This hospice group took care of my Father as he is suffering from a terminal illness. His prognosis was 6-12 months upon discharge from the hospital. They provided hospice care for my Father for around 8 months and in spite of my Father's continued decline (albeit slow), they discharged him from their hospice service and gave us only 24 hours notice. They informed my Mom and sent nurses out the next day to close out their care. They provided a list of Home Health care providers that was seriously dated and there were no providers on the list who could take my Dad as they were either no longer doing home health care or they could not take on new patients. They provided no support and no effective discharge plan. My Father was left with a three week gap in care while my Mom scrambled to find in home health care for my Father. During this three week period they did not check on my Dad once, in spite of their promise to provide support for my Dad until Mom found new in home health care. The new Home Health Care service is now trying to move my Father back into Hospice as the new physician and multiple nurses have evaluated my Father and have determined that he belongs in Hospice. All have indicated their surprise that he was discharged from hospice. This organization created tremendous amounts of stress for our family and they certainly did not exemplify their Lutheran based ideals that they tout on their website and in their literature. My Father's care and well being as well as that of my Mom in our experience was not their priority or their primary concern. Communication is also very poor within this organization and between this organization and the family.

Seems like they spend more time on paperwork than with their clients. Communications isn't great.
