Our family member was sent here for physical rehab following a hospital stay. During their 4-day stay, they never received physical therapy. The only people who came into their room were those delivering and removing food trays. Lab work was supposed to be drawn that was never completed. The medical staff did not take the time to educate themselves about our family member's condition by reading the medical information sent over by the hospital, and one staff member became defensive when asked questions about the medical care they would be receiving. Our family member sat in a room 24/7 with no physical therapy or compassionate medical care. As we observed how other patients were attended to, it was apparent this facility is a poorly run and poorly staffed nursing home, not a physical rehab facility. There is only one doctor and one NP for the entire facility. When patients push the "help/call" button, no one responds. It was heartbreaking to see how patients are neglected here. This facility appears to be a holding station, not a facility where patients receive physical therapy or medical care.

So the customer service is lacking, and they basically don’t do anything they say they’re going to do, as far as the care and rehabilitation of their residents. They can’t keep accurate log of the residence’s belongings and lose their personal items. When I confronted them of these issues I get harassed and then escorted out of the building.

Some employees are attentive while others can care less, the food is disgusting the rooms smell, they need a complete overhaul, once that the owner could do is listen to complaints.

We didnt have a choice where my mother in law went. We live in WY and she was transferred to a UT hospital when she fell because she was sic and weak,and they tested and she had COVID in addition to her CHF. They felt she needed cardiologist care not provided locally. The hospital placed her in the City Creek Acute care for rehab and their COVID floor when she was well enough to be transferred. We were appalled at the lack of communication about her status or her care; took nearly two days to confirm they had admitted her. Difficult to reach anyone in the facility and the higher ups did not return phone calls, neither did the doctors. When we were finally allowed to visit, we observed staff on their cell phones, the lady answering phones was on hers and letting the phone ring; no wonder we could never get through. They had not provided a low sodium diet until her son complained. They made her feel they were so busy she chose to use her diaper vs call for someone to assist her to the bathroom. We had to get after them numerous times to make sure her phone was next to her so we could call and that her hearing aides were charged and she put them in. They moved her rooms several times, which made it more difficult to reach her as we didnt have the room number and they were on lock down. I am certain one of the times she was moved was because she is aware and was telling us about her room mate who apparently had dementia and how she was being ignored or not taken care of; mom was real concerned about that woman. They didnt handle paperwork right, I dont know why they have me listed as 'child'; I am her daughter in law. They didnt understand POA either. Her son is the POA and HE MAKES the decisions, but they asked her. She was confused. Their doctor called her insurance and told them she was ready to discharge when the PT and OT had not released her and they thought she needed about 2 more weeks. They were all told that she lives with us, but I work FT and her son is disabled and not able to take care of her during the day other than getting food and beverages and things like that; she needed to be able to get to the toilet with her walker and complete toileting and shower safely without assistance. We ended up self paying for one more week of care!!! They did not sent her insurance all the necessary papers, nor did they send them to the quality review company who investigated when we complained they were releasing her too soon. The place is not well run, there are some good CNAs we appreciated, but there was also one mom complained about who would change her diaper in bed, and hurt her leg every time even though she tried to tell her she was hurting her. THIS IS NOT a 5 STAR facility and should be investigated. Sad that mom had to go through this and the stress it caused her and us not being able to contact her for days. She honestly thought they would put her out on the street and there would be no one to pick her up. We finally got HH arranged and got her home where she is much happier and doing well. Cant make sense of the bubble packs and medication list they sent; medications changes they made. But we have an appointment with her local doctor and a cardiologist to get things figured out. what a mess. And the shared rooms are tiny; barely room for the two beds, two night stands and the bed tables take up the extra room. Dont let your loved ones be sent to this place!!!

City Creek Post Acute Care warning!

My mother was a resident of a memory care unit in an assisted living facility. She tested positive for COVID, though she remained entirely asymptomatic. However State policy dictated that she be transferred to a facility designated for COVID patients. There are only two in the State, one in St George and one in Salt Lake City; she was sent to the facility, City Creek Post Acute Care Center, in SLC. This review is about the insufficient care she received there. She had dementia, but was doing reasonably well for a woman at the age of 92. She could walk, talk, eat and recognize some family members. Even with the shutdown of her facility to outside visitors, she was handling things fairly well. She was in City Creek Post Acute Care Center for 6 days. Here is our experience with them: Within the first 24 hours of her stay, my mom fell and broke her wrist and hand because they didn’t provide her with a walker and didn’t allow her to bring hers from the facility. It was another 12 hrs before they got her to the ER for treatment. During her stay there my sister was unable to get any information about her for the first 3 days. No one ever answered the phone at the facility. When she was finally able to talk to a social worker and told her we had received no information her call was transferred up to the floor and an aide put the phone next to my mom’s head and left the room. My mom didn’t know what to do with it. When my sister got through again and finally talked to my mom she was totally incoherent. When she talked to the aide he checked my mom’s record and told my sister there was nothing written in the record. This is, at least unethical, if not illegal, to not keep detailed records of a patient’s care. It was decided, since we couldn’t get any information, that another sister would actually go to the facility. When she arrived there was no one at the front desk and she waited 20 minutes before anyone showed up to help her. My sister told them that she was going to come the next day to get our mom and take her back to her assisted living and to have her ready. The staff told her that my mom was not allowed to leave for another week. We knew this was not true because the state has already called her facility and told them she was cleared to return. My sister returned the next day and when they brought my mom out she was in a wheelchair because she couldn’t walk, she smelled horrible because she had not been bathed and the adult diaper she was wearing was completely saturated. When they touched her to transfer her to the car she screamed in pain. When she was examined by the doctor at her assisted living facility upon her return he told us that she was so dehydrated she was in danger of renal failure. She continued to scream in pain if anyone touched her, even though they did not touch her injured arm. It was two months of pain for her doing any movement before it was determined by a hospice nurse that she most likely had a spinal compression fracture, probably another result of the many falls we found out she had after we FINALLY got the medical records from City Creek. When she came out of City Creek after 6 days she could no longer walk, could barely speak or eat, didn’t recognize any family members and appeared to be in constant pain. She went into a steep decline and passed away two months after her stay at City Creek. We don’t know all that happened to her during her stay in City Creek Care Center, but we do know that the lack of care she received contributed significantly toward her death. We would like to warn anyone considering placement of themselves or a loved one in City Creek Post Acute Care Center, DO NOT do it!

My grandmother was admitted after taking a positive covid test. She was barely symptomatic, strong and healthy when she went in. By the time my mom forcibly removed her she was dehydrated, malnourished, had broken bones and bruises, and was hopped up on so many unnecessary meds that she was barely alert. Her emotional and mental well-being and overall health are now on a steady decline. Breaks my heart that she received such abusive snd neglectful “care” at this facility.
