VNA Health Care In-home Cardiac Care program provides treatment, education and rehabilitation if you are recovering from cardiopulmonary disease. Together with your physician, a team of cardiac nurses implements a care plan based on your specific health needs and individual circumstances with the goal of achieving the highest level of wellness and functioning and reducing the likelihood that you will need an emergency room visit or hospital admission. Geriatric Care Management
A VNA Health Care Nurse Care Manager conducts a comprehensive, personal assessment and then develops a plan of care designed to manage and identify all the services that you will need to maintain a healthy and independent life at home. Home Care Aide

Meals on Wheels
Nutritious, home-delivered meals help people who are unable to shop for groceries or safely prepare their own meals to maintain independent living at home. Our Meals on Wheels provides up to two meals a day to anyone who can benefit from the service regardless of age. The nutritionally balanced ready-to-eat meals are delivered right to your door and can be specially prepared to meet any medical or dietary requirements, including kosher, diabetic, low-salt, or puree.
Home Health Telemonitoring
The telemonitoring program gives you and family members the peace of mind that nurses are watching your condition around the clock. The program uses an in-home telemonitoring system to monitor your vital signs and other health indicators. A nurse will intervene at the first sign of health concerns and thus reduce the likelihood that you will need emergency room care or hospitalization. Hospice

LifeLine Personal Emergency Response System (PERS)

Nutrition Services

Personal Care Attendants
VNA Health Care Personal Care Attendants (PCA) can take care of your daily grooming, housekeeping and day-to-day needs. PCAs can assist with therapeutic exercise, shopping, bathing, dressing, meal preparation and eating. They can be available for as little or as long as needed up to 24 hours a day, seven days a week. Wellness Clinics and Education Programs
VNA Health Care reaches out into our communities with a variety of child, adult and senior clinics offered throughout the sate, including: blood pressure, cholesterol and diabetes screenings, nutrition and development counseling; and flu immunizations. I am a current client of this provider
I am my mother's conservator, and we've been using the Ridgefield Visiting Nurses for two and a half years now. I will just call them and they will take care of everything. They provide a lot of volunteers as well as paid service for us. The staff is very incredible and outstanding. My mother has a registered nurse that comes in for two hours every other week and is very professional and quite competent. She has a flat fee, and it works for us.
However, the volunteers are better than the paid staff. Mom has a volunteer assigned to her for the last six months who is her driver and takes her to doctor's appointments and the pharmacy. She also has another volunteer who visits once a week and takes her to the store. They never ask for payments and go way beyond as volunteers.
