Renaissance Senior Care Management

Geriatric Care Managers (GCM's) work with older people and their families to enhance quality of life, encourage independence, and provide a safe and supportive living environment.

We provide the following services:

• Assessment and evaluation of the client’s needs
• Development of a detailed, comprehensive, care plan
• Consultation with family members or other caregivers to formulate, implement, evaluate, and revise plans as needs change
• Ongoing care management, including researching options for care
• Arranging of services and monitoring of services
• Liaison Services with Physicians, legal advisors, and other professionals
