I am/was a resident of this facility

I was here for rehab for a hip problem, and only lasted 9 days. They short changed me with diabetes and cardiac medications and I had to call my own doctor to raise hell. I was only allowed to use a shower twice in 9 days and the women's showers didn't work and I had to use the men's side. The food was terrible and 90% of the time I ordered food delivery. I was left for 9 hours with zero pain control and the call light was always ignored. I finally called a cab, hobbled out to the front door, left and was taken up to Howard County General Hospital ER and treated properly so I could go home. I reported this facility to the Maryland Office for Care and Compliance of such places, and the report I just received was pages and pages long about all of the violations the inspectors found. I wouldn't send an animal here! One night, I had to use a wheelchair and take myself out to the nurses station and found a nurse sound asleep on one of the couches in the day room. No wonder they weren't answering the call light ! This place needs to be closed down IMMEDIATELY !!!

Run, do not bring your love ones here! There is no way your family member will be tended to! On many occasions the staff will not even respond to on call lights or even attend to residents in need. They do not do any personal care, oral, showers, or grooming. On several occasions we found our family in wet, soiled diapers and no one to assist with changing or cleaning him up. They do not keep clean rooms. The facility is kept under staffed and patience are the result of neglect and negligence behavior.

My mom stayed there a month. Some of the nurses were mean and had nasty attitudes. My mom said a female nurse didn't want to change her, because she had an accident. So a male nurse came and washed and changed her. My family made sure we came to visit everyday, then they were nicer to her. She did have a very good physical therapist.

The care for our Love ones is not a high priority in today's society. This is not the fault of some staff members who do their best. I'm currently experiencing similar substandard practices regarding the care of my Mother as I did 16 years ago when my Father was in a similarl facility in Prince George's county. We all deserve better

I am a friend or relative of a current/past resident

My husband was at Patuxent River Health and Rehabilitation for 12 days, and I took him out against medical advice. He was in the hospital up until September the 1st. On September 1st, he went into Patuxent. He was in Patuxent from September 1st to September 13th, when I took him out against medical advice. That night, he wound up going to hospital. When I took him out, I knew he was in bad shape. I knew that Patuxent was killing him. When I took him out, he had a fever. He had a urinary tract infection and was incoherent; they didn't know it. They knew nothing about that because their staff wasn't paying any attention to him. Directly from Patuxent, we went into another hospital. He was in the hospital for four days, and I got him released out of there into another facility. Patuxent were absolutely ignoring him. He was very sick, and they were not paying any attention to him. The staff were rude and inattentive. They were actually hostile. I think he thought the food was OK. The facilities looked dirty. I wasn't paying for it because it was all covered by Medicare and insurance, so I don't know what they were charging. They did not have a gym, and they didn't have a salon. They did have a courtyard. The courtyard was dirty. The furniture out there was broken and rusty. I got to see him in the courtyard because they wouldn't let me in the building because of COVID. The only place we were allowed to meet was out in this courtyard, and the furniture was just kind of piled up in piles. It was dirty and rusty. It was very unpleasant.

My mother was admitted to Patuxent following a brief stay in the hospital. She was admitted for wound care because the facility where she has resided for the previous three years, was unable to provide the required care. When we visited Patuxent to see mother, we were overjoyed with the attention and care she had been given. The staff at Patuxent were friendly, caring, and best of all, responsive to our requests for information. My mother showed improvement in her condition. She was clean, groomed, and well cared for. I highly recommend Patuxent.

Provider Response

My experience with this facility was equally positive and negative. Many staff members went above and beyond. Social worker, business office, dietary, reception, wound care were excellent with each interaction. Nursing left a lot to be desired. I felt that there was a lack of empathy for the family members at a time when visiting was not possible due to the pandemic. No nurse answered the phone after as many as 35 rings. In seven weeks, I spoke to 2 nurses. Now that visitors are allowed, hopefully families will have a better experience. Upon discharge of my husband, I was told by the nurse not to expect much. I’m praying that we can prove that wrong

I wanted to to let the staff of Patuexant River Health and Rehab Center know how grateful I am of the care they have given my sister in-law over the last 7 months. Your dedicated staff were at all times thoughtful, knowledgeable of her health needs and goals. They also did not hesitate to make our family partners with her recovery. Your center has truly concentrated on patient-centered care, from your nursing care staff, therapist, administrative, social work and financial staff. You are HEROS while working in healthcare during this time of COVID 19. Again thank you.

Provider Response

I am/was a resident of this facility

I was in Patuxent River Health and Rehabilitation Center for rehab. Their facility was between adequate and good, their therapy was better, and I don't think they have enough help. It was pretty clean, and did not smell of urine. The food was good, but the staff was very slow to answer my bell. The food was well served, and it was warm. They need to improve in communicating, and respond timely to my call.

I visited this facility

I was a patient in the REHAB SECTION ONLY for four weeks January - February 2019. I chose Patuxent after visiting once but mainly because it was close to family and friends. My choice was correct. The facility (my room, the lounge area outside the door, and the gyms) where my rehab work was done were clean, bright, airy, with a TV, reading materials, etc. Accepting the kitchen had to cook for the MANY patients in the different areas of the facility, the food was better than average. There was a wide variety, a specialty order menu, and I could get seconds when requested. The main missing element was FRESH VEGETABLES AND FRUIT which was surprising in a "health related facility". The staff was trained, conscientious, caring, and most of the time responsive. Floor techs (nursing assistants) were EXCELLENT....friendly, fun, aware of patient problems, needs. Physical therapists were knowledge, healthful, encouraging and did their job well. The only negative was the nursing staff....a few very good but most "just putting in their time". It appeared nursing management and oversight was basically missing.

My Mother In Law was sent here in March 2018 w/ out us being able to tour the Facility beforehand. The first time I walked in this place I walked right out because I couldn't breathe & my eyes were burning from the strong smell of urine & feces- this was during the weekend which is way worse than the week, but the aroma is always there no matter what day of the week you visit. The nurses are rude & clearly do not like their jobs. The way they talk to the patients & family members is appalling. There were a couple of times I went to ask her nurse certain questions & of course both times she was not around & no other nurse could/would help, I was told I had to wait for her assigned nurse & still never got my questions answered. Every time we went to visit she was never clean, there was one day Mom asked me to give her a shower & they wouldn't allow for one because it wasn't her day. She even had to be moved due to a stalker man visiting her so they could monitor her closer & what did they do -moved her into a room with a man. Try to explain this to someone with Dimentia cause she was so confused & didn't understand why they moved her in a room with a man when they moved her out because of a man .... I could go on & on about this place & what a disgrace it is forever, I cant believe they can even operate this way. We have to be our loved ones voice especially when they can't & would NEVER RECOMMEND THIS PLACE TO ANYONE!

I am/was a resident of this facility

I was a patient in the rehab section from October 31st 2017 until February 12th 2018. I had gotten sick in October and was in Howard County hospital for a week with an infection and my strength was totally depleted. I was transported via ambulance to Patuxent River and from that point on all I did was get better. The GNAs, the nursing staff, the physical therapy staff, the occupational therapy staff and all the other support of staff were super. I went from not being able to walk or get out of bed by myself 2 walking out of the facility to go home in February. Even though I wasn't there for weight loss I figured if I followed the diet that was given to me by The dietitian but that could only help and I wound up losing over 55 pounds during that time. I would highly recommend Patuxent River Rehab to anyone needing rehab from an injury or stroke or fall. I was there I witnessed it everyday the caring and the genuine knowledge of what needed to be done to get me home and on my way. Thank you so much to all of the staff.

My husband spent one week there. The rehab. was great but the nursing care was terrible. I went every day and there was a problem on each of those days. He was put in a Depends at 12:00 one night and when I went the next day at 1:00 he hadn't had it changed nor had he been washed. I don't believe they even checked him at all at night. He had labored breathing one morning and was sent to the hospital. He had a 103-degree temperature and died at the hospital. I have to say the nurses were very patient and caring to all the patients but that is not enough. They needed to respond to each person's needs, not a blanket treatment for all.

I am/was a resident of this facility

I am a continuing patient at this facility since May 2017. I was transferred from PGH by ambulance @1030am as soon as my diagnosis was confirmed for a stroke. I retained speech, expression & problem solving but motor skills were a problem. The compassionate and professional therapists were wonderful! I am currently being weaned off my walker using only my own poser - what miracle workers!!! As both an in-patient and outpatient, I received "5 STAR CARE". The food may not have been great, but how many different diets need to be accommodated?! I feel I am qualified to make this assessment since I practiced as a certified and then licensed Nuclear Medicine Tech from 1968-1995.

I am a friend or relative of a current/past resident

My mom is at this facility, she has been here since Feb 2017. I tried to take care of mom at home ,but her dementia was more than I could handle then. I visited many nursing/Rehab facilities before I choose Patuxent. Out of the 30 places that I visited only 2 seemed good. Patuxent and another facility in Odenton MD. We all know that no nursing home is going to be 100% on point in all areas. But we tried to find a place when we walk away we are not in tears or worried about our love one. When I leave mom I feel confident that mom is okay. The staff there to me are good people doing and trying to do the best they can . They always have fun activities for the residents. The staff are friendly and will stop what they are doing to listen and address your questions and concerns. The food is not bad ,especially those dinner rolls, mom makes me try it some times. The people in therapy are wonderful ,they have helped mom recover when I resolved to the mindset that she would never walk again or be able to use her right hand. I am very grateful. Liz in admin has been very helpful when I had questions concerning the finance. There are issues of concern like the ratio of cnas/patients. When there are fewer cna on duty the residents are not able to be attended to when needed. Which frustrate the resident ,family members and the on duty cna that is over worked. Maintenance addressing concerns on weekends seems to be an issue. I do not dismiss anyone's concern or experience because it is real ,I can only tell how I feel. I will informed the facility if I see something out of order that will affect any resident . In summary, when I leave mom ,I feel that she is safe and her concerns will be addressed.

I have a family member in this facility. I have been visiting my family member at this facility for over a year. They are understaffed in all areas. The patients are not being tended to on a regular basis, the food is terrible, the facility needs a lot of repairs done due to the age , when u walk in the front door it has a very strong ammonia & feces smell and the billing statement I received is unacceptable and does not give a break down of charges. It is a down right shame how the patients are neglected and uncared for. I believe that a proper inspection needs to be scheduled and completed as soon as possible before tragedy strikes.

I am a friend or relative of resident

We first visited the facility without my grandmother, took a tour and were not impressed. Some of the patients were outside but it was a 95 degree day. A lot of the patients seemed ignored. My grandmother stayed here temporarily over a weekend. She came back home 5-10 pounds lighter (not good because she was already way underweight to begin with.) She was stiff, as if she hadn't had medications the entire time she was there. Her hair was a mess and she told me that they basically left her in her room the entire time to fend for herself. Her glasses were left behind and they refused to mail them to her home. Instead, we had to go back out (an hour drive) to pick them up. We went on a weekend. The facility was severely understaffed and there were only a handful of GNA's and nurses to take care of the whole entire facility. That's crazy. Some residents were left in the cafeteria to feed themselves but they obviously needed assistance (they probably had dementia based on behaviors.) I would never put another family member in this place ever again.

I visited this facility

My mother just had knee replacements at Laurel hospital and this rehab is across the street. This was actually one of the closest rehabs to us. this is by far one of the worst place I have ever been seen. This "rehab" is so unorganized. The two days I stayed there no staff too effort to listen to her. Her medications were given late, she was skipped for dinner and lunch, I'll say that again they did not give her lunch or dinner because they "didn't know" a patient was in the room. This rehab has bugs , and is very very unsanitary. I would highly recommend to save the life of your loved one and not send them here.

My father-in-law is getting therapy at Patuxent River Rehab and Nursing Home. He's learning how to groom himself and how to talk. However, he's not getting the treatment that he really needs to become self-sufficient. He stays in a sedated state when I know he can do better. There were a few times my wife and I were there and we didn't get a response from him. I think he feels much better when he's not sedated. He wants to get away from this place. They have some good people on staff but they're not doing all they can. I've also noticed that they seemed to care when we're there but who knows what happens in the middle of the night. They also seemed to be overwhelmed but I don't know if they're understaffed.
