There is no communication I have called to talk to someone in charge about reaching a patient in this facility no way to reach a patient the patient has cell phone and was told they can't get out of bed without assistance, but when they ring buzzer for help no one responds I would like to speak to someone in charge!

The nurses and caregivers are doing their level best to care for our loved ones….BUT… there has recently been a management change and things have gone from bad to worse! Staff shortages and sometimes one aide for the entire unit!! Residents not clean and groomed, showers not given Residents’ clothing brought to laundry, never returned!!! Food is very poor. Snack and fluids not provided unless brought in. Staff is wonderful, caring and compassionate…under very poor circumstances. So, Stern Management….how will you proceed???? Nurses and aides should be compensated properly and given supplies to do their jobs properly. Staff to resident ratio MUST be improved!!!! Rooms are not cleaned properly.

Place is disgusting. Especially upstairs. Old stained carpet stained with urine and feces. The smell is unbearable. Terrible terrible food, everything from cans and no seasoning, the best way to describe it is pig slop. Lots of dry instant potatoes with no seasoning. No snacks and hardly ever any drinks other than mealtime. If you don’t bring them in food, chances are they will not eat and will not be getting any real nutrition from the food served. Poop dried onto the walls. Wheelchair was never cleaned. No janitors after lunch time. Sad excuse for “activities”.

My brother is in Apple Valley Center. The staff is very compassionate. They seem to care about his situation. One thing that impressed me is that they have activities all the time. There's always something for him to do. I think that's very positive because it encourages him to socialize, so it's a good thing. The food is resident-dependent because it's specific to the dietary needs of each resident. There's not a lot of variety, but he's getting the type of food that's appropriate for his needs. He may feel that there's not a lot of variety, but it's contingent upon his dietary needs. It's laid out well and it's very clean. Every day, they have different types of activities, playing games and watching movies or doing some kind of social interaction. He went there for rehab back in May. He then transitioned to assisted living and then he transitioned to long-term care or the nursing home.

This place is real bad .If you love someone please do not send them there.I have a loved one that went there. They lost a lot of her clothing. Would not replace I had to buy her new clothes .I could'n afford it but is the only way they will replace it. the staff is bad .I was there when my loved one pressed the button and she sat is a wet pull up i had to get someone to help her . this place relay stinks .Please check out the the place you want to care for you'r loved one. Talk to someone who is already there!!!!

CNA's and a few nurses very caring. Dr doesnt like to treat UTI's had discussions about this numerous times. Mimicked by Floor nurse for always having my loved one checked for UTI's approximately 4 out of 6 times I was right. Before my loved one passed away I had to demand the dr go upstairs and evaluate her after 7ish days of telling them she wasnt right after a fall from a wheelchair and declining health she was looked at 2 days later sent to the hospital.
