Services Include (but not limited to): Care Management, Professional Guardianship, Health Care Surrogate, Trustee, Crisis Intervention, Family Liaison, Transitional Moves, Long Term Care and Assisted Living Placement, Advocacy in Facilities and Medical Appointments, On-Going Support and Supervision, Continuity of Care, Bariatric Care, Medicaid Planning, Annual Health Insurance Reviews and Enrollment, Pre-need Planning, Bill Pay and Monitoring, Referrals to other Professionals, Special Needs Travel, and Senior Enrichment Vacations.

All employees are bonded.

General Rates

Hourly Rate: $55-$100/hour
Assessement Rate: $1,500
