I am/was a resident of this facility

This is for the patients who come after me. No nursing facility is perfect. I am currently a rehabilitation patient after open heart surgery. Cleaning and maintenance 5 stars. Front desk receptionist 5 stars. Dietary 3 stars.To long to write. Skilled nurses 3 stars they get angry at you if you care about your own welfare. Patient bill of rights not respected. This does not include all of the skilled Nurses. Its like rolling dice, you get what you get. Nursing assistance staff 3 stars. Physical therapy and Occupational therapy 10 stars above and beyond the call of duty. Medical Doctors 10 stars they listen and each patient is not the same. I found them to be caring and able to think critically.

I visited this facility

I took a tour of Fountains at Mill Cove. The staff there seem very friendly. The main parts of the facility seemed good. I was a little hesitant about shared living spaces, but I thought it was fine. I like that they have daily activities. Involvement or socializing I guess is a primary way of helping people in memory care. Cleanliness does seem to be a little bit on the wayside. The room that I toured seemed a little dingy and I could see there was still some work being done on some of the walls. And I did overhear someone talking about mold in one of the rooms. So it doesn't seem like the best. They offer outings to baseball games and the beach and it seems like they have performances or activities daily like music, arts and crafts, and things of that nature.

I visited this facility

We toured Fountains at Mill Cove. It was absolutely wonderful. Overall, it was fantastic. The staff was really friendly. It was very clean. Everybody was super helpful. It's just a very warm and inviting environment. The dining area was very, very nice. They gave us a sample of the menu and it looked great.

I visited this facility

We found Fountains at Mill Cove for my brother. They had good food. They had an award-winning chef. The people liked the food. They have a lot of water ponds right there. People can walk around. They have a lot of different things, like card rooms, game rooms, exercise rooms, physical therapy rooms, and a large library. They have a lot of activities scheduled. They have everything that a person would like. The apartments had more square footage than any other place we had been.

I visited this facility

I went and toured Fountains at Mill Cove. It was an old facility and it needed a facelift. The one room they showed me for rehab was a two-person room and it was tight. Assisted living and senior living were also on that same campus, but in a different building. The staff member was a little vague on everything, like ballpark this and that.

I am a friend or relative of a current/past resident

My mom is at The Fountains at Mill Cove. She has been there for about a year and a half now. The location had a lot to do with why we chose it. Her home was 30 minutes away on the other side of town, and it is like 10 to 15 minutes from my house, so it was much closer to me. They have activities directors who have a schedule of activities that they do with the residents. With dementia, they have an activity room in the unit she's in now. Some things go on in the morning and then in the afternoon. Because she doesn't like to stay in a room, she likes to go to the activity room. There are a variety of different things that go on. I have not tried the food, but my mom has gained weight, and she especially likes desserts. However, I don't think their maintenance is as good as it should be. It is an old facility, but the unit that she's in is not as old. I don't know exactly when it was remodeled, but it has been remodeled. It's pretty clean, too. They make a pretty good effort. Of course, like a lot of facilities, it's hard to keep enough staff. The memory care unit is constructed differently, which is more helpful. Of course, they're locked in. The construction of the memory care unit is rectangular with gathering areas on the inside. The dining area and the activity room are on the inside. The outside is the rooms for the residents so it's easier for the staff to get to the residents. Some days staff members may not show up, and there's not enough help, but I would say most of the people who are staff members are very nice. They're nice to the residents and they're nice to the visitors. I go several times a week there because some things aren't done. Like I sometimes have to make the bed and change the linens myself. Their performance could be improved. The nurses are nice and very easy to talk to, though. The problems I've had with them lately in the past six months have been the newer residents who are in memory care with them having access to my mother's things. I've approached that with the man in charge, but sometimes I think that there are a few residents that are misplaced that shouldn't be there. What they need to improve on are supervision and communication.

Fountains at Mill Cove didn't really show us that much. There was no private room, which was what we were kind of hoping for and it seems like the staff was very, very busy. They had a lot of people to take care of. So, it was a very short tour. It's not a brand-new place, but the girl who toured us was very helpful and friendly.

My dad is a resident of The Fountains at Mill Cove. They tried really hard to keep up. The facility is older and needs some repair. I do feel like it is safe and clean, but it is getting run down. I think it is all good. My dad included in his rent or his stay there the services for three meals a day down in the cafeteria or dining room, but he does not like the food. There have been other complaints, but people do eat there every day. I think it's probably sufficient. My dad loves the happy hour on Fridays. They have some entertainment for an hour. They serve wine and beer. They have some music. He really enjoys that, but he doesn't participate in a lot of things they offer. They do offer quite a bit. Once a week, they have a van that takes them to the bank, the grocery store, Wal-Mart, and that kind of thing. Because my dad doesn't drive, it is a real plus for that that place. We've been very happy with him there.

My mother was at Fountains at Mill Cove rehabilitation center for almost a week. She was dropped off at about 11:00 on Thursday night. My mother is 88 years old and has hardly any memory, and they did not check on her all night. She was not able to get up out of bed and she's extremely diabetic. Until Monday afternoon, she was not getting anywhere near the amount of insulin and nobody would call the doctor. I called there Friday night and the only person that answered the phone was a nurse that was from an agency. The nurse couldn't find a supervisor and my mother's blood sugar was 405. Normal is 100, and 405 is extremely dangerous. I could not get anybody on the phone. I near called EMS and the police. I got there the next day and showed them my mother's sugar log because she has a NovoLog patch. Before she went there, it was in green zone most of the time, sometimes it was in yellow, but never, ever red. It was always red at the Fountains until Tuesday. Her diet was not low carb as a diabetic should be. I took a picture of it the last night she was there. It was a broccoli, cheese and rice casserole, and it's way too many carbs. Then, they gave her a sandwich, which was a regular white bread. One night, they were supposed to give her diet pudding, but they gave her pancakes with whipped cream. You cannot control blood sugar on that diet. The doctor never saw her. I asked the nurses when he came, they said, "We don't know." He doesn't come every day. He might come twice a week and at any time. It could be 2:00 in the morning. Physical therapy started on Monday. They saw her for 20 or 30 minutes once a day, Monday, Tuesday and Wednesday, that's it. It was horrible. I have pictures, I have a log, and I have proof. I told one nurse that, "If it gets up high tonight and you're not going to help me, I'm calling EMS." It took until Monday to get somebody to call (with an urgent request) to get her insulin order changed. The rest of them didn't do anything. The food was not bad. It's just what they were giving her was horrible due to her medical condition. The facility was old (and I don't mind old), but it was clean. They don't do anything with the people on the rehab side. They throw them in a bed and check on them when they have to. My mother was in danger at 405 blood sugar level and you can't get a hold of anybody to get her insulin. Quite honestly, had I been able to get a hold of a nurse, she would not be able to give her more insulin because she would have to call the doctor. I called the doctor and I called the director of nursing, but they never called me back.

My mom was a patient at the Fountains At mill cove rehab center. The first night she was dropped off no one checked on her, she was unable to get to the bathroom on her own. No one monitored her blood sugar. It took 4 days before I was able to get her insulin adjusted. Red zone most of the time there. Food was not diabetic friendly at all. Pt came on day 4. Most nurses are good, they don’t have any help from the doctor. He never saw my mom in 6 days there. He’s collecting a pay check

We toured Fountains at Mill Cove, and they showed my brother a studio apartment on the ground floor. The place was very nice. The tour experience was good. It's dated but it's a very nice facility. I've had people that I've known go into that facility and they were very happy with it. The staff who assisted us was very cordial. It was efficient; it would be perfect for my brother. Unlike the other facility, this was more down-to-earth. The facility was very clean, too.

My father has been there for 4 years. They have done an outstanding job with the care. My father is a very independent and honestly a stubborn person. They are very patient and accepting with him. I cannot tell you how much I appreciate the staff. I am very comfortable with my father there. I feel confident that he is in good hands. It is always easy for someone to say something negative. My father has been there long enough for me to know what a good job they are doing.

I visited this facility

We've been to the Fountains at Mill Cove. The staff was very friendly and it seemed clean. I feel like I should see it at a good point in the day because it was a nasty day so a lot of residents were not out. I didn't see anything negative or any red flags that were striking but I heard one of the residents complaining that their room was not cleaned appropriately at the moment. Another woman just said she wasn't very happy there and was trying to leave so I don't think the residents were very happy with the place. It was the independent living that we visited. I think they had a nature walking group there. We also saw an apartment and it was spacious but honestly, it wasn't an empty one so it was with someone's stuff in it. I can't say much more about it just because it was one of her things and belongings and it was very cluttered. I saw a list of their activities online, but I didn't see any of those things taking place during my visit.

I visited this facility

I visited the independent living and assisted living of Fountains at Mill Cove. The independent living rooms were small, but adequate for my needs. The dining area looked nice, but the thing that stood out for me is, they told me that the community has a meeting with the dining staff regularly (I don't remember if it was monthly or less frequently than that) to talk about what they like and what they might want. The day I was there, they were having a Mardi Gras activity that they invited me to come back to, I didn't go back, but they had brought food trucks in for the residents to enjoy.

My dad is a resident of Fountains at Mill Cove. Based on the diet, the food they're serving has a large amount of salt in it, and my dad is supposed to be on a low-salt diet. They provided them with an 1823 form, which means they're supposed to have put them on a low-salt diet, and they won't do it. The facility is OK. It's not anything spectacular, but it's not horrible either. Some of the staff are good, others are not. They have some staff that have attitudes. Their location is good. They're not providing the necessary diet, the low-salt diet, even though they're required to by law. I don't think there were many activities going on, but that could be because of COVID as well.

I'm at the Fountains at Mill Cove. It is a nice place. The way the employees treat you, like the kitchen workers and all that, they're nice. I like the place. They have things that I like and enjoy. They have craft shops for people that enjoy doing crafts. They have a solarium, which I love. You go in there and it's so relaxing. I've been recommending this because it is really nice. I like the majority of the staff. They're good, friendly, and they're always there if we need help. They're there for you. They have a cleaning lady that comes in once a week and does the cleaning for you, the counters, and all that. It's just pretty good. I like working with the plants, but we can't do that yet because of the weather. Once the weather clears up, I can get out there and work with plants. They have little bus trips and they just leave things open for people. They have bus trips to the mall and Dollar Stores. Sometimes, they just go out on tours. They keep you busy. They have three meals a day. You can go in for breakfast, lunch, and dinner, and the food is pretty good. They've got a decent chef, but it's always the same thing. They're real big on fish and chicken and I'm not. Value for money is good. The only time I require anything is maintenance. They are really good.

My mother-in-law is at The Fountains at Mill Cove. It's great. They have crafts and activities for the residents daily. She's only been there a day, though. There are lots of places to walk, and because it's a spread-out facility, you can basically do a lot of walking indoors. So, you're in a climate-controlled environment while you're walking. The staff are very friendly and willing to help. They're helping her find resources if she needs something, and they're coming to her door and introducing themselves. It's an older facility that needs some updates. There is wall-to-wall carpeting everywhere, and I think it would just feel cleaner if they had some kind of hard-surface floors for residents instead of old carpet.

I visited this facility

My husband and I are looking for a place where we could share and the right size place for us was the Fountains at Mill Cove. The place seemed friendly and they're going to renovate the apartment we chose so it's going to be clean. The people who were there seemed content as much as they could be when they're not in their own homes. Overall, we had a nice impression of the staff. We toured assisted living and also saw the independent and memory care side of it. I liked looking at their grounds, they seemed to have enough equipment, and the staff was very nice. I also got favorable comments from the people who live there. I had some friends who had a family member there and they were pleased with the place so that influenced us too. They have a courtyard, a salon, and a private dining room if you don't want to get into the big dining room. They had all kinds of different facilities and transportation for doctors' appointments, things like that.

I visited this facility

I liked the apartments at Fountains at Mill Cove. They weren't as nice as the other one we visited, but they were adequate and people were very nice. They were having an ice cream social in the middle of the day. The ladies took me around and introduced me to the residents. All of them were saying, "This is where you need to be." They also give you three meals a day, electricity, phone, and weekly housekeeping. They would come in once a week to clean the place, change and wash your sheets, and put them back on. The staff was just very, very, very good. Courtney was really good as well. The place was as clean as it could be. They had a patio and all, but no pool.

I visited this facility

We are hoping to move my mom into Fountains at Mill Cove this week. She'll be in independent living. The process of moving, giving information, following up and all that is going great. I've seen the place and it's clean and well-kept, very roomy, the people are very pleasant to be around and everybody's friendly. They've shown us the activities that they can do, and there's a lot there. They have crafts, bingo, reading, outings, and social events. They have an atrium that is sunlit, but close to the outside, so it's a very bright area, with a pond in there. A very pleasant place to sit in.

They've got an excellent staff here in Fountains at Mill Cove. They've shown me the ropes, so to speak. They even helped me fill out all the forms that I had to fill out for my veteran's benefits. They have tons of activities every day, good food, and friendly people. People go out and walk, they have another activity where you walk your dog too. They have a shuttle that will take you to church on Sundays. They have a Daily Chronicle, which is like a little newsletter, it lets you know what's going on. They also have something called a Nature Walk, crossword puzzles, a wheelchair, and walker repair. We have popcorn, usually about 1:30 every other day. They have bingo, card games, and shopping trips 3 or 4 times a week. They have a scooter school so if you get a scooter from either your own accord or through the government, they have a little school that they show you how to operate it, how to put on the gas, put on the brakes, and how to steer it. They have a menu chat where you can talk about the menu and what you like and what you don't like. They have what they call the Wii bowling, the brain twister, and pet therapy, where those who don't have animals can go and they bring dogs in and you can pet them. They have arts and crafts and they just have a bunch of stuff here. They have a resident council too. They have a party coming up this weekend called the Spring Festival. They also have what they call compassionate moments discussion, they have grief support in case you have lost a family member and they also have what they call the calming arch. They have an active committee and a drive-in movie. They got just about everything you might want to find in a community. There's a river that runs through the facility here and you can go out, they've got an island out there where you can go out and fish. They have gardening, you can go out and you can plant and take good care of it. They got dominoes, cards, and you name it. I have one of the smaller rooms, by choice, but it's very large for just a kitchen and a bed. It's almost 500 square feet. There's a nice balcony that you can go out to and overlooks the lake. I'm very satisfied with it. The food's good, it's fresh and well-cooked. They offer a wide variety of choices at every meal, they'll give you a menu and you just pick off the menu what you want. They have a couple of different entrees and they have all sorts of drinks like lemonade, iced tea, milk, and water.

I visited this facility

Everything at Fountains at Mill Cove looked good. It was just an older facility. I am not sure if they are going to do an upgrade. In most of the rooms they showed me, the view is of the parking lot. They have a beautiful atrium with a fountain pool where you can sit. It is all enclosed and beautiful with a skylight. They have an activity room, plenty of coves to sit in different places, and a porch outback that's all screened-in. The staff was great. They came to me and introduced themselves, and they told me about the facility. It was not that bad, but maybe they should upgrade the facility or just upgrade the rooms. Everything seemed okay. It is just an older facility.

My mother-in-law is at Fountains at Mill Cove. So far, overall, it has been pretty good, no real complaints. The room is fine. The only thing I would say is she needs more lighting than what is there. The big overhead light that is on the main room is not a very bright light, and she wants a brighter light, and I need to put in a different fixture for that. I wanted to use whoever the facility uses to come in and do electrical stuff, I did not want to do it myself. We called the guy three or four times and never got a call back from him. We were disappointed with that because if something is broken in these apartments, whether electrical or plumbing, I'd like to think they would take care of it pretty quick, or at least respond to the request and give some idea of when that can be taken care of, but that did not happen. That is the only issue I've got. If the medication needs to be given, they use a person who can give a shot, but cannot adjust the shot for something that's changed. If something is changed and it is not exactly within the parameters of what they're giving the shot for, they just don't give it. I'm like, "No." I like the food. The staff is good. It has everything she could want right there, and it is close to the house. Everything stays where it belongs, it is clean, and it smells good. They have people they use for medication, but if the RN or the doctor is not available at that time, they can't make adjustments, so they just don't give it, and I am like, "That is not acceptable." The place is very nice and well kept up. They have lots of activities. I'd like to see the cost a little less, but I do understand.

My sister is moving out of Fountains at Mill Cove to go to Colorado to her son's house. She has been there for two months now. It's a large, nice-looking place. However, she is not happy there because it's too restricted for her lifestyle. Part of it is because of Covid, but the general gist is that she isn't happy. She doesn't like the food either. She isn't happy with the age of the people that are there. They are all older than her. I don't have any problems with the staff members. She has a 1-bedroom apartment, but they have a variety of rooms. They have a bingo and the usual things you see in a nursing home, but it's rather restrictive because of Covid. The staff is fine. The facility is laid out in a large area, and it's made for a lot of walking. I would give the activities a 2 because of Covid right now; it's probably better then.

Fountains Residence at Mill Cove (Formerly Brookdale Atrium Way) was a big facility, and my mother had a long way to walk to the dining hall. I had a virtual tour, and they showed me everything I needed to see. My uncle lived there for a while, so I was able to see what it was all about when he went there. He started off independent, and then he went to assisted living. They had a lot of things going on there for the residents to enjoy. The location would've been better for us because it's only a couple miles from where my sister and I both live. The outdoor space was pretty good sized. He never complained about the food there.

I visited this facility

I liked it. They have a really good and beautiful facility. It's great except that I need to cook my own food. The apartments are nice but there's not kitchen, and I can't be without a kitchen. The staff members were beautiful people, friendly, and so helpful, so it was a neat and great experience. The place is really pretty, which is what attracted me to it. I saw the picture and I went and it really is pretty and nice. The community is nice and they're very busy people. I noticed that they do a lot of things. It's perfect for a person who needs that kind of living. What impressed me the most was the way the staff caters to their residents. You get the cleaning and laundry done. I really think it's a great place.

We moved in yesterday at Brookdale Atrium Way. So far, it's fine. The place is very clean and very caring. The staff is excellent. They're very friendly, very knowledgeable, and very helpful so far. The food is excellent too.

I visited this facility

We visited and everybody seemed busy, but not too busy to smile and be friendly. It was homey and comfortable from the get-go. It was big and they had exercise rooms, a courtyard, a swimming pool, an auditorium, and a cafe room.

It was a very beautiful place; it’s immaculate. The people were very nice. It was a clean place where I would feel comfortable having mom to live. Staff seemed to be friendly, and most especially it’s close to home.

I visited this facility

This community is the same as the other facility I visited. The rooms were homey. As for activities, the residents look at questions on a TV screen, and they have to decide which choice is the correct answer. If they get the right answer, then they get a star, and if it’s the wrong answer, they have to rethink it. It is like a memory-type thing. They have a small grocery store there, too.

We liked it, but we decided not to do anything at this particular point. We had lunch there, and the food was very good. The staff was very friendly. The rooms were small, especially for two, and they didn't have anything available in a two-bedroom, which is what we were looking for.

I visited this facility

My parents are in this community. They're all friendly, and it's very clean. They provide all of the meals, which are pretty good. The apartments are very nice. It is near a lot of places and shopping areas, and there's a bus that takes them shopping if they want to sign up to go at certain times. It's a nice area. They're finding things that interest them little by little, but they have a lot of activities.

I visited this facility

It seems to be a good community, and everybody has been really nice. My friend seems to be pretty happy. They have a lot of activities, and they seemed to be really caring about her. She has a one-bedroom, one bath, a combined living and kitchen area, and a little patio area. They have music that comes to the facility, they have Bingo, and they take them to shopping and doctor’s appointments. She said the food is pretty good.

My parents are here. The staff is very nice, and they seem to be helpful. They are in a two-bedroom apartment, and it is nice. They have card games, ice cream parties, church services, music nights, and movie nights. They also have free transportation to go to different places during the week. I like that it is pretty and inviting, the dining room is beautiful, the staff seems to try to be helpful, and it is pretty clean.

I think the staff is OK. They are knowledgeable. The rooms are nice. They are a little spacious. They are not the cleanest, but they are OK. They have a lot of activities. They have bingo, a weight room, and arts & crafts. They have a lot of stuff. I like that it is convenient and reasonable.

I didn't like the fact that you have to eat with the same people three meals a day. All the staff was very accommodating, and they were very nice. The rooms were smaller than some of the places I visited. The dining room was very nice.

I like this community. The facility is very clean, very bright, very well decorated. The lady who showed me around was very knowledgeable and very professional. On the time I was there, they had a King Fish contest and they also had a picnic lunch. They also take the residents to shopping, movies, and to the malls. The people are very professional. They had a beauty parlor there and different things; and physical therapy if she needed it. It seemed pretty complete.

I visited this facility

I thought it was a very nice place. The staff was very helpful, polite, informative and very accommodating. The food was good. The whole facility's lay out was beautifully done. The rooms were beautiful. My dad loved it and there's nothing that I did not like about it.

I visited this facility

I chose it because it's closer to where I live and after my mom had gotten out of the hospital for a surgery, they had put her on a rehab side at the Atrium and we liked it. Now she is in the independent living side of the facility. The staff was very professional and everyone is very, very friend. I think they are doing a great job. They have bingo, bowling, they have entertainers like singer and guitar players come in and there are different areas through out the building where there other things that they can do like play cards or make puzzles and they've got a physical therapy set up in there for them as well.

I visited this facility

I looked for a place for my mom and I visited this community. It was a beautiful and wonderful place. The staff were caring and helpful. The rooms were bright and spacious, and the hallways were easily navigate able. There were music, games, dining, they celebrate birthdays of the residents, etc. Over all, it was a nice facility.
