P.A.T.H.S. alterative to nursing homes and assisted living facilities. We specialize in caring for those with Alzheimer’s, dementia, and the medically frail. We accept all stages of disease and provide long term care, including palliative care and hospice.
Our excellent staff specializes in memory impairment, the medically frail, as well as end of life, palliative care and hospice. Quality of life is of the utmost importance, every attention is paid to physical comfort, emotional well-being and everything in between. We offer “total complete care” (call for details), flat fixed rate available and we bill directly to long term care insurance.
Complete care is offered at a fixed rate: this rate will not be adjusted regardless of your loved one decline or increased needs. This allows for estate planning. releving family members from feeling taxed, stressed and burdiened.
We relieve family from tasks, allowing your time visiting to be about “your loved one”, and not their needs. We take care of Doctor appointments, coordinate with other health care professionals, home health agencies, visiting nurse RN Nurse assessments and Nurse Delegation for Medications. We coordinate with Occupational and physical therapy.

amazing just amazing care staff and facility “With a stellar reputation preceding her, Mariam Zinn and I met at a time when I was experiencing grief and loss. Change is never easy. Mariam was able to take the lead and walk me through the process and steps I needed to take in order to move forward and I cannot thank her enough. I've seen firsthand the exceptional and conscientious care and attention to every detail Mariam gives the people she works with.
