Matrix Team Home Health Care

Matrix Team Home Care is a division of Matrix Advocare Network. Matrix has been in business since 1987 and we pride ourselves on our highly educated and experienced Registered Nurse Care Consultants, and our team of well qualified caregivers. We specialize in helping clients maintain their functional status, independence and dignity whether they live at home or in a supportive care setting. Our specific approach provides each homecare client with specially selected caregivers who are chosen specifically for each client. All caregivers are Matrix employees and are carefully screened and fully trained to meet each homecare client's needs.

A licensed RN develops a Plan of Care in conjunction with the client and family, and supervises the caregivers closely to ensure that homecare services are provided skillfully, respectfully, and safely. Matrix Team Home Care ensures you can stay home with competent, consistent, safe, honest, and professionally supervised homecare services designed with your needs in mind. Services are provided based on your needs and only with your approval. An RN is on call 24/7 for urgent needs. RN services are provided in visits of about one hour or in visits up to 24 hours per day. LPN, home health aide (HHA) or homemaking/companion services can be provided from 4 to 24 hours daily.
