A Case Study in How Our Health System Fails to Meet Our Needs. June 20. My sister, 79, fell and broke her humerus near the shoulder. It was immobilized in an emergency room and she was sent home with her 70-year-old roommate. June 22. A follow-up coordinator called her but gave her no referral to skilled nursing or physical therapy, which the emergency-room doctor had suggested. July 6. On a “virtual visit,” she talked to a Physician’s assistant who suggested she go to a skilled-nursing facility immediately. July 8. Nothing in her health network was available so she paid for skilled nursing in a nursing home. However, physical therapy was unavailable to her there. July 22. That nursing home did move her to one in her Humana network on July 22, This facility is Four Fountains. [NOTE: 32 days after her injury] July 26. Occupational and physical therapy began. August 5. Therapy was stopped and she was told that she should go home. Someone did “walk her” on August 6, but she has had no therapy since then. August 10. The administration at the current nursing home has said that she was “transferred to Medicaid” and would begin to get therapy again. August 13. She was told that she didn’t get therapy because the insurance company thought it was her femur that was broken, not her humerus and that she might be getting it again starting Monday (8/16). She also had an argument with a nurse who told her she was too demanding when she wanted a drink of water. Please note that she has had no counseling, although she has a history of major depression and often describes herself as “feeling weepy.” She is unable to walk unassisted and is getting weaker by the day. She has received 9 (NINE) days of therapy in the 55 (FIFTY-FIVE) days since she injured the arm. She has to sleep in a recliner because she cannot roll over on the affected arm. She cannot put pressure on the injured arm and the recliner is impossible for her to get into once she has moved out of it to go to the bathroom or to try to tiptoe about in the wheelchair. I find her treatment unacceptable. Don’t you?

Four Fountains Convalescent Center was laid out well. It has a few separate patient wings, one area was for with low mobility and the other one was for the more active patients. The staff seemed to be adequate, and all the staff members seemed to be professional and caring and they do a good job. I saw the dining area, it was just a large and open area in the building with tables and chairs. The room I saw there was small. The location was fine, it was near a major highway so getting into it was pretty easy. My sister was a resident there before.

Nurses were busy bodies and at night they would not give my relative their cell phone nor dial it so they could talk to me. A nurse a male nurse moved my relatives leg and nearly broke it they were very uncomfortable in that home. The only thing glowing about that home was the fact that people got a bath and got their hair done but the staff I had to get a DNA test to prove I was related in order to spend time with my relative

I visited this facility

Worst nursing home my mom almost died dehydrated pneumonia among other things if it wasn't for my sister who demanded my mom be sent to hospital we would have lost her. I wouldn't trust a dog anymore to them.

I visited this facility

Stay away from this place. My dad went there on a Tuesday and on Thursday he hadn't been bathed or teeth brushed. I tried calling the main number several times and often there was no answer. He asked an attendant for a bed pan while my sister was standing near by and the attendant walked right by him. My sister told the attendant my dad had asked for a bed pan and the attendant said, "Oh, I didn't hear him." They waited for the bed pan then my sister went and told the attendant who was talking with other attendants that they hadn't gotten the bed pan. The attendant told her to go see another attendant down the hall. My sister did and asked for a bed pan and then attendant said she'd be down with it. The attendant brought a container of ICEWATER & STRAW! My sister said they wanted a bed pan. The attendant said, "Oh, I'm sorry I forgot." My sister also watched for over 15 minutes an unmanned drug cart filled with drugs just sit out in the hallway with no attendants around. We got our dad out of Four Fountains immediately!

The nurses are OK at Four Fountains Convalescent Center, but the CNA's aren’t that great.

I visited this facility

Four Fountains was a nice facility. It's clean. Some of the residents were sitting around, but they have activities. They have an activity sheet posted where they have different activities that they offer to the residents. Some of the rooms were better than others, but all of them were clean. Some of them need improvement, but they are doing some improvement. The staff was very cooperative.

Where to begin...i can not say 1 positive thing about this place....our mother with dementia broke her hip and had to do rehab here...a nightmare from the minute the medical transport stuck her in her room and walked away...No. way should our elderly in this country be treated with such non-caring attitudes. My sisters and father an i stayed round the clock because we did not trust that the could keep her safe...speaking of staff there were far too few ofthem...then lets talk food..one evening for dinner she was brought a balogna and cheese sandwich...if you are considering this facility please stop and think twice about your loved one...we had no choice at the time but want others to know...by the way the only reason i gave one star is because sight won't let you leave it blank....

Wonderful staff. Wonderful care. Thank you to everyone who got my mom home safely to me. I've been in a lot of facilities in my day doing volunteer work. The staff here are absolutely wonderful to their residents.

One of my grandparents was a patient at this facility and the experience was not positive when dealing with placing an elderly member of your family in a facility. The staff to patient ratio seemed to be very low as we had to have a family member there most of the time. When we first arrived, there was blood matter on the floor and the staff did not seem so concerned about getting this off the floor which was a red flag. In addition, my family member was sent to this facility from the hospital with a respiratory device and the staff did not even place this device on the family member until we demanded this as her breathing was quite labored. There also seemed to be 1 nurse on duty in our hall and she seemed to be overwhelmed with the needs of the patients. This facility could use more experienced staff at the very least.
