DO NOT USE THIS FACILITY! If I could rate a negative stars I would. The food is horrendous, staff is not attentive or focused. Reports are not written accurately.

I am/was a resident of this facility

I was a resident for 2 days. The room look like a room from a mental health center. No pictures on the wall..a nursing button from the 80s. Rang the button after I had a bowel accident... laid there for over an hour before a aide finally responded. Asked for a nurse to help to be told it was not his job. Could not get the help I needed and had to call 911 to get out of there.

Visited my grandmother in this facility. The staff is friendly, it’s not a new building however it’s clean. My granny didn’t have any issues or complaints

The facility is clean and odorless. The staff are friendly and inviting. When I visit my family the residents look clean. There is always someone doing hair and nail care.

I visited this facility

my relative was altamonte facility and I observe terrible care for him,staff rude and not caring at all for residents and very little effort for residents and family. I watched my family member suffer and staff not caring for him and other residents. adminstrator and nursing staff non caring and unsupportive. I visited 3-5 times a week and watch him die slowly an was placed for short term care after stroke and cardiac issues. personal items missing constantly and request were given to staff and ingored. patient suffers no respect nor care given. family treated terrible when ask for care for him. request issue to help him and declined. he was admitted with rehab in mind never given and force to within 2 day admission to void on himself, no call light for him to reach, facility outdate, family and friend treated terrible. no interaction by staff, no activites for bedside patients, temp in room was set 66 degrees all the time , no adls given to patients, no snacks or fluid offered to residents, med errors, my heart broken watching patients suffer and no smiling, no caring or smiles. I will never return there or recommend .no linen to care for patients were available, no decorations for holidays, nurses stations old and dirty and outdated, phones not answered in evenings, food and snacks were left for him to have and staff would no offer them to him. I was there from 2018-2020 worse experience ever. cna in tv room for day and evening and just sit there on cell phone and watching tv with no interaction with patients and not offering snacks, drinks or interacting with patients-sad. I will never go back and please do not admit anyone there- read survey posted in lobby--poor care, stay away. they like banning anyone who makes complaints regarding care. ZERO STARS for me and worse experience in my life . he walked in ambulatory and within in 1.5 yrs died skeleton and sores all over and no air mattress, food let dry on face and no care. he suffered and died at age 57. please do not admit your family or friend to here. i watched patients negleted , poorly staffed . one day he never assigned cna and i discovered by checking staffing scheduled and him and his roommate were not cared for 6 hours till I check staff assignments and i told them about issue , roommate never changed and meal tray left hours and not awaked or assisted to eat, meds left at bedside and found on floor in hallway, found bed saturated with urine and yellow stain, no supplies , no nail care, clothes missing over thousand dollars, electric shavers broken and parts missing and some times thrown away. after hours staff ingore door rings and attitude from staff to let you in to visit, nursing assessments skills weak and do not care when asked to evaluate resident . I would go home broken hearted and depressed for leaving him and other patients in poor care. Staff need more training, caring attitude and help. Management not caring , rude and concerns not addressed.

Run down facility, but clean. Okay food. Not enough staff to help and most are rude and neglectful. Got some of my clothes missing and my favorite sweater went missing the first day. Physical therapy was the only halfway decent thing here, if you could survive everything else. I chose not to be vaccinated for Covid and that is why I got sent to this low rated facility; apparently, decent facilities don't take in Covid unvaccinated people even if they test negative and been observed at a hospital for unrelated issues. Meanwhile, their staff are vaccinated and have Covid and are still forced to work.

I visited this facility

My husband is there through the VA, they don't use the best places unfortunately, my husband hasn't said much so far, think he mostly stays in bed but, I went there to deliver a Thanksgiving meal to him and didn't have a clue what I doing, have never been there. Anyway, out side the door there were 2 buttons, did not know if they were doorbells or not, the place is pretty shabby and run down some woman, small dark haired probably a nurse came to the right side of the doors and and turned around and left so I knocked on the door, she turned around and literally screamed at me telling me that she had to get a code, I did not know if she saw me, the windows were tinted fairly dark and she gave no indications that she had...When she opened the door I asked her why she was so nasty, I hadn't done this before so I did not know what I was doing, no real answer except that she saw me, left with a bad feeling, hope she is treating the patients better than she treated me...

I am/was a resident of this facility

Was unfortunate to be placed at this facility following knee replacement. Outdated crank beds. String tied to bed to turn on and off lights. Physical therapy done by a man who wasnt even trained as a PT aide. Ice packs brought either in a rubber glove, or large plastic bag not tied up. Ignored when signaling for help. Broke the ice circulating machine sent by hospital. Physicians assistant told me to stop taking all medications my primary care doc and endocrinologist had me on, that after seeing me for less that 5 minutes on day 5 of residency. He spent more time adjusting his clothing which looked like he was preparing for vacation. Attacked by lady sharing room, and called my hubby in the middle of the night for help, since no one responded to the button pushed for help. I could go on but it's just more atrocities. This was I found out months later, at the time listed as the next to the bottom of the worst facilities in the state of Florida. It is a nightmare. The second room I was in, had a lady in a COMA. She was attended to once a day, when they changed her diaper. It's in my opinion nothing more than a cattle herding warehouse. Again IMO it should be avoided at all costs. If I could give it negative stars I would.

I visited this facility

There was no rating low enough for me to rate this facility! Do not, I REPEAT, do not go to or send your loved one to this facility! I cannot stress this enough! In answer to the questions in the sidebar...Rooms are so, so. No phones. Interactions with staff? Upper staff okay, floor staff terrible (don't do their jobs correctly, are snotty, and just don't care!) Activities? What activities? Same answer for amenities. Well, I guess if you call a wall (you know like in a motel) air conditioner an amenity, I guess I'd have to give them that. Are they so poor they can't afford central heat and air for their patients?

I was at Consulate Health Care for three months. It was a nice place kept clean, but like every nursing home, I think there were people working that shouldn't be working there. However, there were other people, who were wonderful. I did get good occupational and physical therapy. I had great therapists. I was able to learn to walk, get up, and move again. I was able to go home instead of staying there longer. They wanted me to put in assisted living, but I went straight home and I did well. I had a private room; it was a good size room. The nurses needed more training. They also needed to have more nurses instead of certified nursing assistants.

This facility allowed my sons' father to be "assisted in leaving this world" today. His Dad had Parkinsons' (we knew that) and insisted (per my ex-husbands current wife) that he was "last stage Parkinsons" which was FALSE !!!!!!!!!!!! 2 days ago he was sitting in a wheelchair breathing fresh air with our son outside and today when my son got to the facility to care for his Dad, the room was put on HEAT and was 77 degrees and his Dad had blankets on him which raised his body temp way too high. We do not know is his Dad is DEAD because this facility barred my son from coming back to care for his Dad. The staff could not have been nicer to my son until he requested an x-ray to see if his Dad had a hernia which is common in Parkinsons' patients. The wife said "Absolutely Not" and today he was being "assisted" with the knowledge of the director and others not known to us to "HELP" him out of his misery. We hope the people involved in this HORRIFIC facility can sleep well.

This facility is poorly run. Not enough staff, old, outdated and depressing. I truly feel sad and terrible for people that live there that do not have family or friends to look out for them. I personally heard staff members discussing patients private matters and talking negatively about them. I had to practically beg to get someone to bring fresh water to my mother. I had to search for someone multiple times to get help for my mother, to change her and clean her up. I hate to think how it would have been if I was not there as often as I was. She was there for 2 months and she will never go there again. Yes, there are good employees that I believe truly do care, but not enough of them to do everything and make up fpr the others. I found much of the staff to be lazy and very rude. Please do your research and consider another facility unless you can be there all the time and question everything! Rooms have old crank style beds and no tvs. CNA thought is was perfectly acceptable to bring my mom out into common area with open back gown on and gave me irritated look because I called her out on it. Too lazy to put proper clothes on my mother. Never again!!

Do not choose this Rehab facility, again stressing do not choose this facility. This facility was recommended. They insisted they had certified wound care nurses on staff. It has been 3 days , here colostomy bag has leaked 6 times. When the nurse goes to attach bag , they start asking questions on how to do it. They don't know what they are doing. Come to find out they are not certified. Her skin is raw and it is leaking again. They still do not have all her meds approved , going on three days. Also, they do not follow protocol on administering medicine. She rings the nurses and they never come, then when you question them they lie. Their food is disgusting "quoted from other patients." The facility is dirty, her roommate had a banana peel in her bathroom for 2 days. Nurses are worried more about chatting then adhering to patients.

I visited this facility

Shortly after my mother went there post surgery, her blood pressure started dropping and kept getting worse. I discovered, with the charge nurse, that the facility was giving her double the dosage of blood pressure medicine than she was supposed to be taking. Rather than have their doctor some to see her, they simply stopped giving her blood pressure medicine. After 5 days, she saw her surgeon who immediately put her back in the hospital. Turns out, her kidneys were failing, her blood count was low, her oxygen level was low and she was severely dehydrated - all because they were giving her double her dosage of blood pressure medicine - by accident. Now they seem to be trying to cover this up. My mother could have died under their care. Don't let it happen to yours.

This facility is truly awful. My parent went in for a few weeks of therapy after surgery, ended up in worse condition and had to be readmitted to the hospital. I honestly do not want to give too many details because the condition of the patient was so upsetting, but want to say we are not just upset family members. It was the opinion of several doctors that saw my parent upon readmission that had the patient received even a minimum standard of care, this would not have happened. If you have a choice and love your family member, choose someplace else. If you are not given a choice by their insurance company as was the case with us, proceed with caution and be sure to keep a close watch.

Consulate Health Care of West Altamonte provide three good, healthy meals a day. The offer therapy. She is happy there, and she does not want to move anywhere else. Everything is outstanding.

Consulate Healthcare is dirty. Though most of the staff is good and caring but others are now right rude. The rooms are segregated. If you are long term patient you get moved to a particular hallway and get inferior care. P.S. The food is so bad it's beyond recognition. It is a good place to take someone to die.

I visited this facility

Consulate was good. They have good meals there. They have good activities for the residents there to keep their mind active. They keep the facility really brand new looking. It might be 15 to 20 years old, but it is still freshly painted and it is still beautiful on the inside. They have a nice aviary. Other activities that they have there are bingo, they have exercise with music, and they have ball toss. They have entertainment from singers and chorus group.
