Health Advocate Partners is an independent case management company focused on assisting elderly individuals to receive the highest quality of care and personal satisfaction as they navigate through the continuum of health care. We act as an intermediary between the client, the family, the medical team and other professional liaisons involved in the well-being of our clients. We specialize in the following:

• Communicating the patient’s health status and the physician’s plan of care to family members – especially those living out of town, which reduces the frequent phone interactions between the doctor and the family and increases overall satisfaction
• Guiding the patient to follow the doctor’s treatment plan, thereby improving health status and reducing hospital admissions and costly readmissions
• Coordinating health services including office visits, diagnostic testing, home health, etc.
• Procuring and delivery of medical supplies and equipment
• Resolving medical billing issues
• Assessing the client’s living environment and helping to transition from one level of care to the next
