patients in high care left unattended for hours, with no supervision and staff dont care. they just look the other way. family members have to go there themselves and care for their loveo nes themselves. DO NOT GO THERE UNLESS YOU WANT THEM TO DIE WITH NO DIGNITY

My husband was at Cold Spring Hills Center for Nursing & Rehabilitation. They were kind, accommodating, and efficient. It was during COVID, so it wasn't an easy in-person contact and visit, but they had good facilities and they were responsive to me. I would still give them a positive review. I was pleased with the people that I met and talked to. Their price was as much as a standard price and they worked with me a bit. So, they were very considerate.

My mom has moved into Cold Spring Hills Center for Nursing & Rehabilitation. She is there for long-term skilled nursing care. It's very nice and very professional. The staff are very thorough. They communicate well; I hear from them regularly. They were all very professional. They have a rehab facility, and they have skilled nursing. The social worker that I'm dealing with now is very good and very nice. I like the location. It's convenient for our family. Everybody is very helpful and professional. They communicate well about my mom's care.

Cold Spring Hills is a very, very big facility. I go up there to visit my wife and I see the way things are run. The people on the floor where she is don't respond to anything. The people there throw things on the floor, but they don't clean up so fast. They leave it as is, then eventually get to it. It's just not good. They have over 500 people for five months, and they're understaffed, as usual, so that probably adds to it. Sometimes the meals are decent, and sometimes are not. They offer activities, but my wife is high-functioning, so she doesn't want to participate in any.

The staff is friendly, caring and attentive with my mother. They ask questions and listen to your suggestions. She is very happy there. The rooms are plain, But safe. Some residents have decorated and added they personal touches in decoration.

Awful place. Residents sitting in hallways with no attention from staff calling for help. Prescriptions not given on time. The majority of staff could not be bothered. Including the supervisors. Front desk were nice but that is where it ends. Do not send your loved ones to this place.

My father moved into Cold Spring Hills Center for Nursing & Rehabilitation. The staff there is very friendly, and somewhat helpful. The place is clean, and from what I understand, the food is not great. From what I understand, they do have activities, but my father's not participating in them. The place is kept clean, but it's not the prettiest of buildings, it's old. The dining area is just plain. The last two times that I've been there, it's been a little messy. The dining area is not inviting to go sit in. The place is 20 minutes from my house, and the grounds are big. They're well-kept and groomed.

I spoke with my father yesterday as he areived at this awful facility for rehab after being discharged from the hospital. He was aware, energetic and coherent. Not even 24 hours later after multiple phone calls where the inept staff hung up in many case in mid sentence I was told my father spoke to a family member stating I’m in trouble and i need help. Overnight from aware to incoherent, i arrived at the facility demanding to see my father. After about 30 mins of run around I was told he was in one on one, then was told sleeping. Again demanding to see him I had them bring him to me in a wheelchair. He was incoherent and struggling to speak could not open his eyes. Immediately I asked, he was just sleeping you didnt give him anything? They then told me he was given halodol which should not be given to anyone with heart condition. This facility is disgusting, has an onvious feval odor and inadequate staff who obviously are not concerned with the health of your loved ones. This place should absolutley not be allowed to operate, they have no interest in care and will drug your loved ones as they see fit to not have to be bothered with them. Absolute garbage there is no rating low enough based on my observations.

Avoid this place. My father was here after he had a stroke. This place basically gave up on care and neglected his health. He passed away after I strongly advocated that he needed to go to a hospital because of difficulty breathing. Honestly I have never seen a facility neglect any patients like this rather then send him to the hospital at the start of the problem. Furthermore, my father was a one to one for eating and I often found they just left the food tray with him and just walked away.

STAY AWAY! Help close the facility or force them to hire better staff. Report all cases of negligence to the state. If you’re not sure if it’s negligence, still report it. Cold Spring Hills rehab is a community filled with unprofessional leadership. The staff is negligent, when calling a Director, you are lucky to get a voicemail box that is not full. I was told by the nursing director/supervisor Nancy, that I never called her that I am lying. Nancy makes up excuses for the staff and herself. I wish I were just an angry daughter that’s overreacting but I was told by other staff members that I am not “making enough noise”. On multiple phone conversations with other department directors and staff they will throw the nursing team under the bus. One even gave me the contact information for nursing director boss and said please don’t tell anyone. With such poor leadership from Nancy, I understand why the nursing staff is lazy. I was given numbers/extensions from the staff to make complaints about the nursing team and their negligence. It makes you wonder ....if their own staff is ratting the team members out what is going on there? The director of PT Michael, discontinued my fathers PT services without communicating to us that this happened. I had to call and question why is his insurance is denying a claim and will no longer pay for his rehab stay, Michael said my father shows no improvement. He has no knowledge of my fathers condition stated “ I am not medical I do not deal with diagnosis, I deal with what is presented to me”. My father needs PT to keep his joints and muscles from contracting, he says the nursing staff will be responsible for his range of motion and maintaining the integrity of his muscles. We do not have confidence in the nursing staff, they leave their patients to lay in pain with saliva and mucus building up in their mouths and trach. He then stated “ if you feel your fathers life is in danger, you can transfer him to another facility”. The Speech therapist, Roseann is an amazing person! She saved my father’s life! He was having an issue and she refused to leave his side until the nursing staff attended to him. Had she not been there and advocated for him I don’t know what would have happened. My father also has one of the sweetest recreational staff member by his side, Jen, I can not thank her enough for all her help! She FaceTime calls myself and my sister 2x week with my father. But what makes her an angel on earth is that she is his advocate, anything he needs while she is there she attends to. Had it not been for Jen I we would not have see our fathers condition. Every phone call my dad has an issue that nursing just does not address until we see him and ask for help. Covid has taken a toll on facilities, understandable. Having a lack of empathy, care and professionalism are not the characteristics needed in a leadership team. Blaming a patients current decline on their history without fully reading their chart and realizing your staff caused his decline is despicable. Please do not make the mistake we did, stay for away! Please advocate for you loved ones.

Rude and unhelpful Nurses and staff. My husband was there for three weeks - could never get nurse or any staff to assist him while on he toilet. I was 80 yrs. old and I had to clean him after his toilet. He became very ill, nobody cared -everybody too busy to come and look at him. His doctor said "if I hadn't moved him out of there that night, he would have died. He went into Septic shock and had 106 temperature, that same night. I thank God I moved him fast enough out of that awful place. I would not put an animal in Cold Spring Hills Rehab Center.

My mother had Alzheimer and often didn't understand what was being said. They asked her random questions and took whatever answer she gave with out verifying. After all meals the tables were never wiped down or the hands of the residents cleaned. Staff often sat in dinning room responsible for keeping an eye on the residents, but were often talking, on their phones, annoyed if they were asked for help. Never interacted with residents unless they had to. I observed them actually teasing several residents at different times. My main concern was that when ever my mother needed to have something done or a DR visit I, the Medical Proxy. was not notified. Even after complaining and letting them know on more than one occasion that it is against the law to not notify me! It still continued to happen. I would not recommend this place except for a last minute emergency option.

This facility should be closed and condemned! My family member was treated so badly. He could tell for an hour or 2 and none came. One night, we came in and his lower body was hanging off the bed! His foot developed Gangrine where there was none. As opposed to calling in a Podiatrist, they called a 'wound care specialist. Look that up and see the credentials they require, then decide if you would like them caring for your loved one with Gangrine. On night there was absolutely not one staff member on the floor. I had to call an ambulance because I opened the bandage on my brother's foot and saw all of the blackness! I called 911. I told the EMT's no one was there. They said 'there has to be someone. They walked the entire floor, returned, and said 'this is crazy! You better get your brother out of here' UNGODLY PLACE!

My Father, unfortunately, passed away at Cold Spring Hills Ctr For Nrsg & Rehab, Woodbury NY. The facility sent a sympathy card to my family, speaking about the wrong person... My Fathers name is not Kenneth.. Shame on the moron that did that... Also, the patients are drugged up to the point where they are sleeping all day and eventually kick the can because the staff doesn't want to be bothered. Half of the facility has dirty carpets that smell and the staff struggle w/speaking English. Please, do not do it to your family member, let them pass in peace in the comfort of their own home around the people they LOVE.

The rooms are dirty There are a precious few nice nurses and aides. The majority are not nice, ignore the patients and infantilize the patients. They put them in diapers instead of taking them to toilet. The food is unpalatable and looks like dog food. The leave the patients in wheelchairs in the courtyard and don't come back. I had to take my mother by ambulance out of the facility back to the hospital due to multiple pulmonary embolisms. NO doctor or PA on staff. Stay away!!!!!!

My husband fell and fractured his hip at our home last fall. I was scared for him and asked friends who recommended Cold Spring Hills Rehab for rehab after his hospital stay. He was admitted to norwich where we had a private room thats a plus for him because in the hospital he had a roommate made it impossible for my husband to rest comfortably. The nursing staff was caring and CNA's actually cared!! I'd like to thank so many people who helped my husband get back on his feet especially Beth, Stacy, Beatrice his Aid and Frank an exceptional therapist who made a not so pleasant situation an amazing experience.

Everyone was very good to my grandmother here. She’s been here for a while now and I can tell she feels very at home with the staff and other residents. We always have lunch or dinner together in the courtyard which my family and myself consider very beautiful. I definitely would suggest this place if you need to put someone you love into a nursing home.

Husband has been in sub acute rehab since July supposedly to get physical therapy for an amputation. Physical therapy not given anywhere near consistently or adequately to the point where he now has a contracture in the leg and can't get a prosthesis until they fix it. He was given a brace that they don't put on him and when they do its incorrect, medication wrong EVERY day to the point where there mistakes could have life threatening consequences, wound care not done correctly. Meals incorrect fluid left within reach that he can't have because he is on dialysis, personal belongings missing clothes, dentures, brace that they lost because they did not safeguard them and now won't replace them. Aides that could care less how they treat the patient they don't even listen to them when they tell them they are hurting them or being rough. No one at this facility cares all they tell you is that they are working on it and it never gets better. Best advice don't put your loved one here this place is pathetic and they don't care about making it any better. I don't know how they are allowed to accept patients without correcting these issues.

If you need respiratory care, this facility has a great respiratory team. My husband has pulmonary fibrosis and I have to say the respiratory team was excellent and the Director exceptional. They took good care of my husband and worked with us to get the equipment we would need at home. Also the physical/occupational therapists were so good ensuring that he was able to rest between sets etc since his condition was such that his oxygen would lower easily.

They dont give the patients any water here. In the afternoons after 4 when the next shift is there, patients are sitting in the main room in wheelchairs with 1 aide to 25 people. Some of the patients are not put to bed, they sleep in their wheelchairs. They have lost my father's clothing several times over. They do not assist with eating unless its lunch when many visitors are there. Do not put your loved one in this place. They dont change the pampers either. Sit in urine soaked diapers all day.

Do not send anyone you love here, ever. The care is sub standard, to say the least. From the doctors to the nurses, and aides have little to no knowledge about taking care of patients. The rooms are not clean and in the vented unit, there's carpet in the halls and lounge area...which is filthy. They can't diagnose any breathing illness, like pneumonia, or bronchitis....patients end up in the hospital over and over again. I can go on and on, but there's not enough time.

Keep your loved ones away my mom was there short term for shoulder therapy came out not walking, bed sores and they over medicated her. One word neglected. Now we have to pick up the pieces after all this. Unless you can be there 24/7 stay away total nightmare. Sad but true.

BEWARE - My mother was recently a patient in the Sea Cliff section at the rehab. Although there was a few nice and caring staff members, overall, the place is FAR from what I would call even good. I could NEVER get a hold of a nurse when I called the nurses station; it was always unkempt and dirty looking. The place needs a MAJOR renovation. I have heard some nightmare stories from other patients family members, and although I wouldn't say her experience was nightmarish, this really was FAR from myu choice of places for her, however due to there only being 4 rehabs on LI with a vent wing, I didn't have much choice. My mom contracted sepsis, and unfortunately passed in September. I will never know the whole story of what happened there. There was a woman there who was the who was cold, and whenever we would have family update meetings, I ALWAYS felt as if my sister and I were 'bothering' the staff with our questions. I once walked into my mother's room to find the ONE cup she was given to drink water was dirty and on the floor. So, please choose wisely

I want to give Cold Spring Hills a zero but then I can not post a review. This is an awful place. My mom was unfortunately here for two years. During her two year stay she was yelled at by aides, told many times there's no hot water today for a shower, left with uncut, dirty fingernails and uncut infected toenails. One night my mom needed help opening her soda and asked an aide. The aide replied that her manicure cost 60.00 and my mom should use a napkin. Another night I observed a young (late 20's) aide tell a patient she was lazy because she needed assistance in the bathroom. My mom was in the dining room one afternoon next to man who wanted badly to go back to his room. My mom mentioned this to an aide who in returned yelled about the work she was doing. My mom was scared and put her head down in shame. I would complain, talk to a supervisor but it meant nothing. Maybe one or two nurses cared and two or three aides cared. Everyone else did not care. I'm sorry are you not there to work - everytime my mom asked for something she was "bothering" someone. My mom was sick, it took a ridiculous amount of time before the staff did anything. They told me to calm down. They told me that her aide did not see a change in her and I (her daughter) was wrong. My poor mom needed an antibiotic and eventually needed an ambulance. They fought me every step of the way. I was asked why do you want her to go to the ER, what can the ER do that we can't do here. My mom was dying and died a few days later in the hospital. All they cared about was keeping her there for more money.

My 95 year old father has been a patient for the last 3 days since he was released from a hospital having sustained a heart attack. Today, at 2:00 p.m. I made his bed since he wanted to read. He had no water in his room which I requested from the nursing station. We also waited for an aide to help him into his bed. After 20 min., I got him into his bed. The place is beautiful, but I think they are understaffed.

I'm only using a 1 star because I need to in order to give a review. My father is a patient in room 107 in the SeaCliff building tonight on Christmas Eve my mom went to visit him and found him with his head completely leaning over his trach, the trach bleeding out, and his facial coloring was changing. She immediately asked for help and was told "oh you're crazy" "you are over reacting he's fine" "you didn't see what you think you saw". She started having a panic attack and called me from his room. I called and asked to speak to the nursing supervisor. I was transferred to Lizette. She laughed when I asked to speak to the last person who repositioned him, when I asked her to call me back and she didn't take my number I asked to give it to her and she refused to take it. She hasn't let me communicate my issues to her she keeps speaking over me and when I told her that I heard her out without speaking and would appreciate if she could return the favor she responded with "Michelle how much longer do you want me to speak to you? How much more are of my time are you going to take up tonight?" Here I am thinking that I would get some help or assistance and instead I'm being laughed at and made to feel as though speaking to me about my father is an inconvenience to them on Christmas Eve. My mother is still there with my daughter who might I add is barely a teenager and they are both crying in my father's room because the nurses are speaking so negatively and so loudly of me that it can be heard all the down the hall in my father's room. I do not understand why anyone would act in such a way especially a nursing supervisor. I do not know who else to reach out to tonight. It's Christmas Eve and my father is being mistreated and might have possibly died tonight if my mother didn't go. My mother is having a panic attack, my daughter is traumatized, and I'm sitting here helpless and heartbroken and full of anxiety because I do not know who else to speak to.

my sister was here for 4 wks - the level of care and medical attention was less than stellar (aka sucked). the communication/coordination between staff members was very poorly executed and were non informative to my sister & other family members. the food was below par. my overall evaluation of this facility is poor to very poor.. I wouldn't bring my pet here!! p.s. the pt and ot were very adequate.

My wife is in Cold Spring Hills Nursing Home (Cold Spring Hills Center for Nursing and Rehabilitation). It’s very nice inside. The staff members are very nice and helpful with feeding, which is mainly what I need. I think the food is very good.

My dad was there for 6 months and developed a stage 4 bedsore from neglect of turning him properly and a faulty air mattress. The nursing staff tries to attend to patients needs but the overall support is understaffed.The administration is far worse and the doctor mostly ignores you. My dads foot had lost blood circulation and we noticed 2 weeks ago but after mentioning it to him he said it was normal for him. After we demanded to have him taken to the hospital to be evaluated the vascular doctor said that it had been going on for 2 weeks and there was nothing he could do for it and the only option was to have the leg amputated or provide comfort for him. We did not want to put him through that and decided to make him as comfortable as possible. He passed away a few days ago and will be missed dearly. I wish I hadn't placed so much trust that the doctor was handling his foot properly and will have to live with that for the rest of my life.

My dad was at Cold Spring Hills Center. We weren’t very happy with them because he didn’t get good care there. At one time, he fell out of bed and was on the floor. He was able to pull the phone down and call me to tell me he was on the floor. I had to call the nurses station and tell them to go check on him. Then in the lunchroom, they left him sitting in a wheelchair staring at the wall. The only thing that was nice was the outside in the courtyard. That was a nice place to go out and sit, and they had music out there. However, the inside wasn’t that nice, and the elevators were terrible; there were only two of them, so you had to wait. The room and dining room were OK, nothing great. They didn’t take care of my dad. They need to improve on the care that they give the patients.

I visited this facility

The admission lady at Cold Springs was very outgoing. The staff all seemed concerned, but there wasn't any availability. They had activities, and the rooms were clean and nice. My mom would probably eventually have to go in to the lockdown dementia unit. She is mobile at this point but most of the people that we have seen there were not as mobile as her.

Horrible I would nvr send anyone here I pushed the help button 11/2 hours till anyone came that is after I called the front desk and she kept me company for over an hour. And ask every 15 minutes r they there yet.I was getting meds for pain all of a sudden middle of nite it changed it changed from 3 hour intervals to 6. I hv nasty nurse assistant who is unhappy that I got her up from her chair. This place gets away with stuff cause most patients here r quite old so they haven't a clue. Day ,3/was the 1st time I got p/t but that wound up being stopped than started again after some schedule confusion being Sunday before thankgiving. If I could gv it 0 stars I would

I visited this facility

Cold Spring Hills was very nice, and the care was very good. The room was of a nice size and was well kept. The staff was accommodating too. They can have some newer equipment because some of them were older, but it is a pleasant place to be.

At night the nurses and aides ignored my Mom's call light. She would need help to use the bathroom ,and her light could be on for two hours without them coming once. During the week days the staff seemed more attentive, but I feel horrible leaving my Mom there.

My father was a patient in this facility. He suffered a stroke in Conneticut and was transfered to this facility to be closer to our family. He was slowly getting better to the point where the doctors at this facility decided it was time to remove the breathing tube. That's a great thing, except they were not monitoring him and he DIED. SENSELESS!!!!! I would not put my dog in this facility to be cared for!!!!

My mother was a patient in this nursing home. She had alzheimers with a vengeance. I found the staff to always be patient and friendly. I went every day of the week to see her seven days a week. I visited my mom at different times of the day. Sometimes lunch and dinner or sometimes for breakfast and dinner. I always wanted to check on her condition. She had to wear Depends and I wanted to make sure that she was taken care of and clean. The nurses would always follow up with me to let me know if the doctor saw her. If I saw something that she needed (a podiatrist or a hair appointment), they would make the appointment for her. I made sure that her account had money in it and the nursing home never abused the account.I bought all of her personal possesions and clothing that she needed. I miss her very much. She passed away in that nursing home.I can honestly say that the nursing home was always kept immaculate and the home never had a smell.
