Concerned Home Managers for the Elderly, Inc. (COHME)

COHME is a small agency comprised of licensed social workers, home health aides and personal care aides. Social workers supervise the staff aides and provide referrals to local resources. A comprehensive care assessment is the first step in formulating the care plan that is tailored to each individual client. COHME staff focuses their efforts on preventing premature institutionalization.
