I am a friend or relative of a current/past resident

I have been visiting loved ones at this nightmarish facility for over 14 years and it is heart breaking to see how residents are treated. I have seen staff deliberately ignore a resident who needs to be changed just so they can finish socializing around the nurses station. And if a resident repeatedly asks (or demands) to be changed, they risk being yelled at to wait their turn. The food served to residents is barely edible, poor quality and frequently so under heated that it’s actually cold. I’ve seen the ice machine and portable coolers so slimy they made employees almost vomit. If a resident is able to care for themselves, they may do fairly well, but those who are fully reliant on the staff suffer tremendously. a stroke victim without the use of an arm was left to feed themselves and frequently went without eating at all. Keeping residents hydrated is virtually a non-priority. Potential new residents are given tours that are completely misleading to the actual conditions of this horrid place.

I went there for physical therapy & nursing care in Feb 2023 until March 2023 after recommendations from Sentara. This place is not good. The staff forgot me the first couple of days. Most of the aids stand in the hallway at their nurse carts texting on their phones all day. The food is horrible, always cold & never on time. The rooms are dirty My shower curtain was disgusting. The physical therapy dept is trying but they are ill equipped. The activities were the high light of my stay you could tell that the coordinator really enjoys the patients - over all though I Would never recommend anyone stay here

My brother is in James River Convalescent and Rehabilitation Center. He's been here two weeks now, and he seems to be doing pretty well. He's bathing, and he says the food is good. It seems like they're taking really good care of him and making sure he gets to bed on time. They've been taking care of his feet and nails. They just cut his nails and his hair is washed and clean, which it hadn't been for years. His room is very good, he came with some bedbugs and they fumigated immediately and took care of that, which was great. He has to smoke at designated times. He gets five cigarettes a day. It's probably better for him, but he doesn't have the freedom that he had, and I think he misses that freedom. The nurses are very nice, very helpful, and very informative. And they seem to be working all the time. I've only been here three times. It looked very clean and neat. They have some activities. One day they had some people come and had an activity, but my brother doesn't participate in them.

This place is bad. My mom was in this place care between June to September 2020. No one called me that she was transferred to the place. I called the front desk to see what room she is in and what is the treatment plan was. No point of contact for the case manager or anything. After calling the second time they finally gave me a number to call submission. They didn't give me any information about lad anything that she might need or have there. Social work called me on Friday which the next day that I need to fill out paperwork and she realized that the last name was for name misspelled and she was going to correct it and made sure she had the right email. I never received the email. I called on Monday to let her know I didn't receive it and she realized that she sent to wrong email and didn't get a bounce back. I was worried that someone now has my mom's personal information. I received the form and it was wrong my name wasn't corrected. I had to white out most of the information to correct it. I had to ask the case manager to send me a welcome packet about their place. My mom was complaining about the nurse not helping her when she needs it and they were rude to her. I have called all administrators and left a voicemail. No one called me back It took few times to get the social worker to call me back when I have questions. They didn't give me an answer on how my mom is doing in order to move her to assisted living. I ask multiple times about the trach collar, they said she needs to see a doctor. They waited for weeks for her to see a doctor check to see if it can be removed. There few times the front desk didn't know how to assist me in getting to the right person. I ask many times to speak with a doctor there and never spoke to them once about her care. I end called the corporate office about all the issues. Some were fixed, but they keep getting worse. They didn't clean my mom's trach collar for a month. I ask the head nurse that they need to clean that daily and they didn't. The doctor took her off one of the heart med and when I called the nurse to see what it was, they told me that they didn't. When I took her to her heart doctor at riverside they ask me why she was taken off this med. I told them they told they didn't, and my mom said they did. After that, I didn't believe the nurse anymore and listen to what my mom says. They mess up doctor appointments because they didn't follow the doctor's order and they end up getting her appointment cancel which cost me an off day, time, and gas. I could go on and on about this place. I have seen staff not wearing masks, as they should. They had a breakout of the whole wing getting covid-19. Please save the trouble, don't go there.

Some CNAs ask the visitors to clean the patients if they are wet assuming that they are aides that are there to do that job and then put long faces if the visitor doesn't do it and in top of that they complaint because the visitor is there sitting doing "nothing". Saw one cockroach in the room after patient moved there and there was no food in the floor to be blamed. Nurses come to see the patient only to give meds and take vital signs, if the patient requires assistance to go to the bathroom they expect the visitor to take the patient to the toilet and wipe them, nursing home rooms very small. Some other staff are cooperative and friendly, but others maybe they think that all visitors are aides but then why some people working there don't want to do their jobs? If they are short in staff they need to hire more. Rooms are old and the furniture is old. Staff is not going to be there checking on your loved one all the time, most likely will be alone. Patient fall, broke a bone.

The staff at James River Convalescent and Rehabilitation Center was very friendly, welcoming, and seemed happy to be there. The rooms were nice. It's an old place, but it's well kept.

James River Convalescent & Rehab did what they could, but they're just not well-equipped to handle dementia people. It's a good place with good people, it's clean, and well-put together, with nice activities. The food seems to be good, and the staff tries really hard. The building is a bit older and kind of musty.

I am/was a resident of this facility

It was the worse 14 days of my life. I never seen an CNA except to bring food. When I rang the bell for pain meds and to go to the bathroom the call bell was shut off at the desk and no-one ever came in. I was on my own. What a very helpless feeling the whole stay was. They missed many medication doses. The room and bathroom was so filthy! I would never recommend this facility to anyone.

I am a friend or relative of resident

My mother is a resident here. She gets good basic care I suppose but I am concerned because she has fallen three times and taken to the ER. This last time she got a black (and swollen) eye, several stitches and a concussion. When queried no one was able to tell us what happened during any of the three incidents. I'm not sure what they provide beyond basic care. They roll her out in the hall in the morning along with other residents and leave them sitting around the nurses station.

I am/was a resident of this facility

The aides and the nurses of James River Convalescent Center were very kind and very helpful. They went out of their way taking care of me during my stay there. I was there for three years and they proved to be valuable assets of this nursing home. On the other hand, I was not pleased in particular with the people from the top management. At night, they would only have one nurse for two units, which in my opinion was just criminal. If there was an emergency, there was no way one nurse could cover it. I think the management cut corners way too hard to the disadvantage of their patients. Also the food was terrible. I would not recommend this place.

I am a friend or relative of a resident

My maternal grandmother was a resident at this facility. She lived here because we could not care for her on our on and the previous facility she lived at did not care for her properly. This facility had such caring staff and they took good care of her to the end. One of he favorite thing about this facility is that every other thursday they have three dogs that come in to visit the residents. On sundays they load anyone who wants to go on a bus and take them to church, she always liked that as well, but rarely felt well enough to go. The food they serve the residents is delicious and they have snacks available at all times. Visitations are always encouraged and looked forward to. Family members can pay for meal cards for themselves so that they may eat with their family member, which is nice and the residents love it. I do recommend this nursing home to anyone in the area.

I visited this facility

During my HS days I hung out with the elderly for class. It was one of the best decisions I've made in my life as it got to see how the older half lived. In some places, the living was great. In others...not so much. Maybe it was just me but this place just seemed 'dead'. Like the staff was just phoning it in. I remember hanging out with this funny little lady, she said she was 76. We would making fun of the terrible food comparing it to airplane food. Now we were nice about it just joking around. But then one of the nursing staff came in and just glared at us. Like what? She then went and yanked the tray from the lady's table in a huff. Apparently staff here lacked a sense of humor. Hopefully time changed this. In fact aside from the residents the place was so ho hum I can see why the residents joked about it's blandness so much.
