I am a friend or relative of a current/past resident

my mom stays here. She has been here for 8 years. She loves the activities that they do. They make crafts, play bingo, cards, have wheelchair games, have different functions for different holidays and other things. Sometimes, like any other place, my mom doesn't always like what they are serving for dinner, but she has options when that happens. The staff always greet you with a smile. My mom's room is personalized with some of her things from home, so she feels at home there. We can come and take her out to eat, go shopping or take her to a family gathering and then bring her back with no hassles. She has made friends there and has lost some friends there. My mom now calls this place home.

Our Mom was at this facility! We couldn’t wait to take her out of here! They missed her meals 2x and Lucky for her we had family there that went and got her meals! The social worker C would not communicate with family! Mom’s facility and Mom’s social worker from My Choice ! Every time I tried talking with C she said she was going to a meeting! Mom arrived there on November 13th on November 17th my Sisters were there and Mom had not been given a shower, my Sisters went and asked for a shower chair to bathe her! I called M who was now answering our families concerns and asked why Mom hasn’t been given a shower by staff in 7 days . I was then told by M that this has never happened before and that the head nurse has now put Mom on a Tuesday/Friday shower schedule! Also Mom was not supposed to go to the restroom on her own! There were times when we were there and pushed the call button and we took Mom but waited as much as 45 minutes for anyone to answer! We have documented all this information daily in our notebook daily!

My mom was told she was going to be released from Rehab on 8/3. The day came and went and they told her she couldn't leave due to "paperwork". She was going into assisted living. She was told if she went home "paperwork" would have to start all over again. Yes, she didn't confirm which assisted living facility she was going to until 8/4, which led us to believe she would be there only one extra day. "Paperwork" should have already been done for her release on 8/3, as they kept reassuring her. They did nothing on the 4th and 5th and over the weekend. My mom had to go into the Social Service Director's office on Monday and she finally completed paperwork. Then we were told a doctor need to sign off on it. This should have already been done. Two more days delay. Then mom needed to sign a form for prescriptions to move. Why did you wait until 8/10 to tell us that when you knew where she was going 6 days earlier? Then they faxed an illegible document to assisted living. They stared asking me for money 8/11 and didn't even provide me a bill. Mom wasn't even allowed to leave until until 8/12. When I got a bill with INFLATED costs, I started asking questions and have been ignored. I've sent 4 requests so far, even to the Executive Director, with ZERO reply.

My dad was at Lake Country Health Services. They provided therapy for him. He liked the food better there than the other facility he was in. His room was beautiful. It was a nice place and they have been very good about communicating with us.

Staff was friendly and professional. Rooms cleaned several times a week. Food was very nutritious and portions appropriate. Nurses were friendly and professional and answered all questions I had. Aides were kind and helpful. Rehab was amazing. Goals were met and I was home as planned.

The staff at Lake Country is so great and the nursing staff too. Therapy was fun and made great progress. I have full faith in the therapy department. The food service and food was good I liked the variety. I feel prepared to go home they answered all my questions.

The staff at Lake Country Health Services was great the nurse's were absolutely amazing. A big shout out to Lois, Madison, Jacklyn and Nathan. The call light response only got better with my stay, I can say it was excellent. Therapy was awesome, friendly and listened to all my needs. I would come back if I needed Rehab again and recommend this facility to everyone.

The staff at Lake Country Health Services is excellent and my call light response was good. Therapy was good and the staff was encouraging. The food improved throughout my stay, with the help of the dietary manager. I would recommend Lake Country Health Services to everyone and return if I needed therapy again.

Staff at Lake Country Health Services are good, my call light response was good from staff. My room was nice and the food was fantastic.

The rooms were nice, and usually housekeeping kept things together. They offer van transportation and it’s so wonderful. The food on the other hand was not good. I know there are different levels but to have some taste to what is served. The nurses, MA’s and CNA’s were really very helpful. Times they were rough when. PT and OT were very positive and helpful. Special thanks To all For pushing me to Do what I needed

Lake Country Health and Rehab Center is new and lovely, but my wife is not very pleased about the services of the staff. Her food is delivered to her, but she claims it's 75% inedible. She likes her OT and her PT.

Lake Country Rehab is a horrible place to be, I was a patient for 11 days and it was a horrible experience. I had a knee replacement, expecting decent therapy.it was horrible, the things I saw and experienced while there was horrible. I would never recommend this place to anyone.pm me if you want further information

We were not sure if my mom needed nursing care or assisted living and Lake Country offered both, so we chose it. The staff is outstanding and provides everything you need, so that is very good. For activities, they do the normal trips and tours. They also have movie nights.
