If you are choosing Avante don’t this facility doesn’t care about staff or residents. Severely overworked nursing staff and support staff don’t get any recognition or respect. Staff who have been there run the building instead of the administrator. You have Director of Rehab is a OTA promoted because was friends with previous administrator and corporate says she is grandfathered into nepotism policy so her husband works under her as a pt. MDS doesn’t make sure charting on mds is completed or tasks for cnas are checked off. Let’s not forget the Dietary manager just does whatever he wants. They show no respect to the DON who works her tale off. They have residents smoking after hours and some even smoke in the room. Nursing is overworked. They recently had a wonderful social worker who made sure these residents came first and understood everything they could about their situation and care plan. When you set foot in this facility you automatically feel disconnection and discomfort. Not a place to be for anyone loved one.

There are large holes in the wall behind the toilet. Mother was left laying in bed was supposed to be in there for for PT only to regain her strength....never happened. They would not get her her teeth so she could eat or bring her a fork or help her eat. She attempted many times by herself and food was left all over her bed and her overnight. She came home with a bad bedsore.....she was only there for 10 days and if I would not have pulled her out she would not be with me today. The nurses with the exception of 1 never even entered the room. She was never bathed and they let her lay in a soiled diaper for over 12 hours I had to make them change it and bathe her myself. Her feet were blue due to the fact that she was just left laying there to die. I bathed her in a wheelchair and massaged her legs and feet to get the circulation flowing so she didn't lose her feet!!!!! They just want the money that Medicare pays for the first 30 days....The nurses/aids would sit up at the nursing station and play on their phones while every patient rooms light was going off! I could go on and on...............DO NOT ALLOW YOUR LOVED ONE TO GO TO THE HELL HOLE! IF YOU DO YOU WILL BE PLANNING THEIR FUNERAL!

I visited this facility

Avante Ormond is Horrific. Our mother was moved her for rehab after Covid treatment in the hospital. There was one RN that was kind and helped me wash her. I had to use soap from the hospital and she could not get anyone to bring her a wash cloth. Mom is mobile with a walker. They were surprised I brought her walker. Mom wears Depends in case of accidents, but was told to use the bathroom in them by a CNA standing right next to her. I work in inpatient care and would not hire one of the CNAs I met there. The building is old and run down and the was a bed with wet sheets in the hall. PPE was provided with no instruction.. There was a roach in the shower, the shower that backed up. To top this all off, her new red walker was switch by removing her bag and luggage Tage to and old black walker. All of this happened in less than 24 hours. The activities coordinator called me to tell me the Mom could participate in group activities after telling me she was in isolation. No rehab. Mom was left in bed for the entire time. I did catch AHCA site before allowing her transfer. I made a mistake sending her here. I wish I’d taken video.

My mother was at Avante for a week and it was not a pleasant experience. I found the staff to be rude, abrupt, lazy and even mean to patients there. On several occasions I had to provide my mother and her roommate with necessary supplies as staff was unresponsive. Administrative personnel were unfriendly and poorly trained (if at all) to answer basic questions. Facility items such as beds, wheelchairs and mechanical items were poorly maintained and even dysfunctional. I witnessed patients in need of cleaning, medical attention and assistance. Food quality was poor according to patients and communication was virtually non-existent. I contemplated removing my mother after a couple days it was so frustrating. Stay away from here!

PLEASE DO NOT ALLOW THIS PLACE TO CARE FOR YOUR LOVED ONE! My mother was there for less than a week due to a broken ankle. We pulled her out due to neglect and took her home. The NURSES do not listen, their excuse is NOT MY JOB! They were always late giving her medication, she never saw a doctor, she got a nasty infection, they left her sitting in a chair all night and day until family arrived......I WOULD NOT ALLOW THEM TO TAKE CARE OF MY DOG!!!!!

Weathered the hurricane very well -bit of damage palms, so definitely feel safe here, in fact they even brought people in from other facilities that were damaged. Rooms are modern updated and very well maintained! Has beautiful outdoors Deck with screened tables. Wish to mention a couple of employees outstanding: Joe CNA on day shift to 7-5 Gentle, patient as I'm a bedridden patient . Patient with changing/empty my catheter bag never begrudges When I defecate need to be cleaned up. Lynn (nurse) daytime shift 7-7 always on time with medications never begrudges changing wound dressings I have for quite a few dressings to change. Always buoyant uplifting attitude!

My mother who has Alzheimer's chronic kidney disease hypertension among other things was transferred to this facility from the hospital after she fell and broke all free bones in her ankle as well as dislocation in tears she had surgery and was total non-weight-bearing for 6 weeks this is her third full day at the center and she has not been seen by The Physicians because he was there Thursday before she got there and he won't be back until Monday and they did the bare minimum of intake and risk assessment on her once she arrived on Thursday around 6:00 p.m. and thankfully today Saturday her physical therapist understood just how bad she was being treated no risk assessment being left out in the open area the safety reasons because there's not enough people to check on her I just could go on and on but it it if you want compassion if you want a chair if you want two pillows if you want a straight answer from more than one person if you want to find a nurse on and on don't come here especially if your person that is here cannot advocate for themselves

My mother is in a rehab facility at Avante at Ormond Beach. They have a new director in there. He sat down with my mother and came to a compromise; now he's got her eating. He calls me up and says, "Oh, she's a lovely woman to talk to." I said, "Was that my mom you talked to?" He's patient with her and accommodating. I don't know if I would give a good recommendation because I've had a tough time with them. I don't feel like their personnel are very up on things. I'd probably give them a 1 or maybe 2. We've had so many problems. It takes a minimum of five tries on the phone to talk to my mother. I bought her new clothes, and they've never gotten to her. I have only been there once, and they would not let me inside, so I cannot tell what it's like inside. I was allowed to go outside the facility because of Covid. I had flown down there for a week to try to see her and try to do things, and I only got to see her one time. They need to put awnings or something where their people can come outside and get fresh air. There's just a lot of them that congregate at the front door because that's where there's an awning. They need to do something more for the residents there. I was told that they have church services, and they have an "activities room" that they do things in. I was told that they have a director that takes care of them. I've asked my mom, "Do you do things," and she says no. I said, "What do you do all day?" She says, "Nothing." She just sits there and watches the wall all day, but I know they have to be doing something. One day I called during the day, and the nurse told me when she was trying to find my mom that she was at the bingo table. If I were running it, I would make sure that there was always music going on outside, and there was always something jovial going on. I would have something uplifting and happy, old funny movies, or something like that, that they could relate to. The new director that they just brought in within the past two weeks to a month ago is really taking a command there in a good way. He went and actually sat down and had a conversation with my mother and said, "What do you need? What can I do for you to help you?" She had a hard time getting pills down at night. Milkshakes were thick and helped her get them down. She would ask the girls to make her a milkshake, and they wouldn't do it. They said they didn't have a milkshake machine, or they didn't have ice cream or whatever. We went and bought ice cream, but they still wouldn't do it. My sister went and bought a milkshake machine as well. It was like "we've given you everything you need." It was crazy. Money wise, it's pretty good, but they could be better.

I wanted to rate it -4. My husband was aqt this facility as it was the only one outside Orlando and Jacksonville that weaned people off a ventilator. I visited twice a day. On the fifth day, I came for an afternoon visit and found the vent tube for oxygen on the floor. He was dead. they said they had just been in his room 5 minutes ago. but his skin was already turning yellow, so it had been much longer. There were no alarms or oxygen meters attached to him, which would have alerted them immediately and is used in all hospitals, one alarm wouldve saved his life. I had assumed if they were the only one rated for this care they would have extra staff and specialized equipment. I reporte dot Medicare, who came one day and said the linen room was clean and some other thign. Nothing related to my husband or review of procedures. Not a single attorney would take the case. I was told that the court would say it couldve been anything, like a heart attack. So who's hands are in whose pockets? Take your loved one ANYWHERE ELSE.

I visited this facility

I was admitted for rehab after a hospital stay for COPD and treatment of lung cancer. Unbelievably i spent five nights breathing in cigarette smoke from residents and employees. There were visible flood lines on walls with a moldy odor in room. Toilet was inpperative my entire stay, Food was gruel and all salt. this place NEEDS TO BE SHUT DOEN!

If I could give less then a 1 I would. My family member was here recently for rehab for a broken hip. Not even a week at the facility and they suffered a bad fall and broke the same hip again. Staff and MANAGEMENT was aware our family member was a fall risk. All concerns including staffing issues like different nurses and CNA's everyday were addressed with MANAGEMENT. They had their state survey 3 weeks ago, so the results which are public record will be available soon for the public to review at-http://www.floridahealthfinder.gov/index.html

I highly recommend if you have a loved one in this facility that you check on them daily. They do not have enough help and there are times the patient will sit for quite awhile before someone will come to their room once they’ve pushed the button for help. The food is not the best. The rooms are clean. Again keep in touch with your loved one

Family member fell within 3 hours of being there. When asked why don't you have bed alarms The nurse practitioner said Ive never seen them here ....Well why not ??? Maybe she wouldn't of fallen on her face. Stroke patients are sometimes impulsive and you do nothing to protect them. When rang call bell ,stating she needed to use the bathroom, they told me they just change her when she soils or use the bedpan.......HELLO.. toilet program consist of toileting them every 3 hours during the day with bedside commode if unable to make it to the bathroom. So she sat in shit for 45 minutes. Think twice before you let a family member go here after a stroke.

Nursing is horrid. The DON never answers my questions or the phone and is ver unprofessional. The staff are always on the phones at tables.. food is mediocre, cold, smelly. Therapy is ok. Mom likes them. The guy that cleans the floor is nice .

My mother was at this facility for rehab after falling at home. I cannot say enough about how well she was treated during her stay. I was pleased to see that they had therapy 7 days a week. Mom is back home now and is doing great! She really enjoyed her stay and still talks about how wonderful the therapists and staff treated her. Hats off to this facility!

My Mother was discharged to Avante while recovering from a surgery. I don't wish to speak badly of the caregivers: some were better than others, but for the most part they seemed sincere and compassionate in a job that is far from glamorous. However, those same workers are overworked and overwhelmed, while the administration, in my opinion, proved less than useless. Total lack of communication, a brusque attitude, and inept follow through added together to make my Mother's stay uncomfortable, unproductive, and potentially deadly. I wish I had reviewed this facility more thoroughly before she was sent there, but I trusted hospital protocol during the time of her discharge, and they recommended this place as their third choice (in the first two, all beds were taken). This facility has more the atmosphere of an insane asylum rather than a skilled nursing facility. I do not believe the care there even begins to approach what I would consider quality, and the whole place is depressing. I'm so sorry that I let my Mother suffer there (she passed away shortly after she was discharged). The reason that I gave the facility two stars is because they do have some very compassionate and well meaning caregivers working there. I think that the best of their workers would be better off seeking employment elsewhere.

Another scary night at Avante! I hope that says a lot. This place is great for recovering from respiratory problems but they will try to make sure you remain a resident afterwards. Stay away from the North Wing. Some of the CNA's and Nurses at night are there just for the pay and there own medical benefits. Some are not there because they care about your love ones. They are the ones who work the 3rd shift because they aren't closely watched and they can neglect their responsibilities most at the same time they can let out their own frustrations and humor themselves as they neglect caring for your love ones. I say this with deep sorrow and hopes that it will deter others from the nightmare I lived as my mother stayed here. PS- I had to give then 1 star to accept my rating.
