I am a friend or relative of a current/past resident

I cannot stress to people enough to avoid this facility. The first thing you notice when you enter the building is the overwhelming smell of urine and feces, and residents left in a living room area with a TV that is either not on, or is left on the main Roku menu. The medical staff is generally rude and cold, aside from a couple of caring CNAs, and all of the therapists are always very polite and helpful. The nurses we have encountered are not very friendly and treat you like a bother for asking for any help or updates regarding your loved one. In the cafeteria during lunch, it seems that much of the CNA staff are sitting around on their phones instead of helping residents get settled in for lunch, getting them coffee, etc. The cafeteria is always dirty, with floor spills not being cleaned up, dirty tablecloths on the tables, and a general lack of cleanliness. The food is your basic cafeteria food, which is to be expected. The cafeteria is uncomfortably loud with the TV or music blaring in an already acoustically loud, echoey room, which makes it almost impossible to visit your loved one without talking as loud as you can, further adding to the noise. The rooms are in a similar condition, with sticky floors, dirty patient tray tables and disheveled patient bedside tables/dressers. Other than therapy, the patients do not seem to be engaged in any kind of meaningful conversation or activities. We have seen a major change in our loved one since bringing them to this facility. Keep searching, this is not where you want anyone you love to live.

I visited this facility

I would not recommend putting your loved one in this facility. My relative is currently there and has experienced a noticeable decline in the several months that they have been there. The majority of the staff is not friendly or helpful. They will often argue with you to avoid doing their duties when your loved one simply needs help with things like going to the bathroom. Instead they will leave them there to sit in their own mess. Overall, this facility is not very clean and is in a general state of disrepair. Large areas of the walls in my relative's room are literally falling apart. The floors throughout the facility are sticky and seem to have not been properly cleaned in a long time. The smell of the place is REALLY bad. We had another relative in a different nursing home and the smell was not nearly as bad. I often become nauseated just walking into the building.

My mother was sent there to regain her strength, but she actually regressed. The rooms were dirty, stains on the furniture and bedding. The furniture was falling apart or missing all together. There were condiment packets that lived under the bed. Equipment did not work or was inappropriately repaired. There was two aids and one nurse that showed any kind of care or respect for my mother. Most of the time the staff swooped in, performed the minimal service and rushed out without even communicating with my mother. She would be left for hours in a wheelchair away from the nurse call button with her feet dangling without the foot pads on her wheel chair. The food was unappealing and she was often given drinks with no straws. The communication with the facility was practically nonexistent. One time I called in and a resident in the Memory Care wing answered the phone. There were always promises of care, answers, services and nothing was no follow-through.

Onion Creek Nursing and Rehabilitation Center is well organized and the nurses and staff are real friendly. The layout of my mother's room is just like a normal room for a nursing home, but it's clean and well-kept. The meals looked well prepared, very good, and nutritious. The common area they have there is like a living room/TV room. It is well-kept, very clean, and very organized, too.

I am/was a resident of this facility

I was admitted with Covid-19, supposedly. I was never tested for it.I shared my room with a very pleasant gentleman. The room was never placed in "isolation" for my Covid and signs pointing out the disease were only placed on the door the one day inspectors came through the facility. After some time with the Covid diagnosis, doctors came in and said I had, instead, MRSA! That had to be impossible.I never left my room for therapy and never went 6o the dining room to eat. BTW, the food was crappy and the portions were minimal. The staff didn't know that I spoke Spanish and bad-mouthed me while they were in my room. The entire time I was in that place, I never participated in ANY activities. I have no ideas about any amenities since I never left my room. This might date me somewhat but I refer to this facility as Shady Pines (A reference to Sohia's rehab place in "Golden Girls"). It's been two years since my time there, so things might have improved. I would strongly advise others to avoiod this facility.

This facility looks like it is 30 years old.. The carpet in the hallways are filthy. Residences are not separated by Nursing Care and Rehab. The food is awful and I would NOT put one of my family members here. I have a neighbor/friend currently in the Onion Creek facility and she is roomed with an elderly lady that doesn't even know where she is. She is out of it and lays in the bed and/or wheelchair in the fetal position all day long. Rehab patients should NOT be in the same hallways/rooms with Nursing Home patients. This is an awful facility

The staff doesn't "do " anything. They dont even change bloody sheets. No wash cloth, pillow cases. Put trays down and walk off. They don't check to see if patients are eating or not. Instructions have been given repeatedly regarding my mother's care, nothing gets passed on to the next shift. Diaper was so full of urine on Day 1, it was dripping out when the family went to change it. Care could be better. When these concerns were brought up with the administration, they basically told us it was the families responsibility

I am a friend or relative of a current/past resident

I really need everyone to know how horrible this facility is. We were looking for a place for my mom after she suffered a stroke. This was not going to be a permanent stay and my sister and I were not going to leave her side during her stay. When myself and my sister visited the facility they were all loving and went out of their way to make it sound like it was an amazing place. My mother was there in December of 2015 and I have now been able to finally come to grips of righting this. I have been going thru a really rough time after my mother's passing. My mom was only there for two days and I was with her every minute and thank god I didn't leave her there by herself. I would call over and over again for help at night because I never left and seriously it took 6 hours sometimes for someone to come in. I can't imagine if I was not there how long she would have laid there with NO help! Please do not leave any family member there!! There nursing homes need to be held responsible for how our elderly are being cared for. This is a very serious problem and my heart breaks for them.

I visited this facility

Everybody was nice at Senior Care of Onion Creek. I saw one room filled with bedridden people, it was not too big. There was an odor in the hall because a patient had an accident. But they put out fans in the hall. They have several Alzheimer's or Dementia patients.
