The doctor who was supposedly the staff doctor there showed up in his room once in three weeks my father was in there. Nothing was done for my dad that was constructive in three weeks while he laid there after a stroke. Doing so much better out of that facility. Don’t put a loved one in that place. The one shining light in that whole facility for sure was the social services worker that helped get people to the house once they kicked my dad out (a blessing in disguise). Getting so much better with the actual services these wonderful home health people are giving. All doom and gloom at Lakeview but when you have Dr. Kevorkian as the house doctor, you really can’t expect much else. Awful food to go along with the whole thing. He ate ice cream, pudding and apple sauce for three weeks and lost 13 pounds.

Worst experience. Many medication errors- poor care planning-/discharge planning/ rude staff. - every conversation involved money and not the care of the patient despite adequate insurance coverage. Families beware!

My wife went to rehab for stroke. Management was terrible. AIDS were terrible, rude, uncaring. 2 hr. wait for bedpan was not uncommon. Aids cannot identity meds being given. Most meals terrible. Stroke caused vision impairment. My wife asked for nothing to be placed on the left side of food tray. She could not see silverware. Spilled hot coffee on lap. Never go to this facility.

Do not put your loved ones here. The staff communication was extremely poor. They had not idea if the catheter was in or out when asked. The administration could not explain to me anything about what was going on, she said I will call you back the next day and guess what she never did. So once again keep your love ones away from this place.

My husband has been at Lakeview for almost 7 months -- in both rehab and long term care. The facility is wonderful and the staff is amazing. He is getting good care, good food and lots of attention. I am there every day and I continue to be impressed, not just by the care........but by the love and support I feel there every, for him and for me, too. I am so glad that we chose Lakeview!

I had a stroke and was transferred from delray hospital to this facility. The people were so nice and I felt totally welcome. The nurses are WONDERFUL. Any concerns I had were addressed immediately. The food was not the best. Most patients are whiny and don't understand this is a rehab, one must try to help themselves at times.

I visited this facility

I absolutely cannot recommend this place. It is horrendously bad. In fact, it should Be shut down. I wish they were being monitored for their standard of care more closely. Your loved ones are at risk if they are here. My father was a resident there after having a stroke. They did not give him and food or water for two weeks. They claimed he refused. He didn't refuse for me when I flew down to check in on him. He had an inability to swollen, which is Not uncommon post stroke.Pain medications are not given even though he was in excruciating pain. Dressings for wounds were changed maybe every third day if he was lucky. His Coumadin levels were not monitored. They said he was stable. You don't know that unless you do blood work to check it out. People were crying for help. A patient fell out of bed and was left on the floor. I had to help Them get back in bed. The place smells awful everywhere. The rooms are dark and patients are neglected. This is a sin. No decent human being should treat another human being this way. BEWEARE of this place!!!!!

I visited this facility

The first day there I visited my father after work and he had been transferred from a hospital near by that afternoon. I inquired about his medications. The hospital prescribed antibiotics to address the uti infection that put him in the hospital in the first place. And he had a host of other meds including dopa for parkinsons. The staff at lakeview did not have any of this information. I spoke with the on duty nurse and got the ball rolling, to actually send the fax of his meds to their pharmacy. He got the antibiotics started late that evening. other meds began afternoon the next day. And he had no pain meds at all (still recovering from a surgery). The next day I found out that he is BPR for pain meds. I asked the nurses to ask him. From then on he was getting something as needed. 2 full days to get back on track. Each time I visit, and on occasions when my mom visits, we need to bring something to the attention of the staff. The call button being on the floor or out of reach is most common, and I saw the nurse leave it sitting over the wall unit it is connected to once, and found it properly clipped to his bed but out of reasonable reach twice. We had a swarm of ants around spilled food once, and he was left needing to be changed for a long time as we repeatedly tried to politely inform staff. The staff I've met are good, polite, and attentive. There are around 6 rooms per staff and they move from one to another at a nice pace. Somehow, I think they just need more. My dad is totally dependant, and can't really feed himself either. he is on blood thinners, and had a tight stocking around his legs that may have contributed to a large bruise on his theigh. And he is expected to do PT with that tight brace. He is back in the hospital receiving a blood transfusion and we are deciding on surgery to address a hematoma in that bruised theigh. I am left to speculate. Why do I see committed hard working staff and yet there is always something not done or not done right? Why does it take so long to get started on meds when the transition from hospital to neighboring rehab is planned in advance? and why is he needing surgery from a condition that is suspicious in origin? I prey these suspicions are false, because if not my opinion will be solidified. This is a decent place for more independant people and anyone who can have daily family visits willing to help identify things.

I am/was a resident of this facility

I am a rn in delray beach for 30 years.Dad went in for rehab,pt,ot which was good,after a uti and gout in his knee.Dad gets less than 2 hours daily,and he is left in a wheelchair the remainder of the day which is counter productive.He has kidney failure,diabetes,oxygen,and bipap at night.Nurses are only their to distribute meds which they made more than 1 mistake,dad is independent at home,has all his faculties and knows all his meds.The supervisor did not write an incident report or question the issue,the nurses aid dropped a sterile catheter bag on the floor and attempted to connect it to my dad,luckily she did this in front of me!Dad just got over uti.The aides have way too many patients and cut corners everyday.I have seen supplies taken from one patients room to another,clean sheets put over dirty ones,used gloves on floor after they miss garbage cans,overflowing cans,dirty floors.Imagine leaving a paying customer or confused patient in this place?The lack of management to hold staff accountable seems nill.Basic standard of care is limited.I am their 5 hours a day to protect my dad and a private aide is there 4hours a day and my dad just needs rehab,but he is a human being and has other medical issues which they have fallen short of overseeing.You must show up for your loved one to oversee them.I would never recommend this facility.Looks good from a lay person who does not know what they should be doing,wrong.I believe it is just as easy to do the right thing as the wrong,do not cut corners with human beings lives,who entrust your facility with your lives.Patients should not have to wait until other complications occur before staff recognizes.Storage facility for the poor,sick and incompetent human beings.I pray for those people and their loss of dignity and for some end of life.Show up for your loved ones and hold all staff accountable!Know better,do better!

My wife's mother is a current resident, and has been for 3 years. This company is more interested in a patients money then their care. They have been cited for many violations year after year. Most of the staff do their best but their supervisors prevent inmates from getting real care. If you smell urine in the hallways not enough is being done, well, every time I visit I smell urine, The meals look OK, but at least for my wife's mother they did not follow the food guidelines, no rice, yet it was given anyway. I suggest looking up the record on the health code violations for this business before you place a loved one there.

this is the worst place to send your loved ones. they are short of staff and the food is horrendous and not nutritional. the caregivers are scare and your loved ones will be left sitting in wheelchairs when they want to lie down and rest. It smells awful throughout and it is like a prison with no way out. people crying for help everywhere and being ignored.

My mother has been in Lakeview for reahb a couple of times. This last time her kidneys went into failure and she was moved from rehab to long term care and placed in hospice. I did not expect her to survive Mother's Day weeked. It is September 1, 2016 and I am peased to report she is doing well. While I will not discount prayers answered, the staff at Lakeview is very caring and attentive at every level of service, and I beleive it has been their care which has been largely responsible for my mother's miraculous recovery.. The facility has had many recent upgrades in 2016. It is kept very clean and there have been NO incidents of any type of contagin that I am aware of. I work in the hospital industry and frankly even though Lakeview is operated by a competitor, they do an outstanding job. Why not five stars? The food could be better and dialing into the facility can be challenging at times. Overall, my mother is very happy at Lakeview and that make me happy and my life easier under difficult circumstances. I highly recommend Lakeview Care Center.

My sister just left Lakeview Care Center yesterday and I am very impressed with the caring staff there. She was a very difficult patient having schizophrenia on top of the physical issues that placed her there. Her nurse Katie went out of her way to make sure that my sister was safe when her discharge became very complicated because of my sister's noncompliance She loved the special activities they had there for the patients and the therapists were wonderful.

The only place he had stayed at was Lake View which turned out to be a disaster. The comb that they gave him had hair in it. He was supposed to be monitored 24 hours a day when I wasn’t there because he likes to get up and move around but they left him in a hallway strapped to a wheelchair in front of the nurse’s station. So he spent most of the time there, and there were still occasions where he fell out of bed. A few nurses had walked by and because he wasn’t the patient they were dealing with, he ended up lying on the floor one time for half an hour, and the second time for 10 minutes. When I addressed it to the main panel there, they said they would look into it but they never did because I couldn’t find out the name of the actual nurse. He was there a month for therapy to get his legs walking again and his talking back. The outside was the only decent part about the place. The inside I would say that it wasn’t filthy, but I wouldn’t say it was exactly clean either. The hallways were rugged, and the rugs had stains on them throughout the corridors. He was there for a month, and unfortunately when I got him out of there, it didn’t do much help for him that’s for sure. He always eats normally in front of the nurse’s station mainly because they didn’t want him getting out of bed, and they didn’t want him walking around. He was supposed to get therapy every day when he was there, but it ended up that he didn’t start getting the therapy on his legs until a week before he left. When I addressed that to the counsel, they had like this progress report, they said they start from top to bottom. His main problem was his leg at that time, but they were focused on his arms the first three weeks when there was no problem with his arms. During the week, there were two nurses for every patient, and during the weekend there’s one nurse for every four patients, so it was a ghost town for nurses. Although they had activities there and despite his mental status, they could still have gotten him at least if not involved instead of just being sitted in or leaving him in front of the nurse’s station.

The theropy staff is outstanding, but the nursing staff is dangerous and the administration is either in denial or totally oblivious. My mother came out much worse than the way she went in. They nearly killed her several times with incompentence and neglect. I don't have enough room to tell all the stories of the problems. If I did not open my eyes and mouth about the problems, my mother would have certainly died. When I complained, the administrators handed me off to other administrators. The facility looks very nice and the theropy staff is great. But do not go there or send someone if they require professional medical attention.

This is a beautiful facility, but looks can be deceiving! My mom is a special , being that she is a post stem cell transplant patient. She has many special needs that were not addressed! She has only been there since yesterday, and based on her non existent nursing care there, I am not too impressed! We sent her there with all her meds, and a binder with all her information (which was hers) and a prescription for pain meds. They took all the pages out and left her with an empty binder??? Then argued that the pages were not for her, but for them? She was improperly given a bed pan, and it tilted and caused her whole bed to be wet, and just left her lying in it for hours! She was not given any of her much needed meds nor gave her anything for her extreme pain! I am not happy right now, and I hope her 2nd day goes much better then her 1st! I am headed in there now to talk with the director.

My wife is at Lake View Care Center at this point. Their physical therapy department and their nursing staff impressed me the most. Their rooms are bigger than other places I've been.
