I was on the phone with my friend for 40 minutes and she had to go to the bathroom and no one came in to help her on the bed pan she was ringing and screaming out in pain because she has a fractured pelvis and can't do this herself. When the nurse finally came in she yelled at my friend saying she didn't wait that long meanwhile I was still on the phone listening to her. This was on the 2 floor of your rehab. And now the nurse is saying they don't have any clean sheets so my friend has to lay in a wet bed.

Do not evaluate this facility based on their website or reviews of Independent and Assisted Living floors. I see they say nothing about the nursing home floor, and no photos of that floor or people would see how bad it is. The independent and assisted living floors are much better than the nursing home floor, #3. High staff turnover since new management took over, call bells not answered for up to 30 minutes, staff seems bored, no one answers the phone and the dining room is a disgrace, with broken chairs, staff shouting, a tv blasting and more. Wish I could move our relative somewhere else.

Came to The Watermark for post-surgery rehab because I thought a residential rehab facility would be best for my short term rehab. I arrived at 12:30PM from Pennsylvania Hospital via emt escort. There was no indicatin that they wee expecting me or what my room number was and only the emt people found out the room I was assigned and dropped me off. I received no greeting, no visit from any admission person or any sort of visit from any healthcare professional for seven hours. I was left in the bed and the only contact I had was from a person who set up the TV and installed a new house phone. I was refused a walker and a urine bottle and persistently throughout these seven hours was told I had to be evaluated first before anything could be assigned t me. No one would bring me a walker or give me a urine bottle or even help me to the toilet. When a nursing assistant got involved after I threatened to call 911 to get me back to the hospital , she said the evaluation would not happen until Saturday at best. Several staff people who I flagged down by yelling for them as they passed my door and the "ambassador" who said he was there to ensure I got all I needed all refused to get me a walker or a urine bottle. They said they would check and either told me I could not leave the bed or just never returned to my room. I threatened to call 911 and a nursing assistant told me not to and that my evaluation would not take place until the following day at best. I had to use the rolling meal tray to wheel myself to the bathroom a good twenty feet away. I almost fell. Finally at 7:45PM a nurse came to take my vitals and she was shocked that I had had no one visit me before her shift started. She procured a walker and a urine bottle for me and took my discharge papers with the medication list, promising to return it to me. She never returned. She did manage to set up some of my meds such a heparin and senna but nothing else. Luckily I had my own gabapentin and lorazepam. I was distraught and felt abandoned. I slept but woke at 5:30 am waiting for someone to come in and at least acknowledge that I was there. A woman who told me she was my "ambassador" for the day and an agency worker to ensure I got my breakfast and all I needed. She never came back. I waited until 9:45AM and as no breakfast was delivered, I used my walker to go to the door and tried to get kitchen staff to get me breakfast. I might as well have been speaking French, The day nurse did respond and procured some scrambled eggs and a tiny muffin which she microwaved and a banana and another kitchen worker was kind enough to get me a coffee from another floor. Still no word about the evaluation or any sort of indication that anyone knew anything about my care, other than my meds, I did not feel that I even existed. I realized that it was less than 24 hours and just wanted out of there as I felt unsafe and abandoned and did not want to be there at all. A friend came to rescue me and take me to his house amid some drama and a sudden outbreak of attention from people who I had never met who all apologized and claimed they did not know that I was so unattended to. I left and will never go back there. I am safe now and will undergo in-home nursing and pt care at my friends' home. No one should eve go there.

The Watermark at Logan Square was very nice. I thought the apartments were really small. I didn't like the fact that they didn't have a pool and it was a lot extra per month to park in the garage, but they were all very friendly and nice. They gave me a box lunch to-go and it was alright.

My Mother entered Watermark Rehabilitation Center after having a serious surgery. They ignore the patients for lengthy times, they talk to the patients disrespectfully, they don't have her proper medications, and someone is always passing the buck. I am in fear for my Mother's life forcing my siblings and I to remove her from this facility and rehab her ourselves. I would absolutely not recommend this facility to anyone, and if you do have loved ones there please check on them , they have patients that scream for long lengths of time with no response I've timed the time that it took them to respond to my Mother ringing her call bell and being HONEST they never showed up reason being they have other patients.

My brother is at The Watermark at Logan Square right now. He went from rehab to assisted living there, and we're happy with it. There's not much I don't like about it. With Covid, they're extremely diligent about keeping things clean. They're handling his meds and doctors' visits really well. He falls a lot and he needed more coverage than they were able to give, which is a reasonable request. We hired somebody for him during the day. They visit him at night, but they're really extremely supportive. He fell three times in one day because he would get out of bed, and he would not remember to press the help button on his wrist. They serve his lunch, and they would leave. He would get up without letting them know, so I place absolutely no responsibility on them not being there. I think it's a great place. He likes the food. He feels very comfortable there, and they don't rush him either. They're introducing him into the population, as he wants to be introduced. The marketing department was excellent. When my brother fell three times in one day, the nurse on the fifth floor researched what my brother's condition was to see what other possible symptoms or conditions may occur. When I met with her Monday, she knew everything about the disease. It's a progressive supranuclear palsy, and she took the time to research it, to figure out what would be best for him. She called me and said his condition and he could fall any time, so we just have somebody there during the day, and it's worked out perfectly fine since. They take care of him, and they're always friendly to me. For instance, just today a speech therapist who is independent from The Watermark was with my brother. She wanted to start speech therapy with him. She told my brother that he needed to get a prescription from the doctor at The Watermark. My brother called me and told me, and I called the head nurse on the fifth floor. The nurse got absolutely upset and said that's not my brother's job, and she's going to address the speech therapist for not doing her job. All the speech therapist had to do is go down to the doctor's office and say my brother is his new patient, and he needs a prescription to start his speech therapy. The head nurse said, "Don't worry about it, I'll take care of her right now," (and I'm sure she did). They're good. My brother said, "I feel very pampered." It's clean and organized. They have a monthly calendar outside the elevator, and there are two or three things going on every day. I just don't know how good they are.
