Constellation Geriatric Care Management is under the direction of a Geriatric Care Manager who is a registered nurse and certified care manager (CCM). She also has fifteen years experience in provision of direct geriatric care management services, in addition to nursing, direct care and management experience in varied settings serving elders and their families.

In coordination with Constellation Home Care and Constellation Private Duty LLC, the Constellation Geriatric Care Management program brings a comprehensive array of professional experience and expertise in elder care and issues associated with aging. The focus is on comprehensive assessment of individual client needs, which includes assessment for Medicare and Medicaid and other means of paying for care; family support and community resources, housing and referrals to other professionals and appropriate care providers.

The Geriatric Care Manager formulates individualized, client-centered care plans with recommendations and options for short term and long term care needs to help elders remain safely at home as long as possible, minimize crises and ultimately bring the elder and family peace of mind and confidence for today and the future.
