How can I ease the transition of moving?

I think it might be time to place my 86-year-old mother who has Alzheimer's in a Alzheimer's care facility. I'm to the point that I'm not sure I can mentally handle her care anymore. She was diagnosed about five years ago. She was managing fairly well, until my father because ill and passed away. During his illness, I know she didn't take her medication as she should have. That combined with the stress of his illness, followed by grief at his death, I believe, have contributed to the advancement of the disease as well as just the passage of time. I currently have her in a day program four days a week in hopes of getting her used to being with other people. I have been her sole caregiver, 24/7, for over a year and a half. She has become very attached to me, and frankly, I'm not helping her very much. During any given day, she spend most of the time sitting in her chair. I'm not a natural caregiver, so I was happy to find the day program where she is safe, well care for and receives stimulation. I am in the process of finding placement for her, hopefully at the same place where she is currently attending the day program. This is going to be the hardest thing I have ever had to do and I know it will be a hard adjustment for her. She has lived her whole life in the same town, the last 60 years in the same house. It is her comfort zone. I know it will take some time for her to adjust to a new living situation. My question is about how to make the transition easier. I understand about moving some of her things there, etc., but what about visiting her. Should you visit every day or every other day? What type of family visitation schedule would be best during that adjustment period?
