Can my grandmother sell home to me while she's in nursing home and on Medicaid?

My grandmother recently went into a nursing home. Right now insurance is paying for her care. However, in the near future she will have to fill the medicade paperwork out and medicade will take over. She will never be able to return to her house and I am in the market to buy a house. The plan is for me to purchase her house at fair market value. However, I can not afford the closing costs and downpayment. Would medicade have an issue with her selling it to me with sellers consent which would cover the closing costs and downpayment?
