I visited this facility

My family and i had a wonderful experience taking our loved one here. We enjoyed the private rooms and the food that they got to pick from. We especially like the staff and how friendly it was

Communication is poor or non-existent if you are not visiting often to check on your loved one yourself (if they are on the long term care side, the rehab side was decent). Most of the aids are nice and accommodating but they appear to get stuck doing most of the work and they don't seem to have much information about your loved one or their needs. One of the head nurses named Michelle leaves much to be desired, nasty to the staff and very rude and nasty to the patients. i firsthand witnessed her behavior with my loved ones roommate and one of the aids i had requested to come help her. Which i had heard her getting nasty with the aid in the hallway i stepped out to speak to her and let her know i had asked her twice to come in and that is why she was in the room and her behavior was the same with me, wanted to argue and badmouth the aid that was helping me. All of this happened the day my loved one died while i was waiting for the funeral home to prepare and take my loved one out of the room (her roommate and family had been left in the room for hours with the body which the head nurse also tried to blame on the aid). Never met with an intake person when my mom went to LTC, never got a call from a doctor about anything, can't get Nicole to ever answer the phone when calling about her bill. If i called to check on my mom their response was she is the same.... Same as what??? i could not recommended this facility if your loved one can't fend for themselves.

Our poppy who was admitted here at Tyler's Retreat for rehab has had a wonderful experience. The nurses and support staff are top notch. They got him where he is noe mobile enough to be in a wheel chair and can move around. He was literally at deaths door and was referred to Hospice 3X BY the hospital. He is now on the long term Nursing Faculity side and he is very happy. His room is always clean , the food is good and he receives excellent care. They have an excellent staff that is well trained and the supporting staff is always available to answer any questions for me. Poppy has improved so much, being cared by them has totally changed his diagnosis. He is healthier than he has been in years with them monitoring him 24/7. I hate to admit that apparently we, his family seriously was not getting his needs taken care of to the opium level he needed. I cannot thank the staff enough for their care. They have 24 hour open door policy and have nothing to hide. Sometimes I have towait at the security door to be let in but that is because the staff is always busy with the residents and not sitting o their phones ! i appreciate that and have no problem letting them take care of the resident needs first. After a bad storm a birdfeeder was blown down I keep at poppys window and one of the staff went out and set it back up for him. You can't ask for more than that when it comes to above and beyond their duty. They have various activities to keep the residents active and involved. You can personalize the residents rooms to make it more like home. Poopy has his recliner, dresser and photos of his family . I have never had a response of " it's not my job" from anyone there, they are very prompt with every request. Nicole has held my hand and has walked me though the entire procedure of the business side, she is a Blessing. I would recommend Tyler's Retreat to everyone who needs Rehab or long term Nursing facility.

My husband was there in May for rehabilitation. The staff needs to be retrained. Lack of compassion and common respect. My husband fell trying to get to the bathroom because no one answered his call. He broke his stitches in his incision and bleed profusely. I however did received a phone call. He sometimes were wearing other patients clothing and his food was left untouched and he was a zombie always medicated.. Our experiences were awful and I Will never recommend this facility to anyone

I only chose one star because I had to! I would not send a family member here much less refer a friend to this place. I have worked in the medical field for over 30 years and I was shocked at the lack of care or notification my mother received. She lost 15# in 14 days, was given very little help with feeding herself, fractured humerus on one arm and fractured forearm on the other, both surgically repaired. I am born and raised right here and have never heard of Tyler’s retreat and surprisingly the providers I work for had zero knowledge of this facility—shocking!!! My mom checked herself out after the head nurse chose to argue and upset my mother. I would search heavily for another facility to place my family member

My mother is in Tyler's Retreat at Iron Bridge. I have no problems with the staff. They seem to be very knowledgeable. She's only there for rehab, so they have been working her through physical therapy and making her eat, cause she'd lost a significant amount of weight. It's been very, very clean, very tidy. It's been COVID, so it's been very difficult to visit loved ones in that situation. And I find that the rules changed quite often on how that worked, but other than that, it was fine. Her room is very large, single occupancy, her own bathroom, very tidy, very clean.

Could not get my father out of there fast enough. He contracted Covid19, THERE. When asked about the process they said the would have to put up a partition due to the number of cases they had. WELL, i did get that news letter. I had he prepped and ready to go in15 minutes. one week later, over 80 patients and staff members contracted Covid19 with 4 deaths. NOT A WORD FROM THEM regarding his safety.

My uncle stayed here for skilled nursing and was cared for excellently. We visited daily and saw no red flags. The admissions process went smooth and very clear cut. The rooms are average size and maintained. Staff in all departments were always friendly. My uncle enjoyed playing bingo and not missing out on church. The facility was conveniently located for our family. I am glad we went to tour the facility before admitting my uncle because we were able to see how clean it was, what the nurses were like, the food, and see activities happening. The main thing we noticed throughout the admissions process and my uncles stay was how clean the facility was. Staff were very helpful if we voiced any concerns.

I cannot get my mother out fast enough. Food horrible and not healthy. Nursing and doctors/nurse practitioner all giving different drugs. Thank God nurse caught it. Do not send a family member here if you love them and want them to get better. My mother is getting worse in the week she’s been there. Working with insurance and doctors to get her out.

The CNA staff has a usual turnaround. The professional staff, ie, Director of Nursing, Senior Nurse, administrative staff have been accommodating. There were a couple od instances and concerns, however, the staff was engaging in a timely manner. I would recommend this facility. CVS

I visited this facility

This place is the worse, office management is rude..to many family members working here...They DNT know how to talk to the staff , anybody' to the matter...I wouldn't suggest anyone to put love ones in Tyler's..I wouldn't even put my dog here....sad...the staffing is always Short..so how can patient's get the care they need...I think they need no management."in the offices that is" it starts there...

My mother was placed in here on Tuesday--July 11. She had fallen, has two broken shoulders, a fractured wrist & cracked rib. I stayed with her until late afternoon. When dinner was brought to her she was told to feed herself--her reply was she couldn't use either arm or hand. The person stomped out of the room & sent someone else 30 minutes later. They gave her one spoonful of dinner said they didn't have time for this & left her. My brother & I stayed Wednesday to make sure we could feed her thru dinner. That evening she informed them she had to have a bowel movement & they told her they didn't have a bed pan and to go in the bed. Quite amount of time before they cleaned her. When we got there Thursday morning the same thing happened & she laid in her feces for 45 minutes! I had her discharged & moved to a facility in the West End that could provide the "skilled nursing" that she needed. I cannot rate this place because there is not a number low enough. The sad thing is the occupational therapists were wonderful. If you are putting a family member in there be very aware of nursing and care givers!!! The new facility indicated the condition she was in when she arrived was deplorable. They have gone above & beyond since Thursday to provide the care that she cannot do for herself--thank God I got her out of Tyler's!!!

Patient was put to and oxygen mask was not put on his face by worker. Family member caught the oversight and alerted the caregiver who had already left the room for the night patient was to be on oxygen all the time because of a breathing problem. This is the honest truth
