Our team of Professional Care Managers consist of seasoned social workers and nurses as well as Certified Care Managers and Certified Dementia Practitioners. Our service area includes the Tampa Bay Florida area (Pinellas, Pasco, and Hillsborough Counties)
Our Professional Care Management services include comprehensive assessment, individualized plan of care, consultation, professional recommendations, implementation of services, oversight of care and treatment, healthcare advocacy and education. Having Certified Dementia Practitioners on our team, we specialize in those with Dementia or Alzheimer's.
We provide consulting on Medicare, Medicaid, insurance and other benefits such as VA. We perform annual reviews of Prescription Plans and will advise you of lower cost plans available. Our bill paying services include balancing books, writing/reading for visually impaired, paperwork organization, budgeting, disputing bills, and mediating negotiations.
We offer placement guidance, transitional assistance and relocation services. We can arrange for home care, community services, attend medical appointments, arrange and oversee home repairs, offer appropriate resources and be a local point of contact in case of an emergency.
