I am/was a resident of this facility

This is a long term manged care provider. They are completely unorganized. I had two nurses show up saying they were both my grandmother's nurses neither one knew that the other had visited. I asked them to make a contract with the medical supply shop that we having been using for years that was not under their"managed care provider list" and they said they would yet no-one did. A physical therapist showed up without warning or calling that never came back who was sent by Elderserve to do an eval; I guess no-one thought it was necessary to contact me. They have list of which doctors you can or can not use majority of them do not do home visits (the whole list of podiatrists in Brooklyn not one of them do. and My grandmother's regular podiatrist (who does home visits) was not on their list and had to make a deal with not to charge Medicaid so he could continue seeing my grandmother.

If you or loved one are being forced to switch from short term to long-term care (as everyone is with the new Medicaid cutbacks) do not enroll in Elderserve! We are being switched to a better and less controlling long term agency ICS in January.
