Spent some time in rehab recently after having a leg amputated. Both the therapy and nursing sections treated me like and lord and catered to all my needs caringly, professionally and with great dignity. I am now home and getting along excellently due in large part to their help.

Staff in the area were fine, he office manager was not. I prepaid to retain a room after my father fell on their watch. I was told the unused portion would be refunded. After many unanswerd phone calls, emails, and visits I finally found a supervisor in customer sevice who helped me. Almost three months later I received the refund from Sara, the office manager who would not reply. I do not trust their office staff and so would never recomend this facillaty to anyone.

Our loved one has been residing here for approximately 3 yrs. I was somewhat satisfied at first because I became familiar with the staff and they knew that we would visit at least twice a month. We were finally able to visit him in person since COVID. I know that there is a shortage of staff but we should not have to see our loved ones like this. Dry skin was caked on his face and ears, fingernails were dirty and long, hair and lent was all over his jogging pants and it wasn't his hair. The staff that brought him did not appear to be clean either. Staff was unprofessional at the main entrance and at the temperature checking point. Stated that they did not have number to talk to someone if you have a complaint but had an email address. The one at the checking point had her shoes off and and was using her personal phone the majority of the time and of course the visit was 45 mins. late. The cleaning staff was more professional then they were. If you have any love ones in the nursing home, please check on them. I can't believe the staff/administration let this happen.

it was a terrible experience. I want my refund that is owed to me. I would also like to talk with the CEO or Director of this facility. My wife has been calling every day since March 6th.

This place is horrible. They neglected my father while he was there. Left him sitting in the hallway for hours and hours even though he was in pain. When he left this facility and I went to pick up his belongings his dentures and shoes were missing. I called several times regarding this and was told “I don’t know anything about that”. Also discovered later on that someone used his cell phone to call Jamaica 2 times and send several text messages. My father was in no condition to make any calls and never texted. We reported this and nothing was done. I am sure it was a member of the staff who did this. This is a horrible place, the first day I went to visit my father after he arrived it was clear he was very sick and when I finally got someone to come check him they just called the paramedics and left the room and never returned to check on him before the paramedics showed up. He was also left to sit in his own excrement until I showed up. Then the aid came into the room to change him and she tried to get me to leave the room. I refused to do so, she wanted me to leave so I wouldn’t see it was dried on him. This place is horrible and I would not recommend this place to anyone. They never forwarded his birthday cards to us for several weeks after he died. I am so glad we took him out of this place and took care of him at home for his last few weeks of life. I would have hated for him to have died in this terrible place.

I visited this facility

I am a family member of a patient in the rehab center. He is extremely lucky to have enough family/friends that stay with him from 8:30 am - bedtime. Even with an advocate with him, it would literally take hours to get help. I’d hit the call button and they would check in, but never solve the problem. The staff is not very consistent and don’t seem to communicate between shift changes. I am currently waiting for someone to help me put the catheter back on, change his urine soaked bedding (because they never came to put it back on) and help me move him up in the bed because his legs are scrunched at the foot. The first time I asked was 6:30 and it is 9:15.

They would take hours to come help after you pushed the call button. They are mean to the patients. The food is horrible and unhealthy. They had pretty good activities to do. Some rooms are clean and others are not. The CNAs would throw trash on the floor and there was a problem with bugs. I wouldn't put my pet dog in there.

My grandmother was on the Monticello unit and fell 11/29 at 830AM. They stated she hit her head on the sink. She didn’t not get sent out until 1PM to the hospital. I can’t believe a head injury was considered non emergency. The amount of neglect is unbelievable. I have also contacted Mark Woodard several times regarding this and he has not responded. He was seen in person and quickly avoided the situation. No one from that facility has yet to respond.

I was admitted to strengthen my limbs after an illness. Pt and ot worked out great and all were helpful. Only seen the dr twice during my one week stay. The nurses were good but understaffed. My husband was allowed to stay with me making all easier for me over a week stay. All was ok till day of leaving. I was told about 11 and now at one still waiting. When I ask about issues they say none as they are working on it. And I am still waiting

Overall, badly run and poorly staffed, except for the rehab folks and a few RNs. Rooms are Perpetually unclean, trash piles up, laundry undone, beds unmade/unchanged, paging calls ignored, medications haphazardly administered and frequently run out. Complaints get you nowhere. Family must be present and pro-active to ensure minimum care, especially on weekends. Security is non-existent—people can walk in the back door and pass through unchallenged. My observations based on 5 continuous weeks in residence for pre- and post-op care and for rehab. Physical therapists were skilled, caring, and responsive.

My MIL is currently in there for rehab. I would not take my dog there! Staff is lazy and unresponsive. Requested that bed linen be changed, they brought it in and placed it on the bed and never returned. They do not keep patients clean and we had to direct them to give a shower to MIL. We provided private assist personnel while she is in there and the staff did nothing? On the weekend we had to request when meals would be served. At 2pm she had not received any meals for the day. We spoke to management about our concerns and no improvements were noted. Stay away from this place.

Horrific place! Aides never answer calls and come turn them off and don't return! Your love one lays for hours wet or worse!! The dining area cooks give food to employees and private sitters!! We pay for this!! People cry for hours ! Cnas always hiding!!

My mom is currently at The Gardens at Warwick Forest. The place is nice and clean with no strong odors when you walk in which was my concern. The place is also quiet. I can see no patients just sitting in the hallway. They have a church and a bistro where we can get sandwiches, hamburgers, or whatever you want. They also play bingo.

The absolute worst place I have ever been in. Would never stay there again even if it was free. Worst staff, and the mist horrible food I have ever had. Dirty rooms aides never arounf.bad attitudes just for .starters. no rating because it was that awful.

My mother is a resident at Warwick Forest and I am almost considering moving her elsewhere. Sometimes I go in and she has a sour smell on her, not being bathed properly, she sweats underneath her breast area and we (family) could tell she had not been bathed adequately. I appears as though quite a few, just out of college, are coming in and they are not trained adequately or still learning, I don't think my mom should go around stinking or not be assisted in her feeding because of that. When she first started there (my mom), it was very nice and she was kept up much better than now. I've noticed alot of the ones that were there when my mom first came are leaving or moved in other areas, they should have kept the ones that were familiar with working in that area.

I recently had visited my Grandma here after a knee surgery. She had her right knee done and had to go to a place from her current assisted living place to go heal up and manage her progress on walking "“ you making sure she walked and worked her leg while being monitored. Now let me note, Grandma has slight dementia. She repeats a few things often, and sometimes gets confused. This is though, if she is in a familiar place and with familiar people she does fine. So much so that you may not even notice it at all. Now, her insurance had her placed in a sort of rehabilitative home? To help her heal. What we (the family and I) were not told was that they were to place her in a nursing home. Now this in itself wasn't too bad. But remember she gets confused if in a new place. Confused, a tab ornery but relatively harmless thinking she has to go pick us grand kids up for stuff or that's she's going home in an hour. But this was enough to place her in the mental ward of said home. This had my Grandma who at worst would demand to call my Ma to pick her up with persons who stared at walls, wandered into her bedroom thinking it was hears, talking incoherently and generally people with conditions that made her look sterling. So whenever we'd visit we be walking into a rather depressing situation the was reminiscent of One Flew Over the Cooks Nest. I say this not because of the patients themselves but because of the overall mood of the staff. Some were very nice; a lady as we left promised to give Grandma her water as she had asked and we had to leave. But others just looked bored out of there mind there hearding their patients like cats. The fact that her resident doctor their changed Grandma's meds (many that is changed or taken away causes massive mood swings to her) without consult her power of attorney, my Ma, did not help our opinion. Ad in the 70s dated decor and poorly maintain paint and rooms (in stark contrast to the visitor area) made the place just give a feel of dread. Grandma from day one wanted leave. Even without dementia I seriously agreed with her. Rather uncomfortable experience here. The day she was sent to her regular assisted living residence, the family was relieved.
