While my relative was there. The nursing staff and support staff did a great job with the person and making the person feel like it was their home. The staff goes out of their way to help and meet the persons needs. The facility itself is very old and neglected. The rooms are run down, just meeting standards apparently because they have had to pass their inspections with the state, I would guess. Beds are old, TVs are old. The furniture in the rooms are run down. But the staff makes up for this. The management on the other hand is very difficult to deal with. For us they never submitted the appropriate applications for payment, as they said they would. They in turn just sent a bill for payment. Understand and know your rights. food: management met standards nothing more nothing less, and staff made portion size right when they could. Activities: is what they make of it, but good Value: staff makes up for the deficiencies of management. But I can NOT refer anyone to this place.

I would not recommend this home to anyone. Staff was great most of the time but they don't employ enough to do the job with so many patients. Sometimes took 30-45 min. to get on the bedpan and another 30-45 min. to get off it. Food was uneatable and they rarely matched menu, ran out of things, i.e. coffee, peanut butter etc. Meat patties was no hard that you could cut them, how could you ten, chew them? Was moved 3 times in 3 days...rooms are very small with 2 beds in them, add walkers, wheelchairs etc..rooms were crowded. Beds were not changed, sometimes for a week. Electricity was weird in my room several times, needed service by maintenance. Was given a very old bed with a taped up remote but when refused, they got me another one. Has 2 tvs in this small room which caused a sound war with room mate. Would give therapy low marks at times too. Had major problem with billing too. Man in charge quit or was fired but new lady is great...He never submitted my bill to my insurance and tried to get me to pay over $14,000 in co-pays.....he kept saying he sent it to my insurance which ended up being a lie. Once again, the nursing staff and aides were very nice, just very busy with the lack of help.

Never heard f this facility till loved one was transferred there. Wish it would've stayed that way! IMO/E an awful place! Loved one fell often; had bruises; jewelry went missing - never to be seen again; personal items weren't returned upon death; just an awful place - again, in my opinion & experience. Staff didn't answer phones, you'd call & call and no one would answer. When they'd leave messages, didn't give their name. Rooms are as basic as it gets. Not bright or cheery, really a depressing place. Bathroom that staff and visitors uses stinks as though it is never cleaned: strong smell of old urine. GO ELSEWHERE!

Amazing people,they we so good with my mother. If I could give them a 10 I would. The staff is what makes that home so good. I was always informed every step of the way. Up until my mother passed away. Wonderful staff

This place is bad. Dirty and run down. The staff is in efficient and not attentive. Food is horrible and an accident waiting to happen since it sits at room temperature long hours before served and is un edible. Care or lack of is the norm. The administration has no idea what they are doing since they needed to admit my mom paperwork wise owner 48 hours after we removed her for being so bad. They really need shut down until they can get their act together these seniors are at their mercy not far since they are collecting money.

I visited The Hearthside Rehabilitation and Nursing Center, and it was very warm. The place is clean, the surroundings are warm, they use soft and neutral earth colors, it's lighter, and the windows are larger. They have music and different activities available. The staff is welcoming, and there's a visible cleaning crew. They put down pads around dad's bed prior to his arrival. They have music, ice cream socials, outside activities in a courtyard, speech therapy, and physical therapy.

I visited this facility

Hearthside wasn't a place where I would want to put my mother. When we toured, we used the public bathroom and the floor was so slippery that I was afraid we would fall. The dementia unit was small and crowded. The staff was fine, and the head nurse was great. I saw a couple of people involved in a balloon activity, and they also had a crafts area.

everytime I asked for help lifting my brother from chair to bed or the other way around everyone is on break waited an hour and ended up lifting him myself . been there for couple of hours and depending on the aid no one checks his diaper many times found bm on his hands I ended up washing him

I visited this facility

my mom was over medicated and not well attended to. aka a lot of lonely smelly time in the hall way with a dirty diaper. a lot of un needed insulin and drugs. couldn't the cna's just show up when she asked for them? i witnessed 20min waits in her room while she soiled herself several times. i brought her back home.

Just not a place I'd consider for my loved one after I put my mom there for just 1 month. Staff that deals with residents actually tries but just understaffed so my mom gets left in dining room alone with food all over her. Repeated emails and calls were ignored by office staff including the DON ( director of nursing). Was only invited to 1st of care plan meetings and was never informed of other weekly meets. My mom clearly needed a pureed diet and was given a regular diet. Woulda given place 1 star but some of my mom's nurses did have good empathy and bedside manners.

When I first went to visit Hearthside I noticed the personal attention my relatives received. Every time I was there whatever my relative needed staff was ready to serve. If it was help to the bathroom, or whatever it was they needed, I offered to help the relatives myself but they really trusted staff. That tells me they were very helpful to all the patients there. I know when we would take my in-laws or my father on a trip or something; they always were in a rush to back there. They always had activities and they would want to get back to hear music and singing or do craft work whatever the activity was for the day. As for ease of access in they are fairly easy to get in and find your way around and are always happy to assist you to find what it is your looking for. All in all this is a great place if you are looking for a place for a relative to have a big family when you're not around and be safe when you cannot be there to watch them. I would recommend this to someone as a place to look into.

I believe they did a very good job. He was very happy and even called the workers there his ladies if we would take him out for a dinner he worried that his ladies were missing him. So that shows they make them feel important and appreciated still which I think is huge for the elderly.

My visit to this facility did include a tour. The building was quite old, at least 40 yrs. The upkeep was rather poor, I mean the grass was long, a gutter was clogged, doors stuck. The banquet room was more like a school gymnasium.The building was very cold. On the other hand the staff was very friendly, I remember the pastor telling us all how much she really enjoyed the place. And how she was almost gonna be sad to leave them all. The people that I personally talked to were also very friendly. there seems to have been hardly any turnover as far as staff which can only mean that they all really enjoy there jobs and those they. Thats really important to me as far as the people i'd like to take care of someone in my family.

The living conditions of this nursing home were very poor and many residents were visibly neglected. I would not allow any friend or family member to stay at this facility
