How can Mom apply for Medicaid post-stroke?

I don't have power of attorney in order to gain access to my mother's financial records for her Medicaid application. Unfortunately this wasn't arranged before my mother had a stroke, which left her paralyzed on her right side (she's right-handed) with aphasia. What do I do if the documents I supply are incomplete and does this impair my mom's chances of qualifying for aid?
