What happens if someone has no one to care for her and no money to pay for care?

A good friend of ours (age 49) just suffered a major stroke 4 nights ago. She is in the hospital and seems to have lost her verbal skills and use of her right leg and arm. She has been unemployed and on Medical for the past year. She can barely afford her rent, and has no assets. She is not married, has no children, and does not have any family members who can take care of her. When and if she makes it out of the hospital, what type of care and housing would be available for her?
