This is a very poorly run facility. The administration does not care about the residents. There is no communication with family and no care plan meetings. Turn over of staff is extremely high. State is there often doing investigations for neglect or medication errors. The place is filthy and smells terrible.

My stepfather is a resident of The Heights of Tyler. The place is amazing. The activities and the care that they provide are outstanding, I could not be happier. The staff, and everybody there, truly care about their residents. The facility one is very clean, and it stays clean. I have never found that facility or his room to be dirty. They take care of their residents and keep them clean. The activities that they recently did, were trunk or treat, which was a large activity. Then they had trick or treating, and also provided an outstanding Veteran's Day program and honored their veterans, which my stepfather is one of them. They have things coming up next month and all kinds of activities every day. They offer Bible study classes, games, and Bingo. There are so many things that they offer that get the residents out of their rooms to where they just have something to do and not just lay in the bed. We have kind of adopted his roommate, too. When we bring stuff to the facility for my stepdad, I bring extra for his roommate because they are two peas in a pod. His roommate is wheelchair-bound, and a lot of times you'll see his roommate pushing his walker and he's pushing the wheelchair.

My mother-in-law has been at the Heights of Tyler. They help her with everything. She needs full-time care. She had a stroke and she's in a wheelchair, so she can't walk. She has therapy there. They provide meals and all of her personal care. They have full-time nurses, too. It's a full-time facility. There are two to a room with two beds, so my mother-in-law has a roommate. It's fair-sized. There are two twin beds on each side, and they have their own space for clothing. We are able to put a nice little lounge chair in there for her to watch her TV. It's nice. It's very clean. The floors are pretty much kept clean. Sometimes we'll go in there and sometimes there's a little bit of trash on the floor or something like that, but that's not big deal. My mother-in-law seems to be happy with the menu. They have several dining areas. It's a pretty big facility. Each wing has two or three dining areas. The staff is wonderful. And they do lots of activities there. They do church services. They have hair and nails that you can get. They have a bus that will transfer you wherever, here, there, and yonder. We like it because they have a doctor onsite, so my mother-in-law doesn't have to go see another doctor off premises, so that's nice. They have lots of activities, and they're always busy.

My stepdad has been in this facility for 3 months (Long Term Care) and the experience has been amazing! The staff are friendly and so helpful in all aspects of his care. The Activities Director is exceptional - she plans great activities for residents inside the facility and outside of the facility! The Admissions Coordinator made the check-in process easy & it was FLAWLESS. I cannot say enough good things about this facility and the staff!

This place is not for someone you love. The staff yells at patients and try to intimidate them. They don't give medication correctly. They don't change patients. My loved sat in their own waste for 2 hours until they called us for help. Some of the staff is just hateful.

This place was AWFUL. Brought my 81 year old dad here and had to complain to the admin constantly because they weren't doing their jobs. They finally got tired of me and released him before he was ready to be released. His cardiologist agreed that he was not ready to be released and so did his PCP who we had video conferences with while there, but they insisted just because I kept complaining. They "forgot" to take him his supper twice during the couple of weeks he was there, would not take medical concerns seriously (showing signs of another cardiac event and the NP didn't even respond till the next morning and none of the "doctors" will talk to family members - my husband refused to leave that night and stayed the whole night with him so he could call 911 if he had to), did not do his PT like they were supposed to (didn't even start it for days after he arrived until I threw a fit), kept changing the sign-in rules for visitors and then would yell if you didn't sign in "right", didn't treat the bedsores he got and they were so bad when he got out that we had to take him to wound care at our local hospital for weeks to have the wounds debrided, missed taking him to his heart doctor appointment and I had to reschedule it and take him myself, left him without toilet paper all day long one day even when he kept asking for toilet paper so he could go to the bathroom, left his toilet clogged one day all day until I came that evening and insisted they unclog it, and there was so much more. Some woman representing administration came in and chewed him out, argued with him even though we had proof, and refused to tell us her name (I voice recorded that conversation - she was very hostile). I kept all of the detailed texts and emails I had to keep sending to the admins. So I have proof and am happy to share that proof with anyone who is questioning leaving their loves one there. I DO NOT RECOMMEND!

We placed my 86 year old mother in here. It seemed like such a nice place. In 8 days she was never changed or showered one time. I had to go in and do everything. They have a schedule of activities, and I never saw them do one thing. One day at lunch there was a blind resident just sitting there with her food in front of her. Nobody was helping her. They were going to move her to a more permanent hall, and even the nurses said don’t let her go there. I walked down that hall several times and it was walking down the green mile. If you care about your loved one don’t put them here. Yes, it’s nice but the staff don’t care!

To Whom it may concern: Our mother and Mother-in-Law was admitted to The Heights of Tyler rehab and nursing facility 2650 Elkton Trail, Tyler, TX 75703 after being discharged from UTHET Tyler Hospital. During her stay at the heights from she was readmitted to UTHET Tyler Hospital for a GI bleed. IT was discovered she had not been given her Synthroid medication and her TSH level was > 100,000. The Heights of Tyler also changed her aspirin 81mg to aspirin 325mg prior to the GI bleed which I am contributed to her GI bleed. The care she received while staying at The Heights in Tyler was terrible. They were not attentive, did not answer the call light in a timely manner and were always short staffed. I would call and request a call back from the DON and NEVER received a call. The business office manager failed to fax information to the insurance company and after going into her office and seeing the piles of paperwork in stacks, it is no wonder why it did not get faxed. It would have been like finding a needle in a haystack. It is very unfortunate that this patient has had multiple hospitalizations and set back in her health because of the care she received during her stay at The Heights in Tyler. I strongly suggest that they be examined before another patient and family has to endure the pain and suffering that our family has gone through. I just pray my mother can eventually pull through this tragedy.

The care is exceptional! This is a very loving environment. The nursing staff is on top of things. If there is a slight issue they do not hesitate to call the Dr to proceed in care. Communication with the family is very good. Activities are daily and so many fun things planned that their days are filled with fun. My Mom is so happy there and never asks me to take her when I leave. She is at home here at The Heights. We are very pleased with the care she is provided.

My mother-in-law is at The Heights of Tyler. She has just gotten there, so I really can't form a full opinion. We received a few recommendations from some people who have their mother there, and they highly recommend it. I really can't go visit, so we made our determination on that, plus the admission coordinator was very helpful. Right now, my full recommendation is limited because of not actually being able to go and visit her. Once I'm able to do that, I can make better qualifications and recommendations for the facility. Sharing of communication probably could have been better initially. Location is very close to us. They have full service. They have rehab, full nursing services, social work, and activities. It's a building, just one floor. The good thing about it is that many of the rooms have windows that are accessible for window visits. That's a plus if they have a courtyard area, but still accessible for window visits at this point in time. They are friendly as far as staff contact.

My husband stayed in The Heights of Tyler for six weeks. The place was a lot smaller but newer and prettier. I like the way that it had one big central nurse's station in the middle of the four halls that branched off of it. The halls were short, and there was a dining room on each hall. Their food was very good. My husband was in a double room, but they let me had the other bed because it wasn't full. The staff members were very good. They did a good job. They also had a church service every Sunday morning, they had a good physical therapy, and they had laundry services. There was a nice outdoors, too. It was a very nice place.

Our loved one is a long term resident. When we came to The Heights , everything was outstanding. We have seen a tremendous decline in the quality of care that the residents receive. The office staff is good. The majority of nurses there are good. The aides there are ridiculous. They neglect the residents on a regular basis. It is so bad that we are currently looking for a different facility for our loved one.

Let me qualify this review by saying the compared to the previous nursing facility my mother was in, The Heights is a vast improvement. First, you may not always have a happy visit with your family member here. Making this place happy and comfortable visit will almost always be a job. Also, always bring a witness just in case you have an "altercation" with stuff. At times I've had to pick up in the room bed, closet and drawers as well as make the bed. However, housekeeping generally does a wonderful job. Unfortunately, some aids are on the phone a lot, playing games, texting and talking loud in hallways. Hardly ever see interaction with residents other than cleaning them up. Nursing staff are very good, generally knowledgeable and attentive. The community often performs different activities (music, church services, Bible study, etc...), but haven't seen much encouragment for residents to participate. Again, The Heights is far superior compared to other facilities we've experienced.

My husband had to go to The Heights for a short stay. It was a very warm and friendly place. The staff did their best to accommodate all of the residents. They have a physical therapy section for rehab and the equipment helped to get him back in shape and strengthen his body. I never smelled urine in the facility. The food was just OK. He ate the breakfast, though. I would make sure you ask the nurse what medications your loved ones are getting. I would recommend a chair in the shower room for people to sit down and dress.

My mother lives at The Heights. I am noting but happy with this facility. I can not think of one thing I have a problem with. Every single person I have dealt with is just wonderful there. I do not think they could be any more friendly or helpful. The food is pretty good! The facility is always clean and never smells. My mother tells me how much she enjoys The Heights and what a wonderful place it is. She also tells me how happy she is. That is all a huge relief for me as I was feeling very guilty having to move her into a home. I LOVE The Heights of Tyler and everyone that works there!!!

Everybody was exceptional, so nice, and quick to respond when something was needed... Rehab people are excellent +++++ This is a very caring facility. If necessary I would request this facility again. I visited three facilities before choosing, I am very pleased with my choice. Food was just ok.

My mother came here for short term rehab after a 4 day hospital stay.The lack of care is pathetic,My mother was there for 3 days and never was given a bath and did not see a Dr. until the third day.The temperature in her room was freezing 66 degrees and there was no chair in the room for her or her guests to sit in.When she was administered drugs she would ask what certain medications were for and the staff would not answer her.Upon being told that my mother was leaving after 3 days of being here the staff came to her room and tried to intimidate and coerce her into staying.She was given a series of questions trying to determine her mental state earlier that morning.The staff were trying to give the impression that she was mentally incapable of handling her own affairs.She did not appreciate these questions and let the staff know this.In my opinion this place is not about rehab, but is about stripping senior citizens of their dignity,their rights,and their money.

My father was at the Heights of Tyler. It was good until he got sick. Then he went to the hospital and came back; they did not take care very good care of him after that. He will not go back to this place. If you needed basic care, I would recommend them. The facility was OK. The rehab itself, I can rate a four.

My mother in law was there for three weeks for rehabilitation. One night a nurse was very rude to her. She was upset because my mother in law needed constant assistance to use the restroom. Finally she just threw her on the bed with no covers on, and the call button was way out of her reach. The call button was also out of reach of the other woman on the bed next to hers. They kept shouting for assistance to no avail. Finally, hours later the nurse returned, and stated how much she hated her job. After reporting the problem, the nurse made comments about quitting her job, but she never did. Nor was she fired. I hope this was an isolated incident. However, my mother in law had to deal with this woman day after day.

I am a current client of this provider

My brother was in The Heights for physical and speech therapy. Their rehabilitation was very good. The rooms are adequate. Some staff are great while some staff are adequate. Now with my uncle there, I can say the same thing except sometimes the cleanliness is lacking in the extended care section. They also have a problem with their accounting processes. They made several corrections. I think they could improve on their billing structure. And they could follow up more closely on taking care of people who need more assistance.
