Where can I get help me move into low income apartment or house?

Who can I get or where should I go to see about moving into a low income apartment or house or is there agencies that can help me repair my home to an adequate living place/ I have no heat or air inmy home but a couiple of space heaters and only ceiling fans for cooling, I live in a double-wide trailer and the floors are falling in, the roof leaks, and my kitchen ceiing id fallin in from th eleaks, the front outside wall of my trailer is rotting as well, I have repaired most of what i could on the floors with subflooring but i still have one room to go, however, now I live on a set income each month and can not afford to buy the supplies or pay anyone to help me, I need help for I am unable to do these things by myself due to arthritis and asthma, I need a ramp out in front of my house also for the steps are very steep and hard for me to manage walking up and down them. I have fallen three times in my tub and luckily only got a bruiss so far. i live alone and stay alone, for everyone around me uses me one way or the other so I stay alone.
