical care or allow family members to help her.
