LivHOME

LivHome, formed in 1999, provides daily log keeping, frequent visitation to clients and monthly in-service trainings for caregivers. Staff members work together to address complicated family dynamics, dementia-related issues and psychiatric challenges of clients. Every staffer—even the intake professional who fields initial calls—is an integral part of the team. Care managers tailor and supervise care plans, and join caregivers at least once a month for home visits with clients.
