I am a friend or relative of a current/past resident

My Mom shares a room with one roommate. That keeps our costs down and the staff do an excellent job of keeping the space neat and clean. It is wonderful to see the staff interact with the residents in a knowledgeable way, recognizing and responding to their differences. The activities (live music, arts and crafts, field trips) and group meals are very important.

Provider Response

Thank you for sharing your thoughts on our Bridge to Rediscovery memory care program. We appreciate your feedback on the activities and meals -- and are glad to know you have been so impressed with the team and the individualized attention they provide for each and every resident.

I am a friend or relative of a current/past resident

My mother was a resident here for 2 months, and it was not a good experience. I feel like I owe it to other family members looking for care - to post a review. If I had been warned - I would not have chosen this place for Mom. They don't have enough staff - which I know can be related to post-COVID - but I feel like management should make sure the staff is sufficient. They also don't provide care that is promised (ie bathing 2x /week, checking every 2 hours). Mom missed meals on several occasions. Also people roamed in/out of her room, sometimes in the middle of the night. I know that this can be hard to control sometimes with memory care, but I feel like they could do better. Several of mom's items were missing, either due to someone taking from her room (resident) or doing laundry combined with other residents. The communication is not good for the most part. There are a few staff members that were good about this, so I can't say that all staff were terrible. But overall not good. When Mom passed away on 9/13 - we as the family were never contacted by the staff. We were notified by the Trellis nurse, over an hour later. It was clearly stated that they were to contact us first, then contact Trellis. I had someone from outside checking in on mom several times - and they also reported lack of care. They wouldn't even wash them up in the morning before breakfast. There is nothing that I can do for mom now, and I know she is at peace. My hope is that this helps the facility to fix some issues, or warns a family member to make a better choice.

Forest Heights got a friendly staff. They seem to be taking good care of the patients. They're able to accommodate dietary needs of mom. Those are some of the basics. The facility is very good. It's modern looking and well kept. The food is nice and very good. They have a multitude of activities, such as the basic ones like bingo and other memory games, as well as crafts. My mom plays piano. They have a baby grand piano that they can use and other music type things. Sundays they have events for church. Also there is gardening, which is what they're working on now.

My mother moved into Forest Heights Senior Living Community. Our experience is not good. They do not have help, they don't have enough people to run the place. They do not have qualified med techs, the nurses, the CNAs, the maintenance people, are trying to run the dining hall. They have two directors' positions empty. They don't even have a food director, the nutrition is horrible and it takes them an hour and a half to two hours to have a meal. There's no one around, and you get no service. There are maybe only half a dozen people that you can carry on a conversation with. Her room is great, it's large, she has a sitting here, a kitchenette, and a bathroom. They play bingo about twice a week and they get their nails done once in the last three weeks.

Do not care about their residents. Terrible food, unfriendly staff, medication errors.

Stay far, far away from this place!!! Do not put your loved one in here. They roll out the red carpet on your tour and must literally shred it after! There is NO management and when there is a director for a few weeks they appear clueless to the mess they are inheriting!! Medications for my grandfather were constantly missed!! The nursing staff is truly a JOKE ! They all work together to sweep stuff under the rug!! Good thing my 85 year old grandpa is not your typical 85 year old as far as memory!!! There is so much wrong here- we are leaving in January when he gets out the hospital ! Looking for a better quality place!!! I hate to give them one star. Pattie, Lindsay, Chrissy, all of them need training!!! There are a few wonderful CNAs other than that , it’s pretty scary the level this place operates on

We were impressed with Forest Heights and it is in the running. We just haven't made a decision yet. Everything there felt good, we also ate there, and the food was good. We liked the feel of the place and the way it looked, especially the cleanliness and the people there. We looked at the assisted living during that tour and the staff was friendly. The room was nice, it was not a small room, but it wasn't oversized either. They have activities like arts and crafts, they take trips, they do things like ice cream socials, and things that families can come to. Overall, we were impressed.

If you are considering placing a loved one(s) at this facility, I beg you to reconsider! This property is part of 5 Star Senior Living, but there is nothing 5 star about this place. My grandparents have lived here since 2016, in those 3 years, there has been at least 3-4 different executive directors. Each time, promises of improvement and changes to come are said, and then either the ED is reassigned or leaves, and the poor work culture returns. My grandparents have received rude and intimidating treatment from staff on a consistent basis. It costs a considerable amount of money to live at this facility and yet, in spite of this, oftentimes residents are only given small or half portions of food and leave the dining room hungry. The quality of the food that is prepared is less than appealing. In addition to rude staff, there are often medication errors in dispensing my grandparents medications. My family has repeatedly had conversations with the executive directors and even to the company’s corporate office and that has fallen on deaf ears. There are some very loving and wonderful staff members that my grandparents and my family dearly love, had it not been for them, it would have been completely unbearable here. Unfortunately, their good deeds get overshadowed by the substandard treatment and poor performance of job duties by some staff. I shared this same review on google. I want to ensure that the word gets out to the community on the extensive problems at this facility and the poor company culture that exists here as well as at its corporate. These are Unacceptable conditions for my grandparents and fellow seniors, many of whom are military vets to have to endure! If President Trump could see this place he would say, “Drain the Swamp!”

Everything was great at Forest Heights. The staff was very informative. The dining area looked nice to me. They told me the things that they do and they showed me different areas. They had a calendar of what takes place on a daily basis. They had a lot of different activities like daily meals and upcoming events. The room I saw was being renovated. It's a place that I would put my uncle in. I felt comfortable in their care.

My brother is a resident of Forest Heights Senior Living Community. From what I've seen, I would say that I am satisfied with the accommodation and the staff. As far as his accommodation is concerned, it's adequate. We chose a studio for him with adequate space and a the bathroom that is very open and large with a walk-in shower. They also have nice, huge closets. He has participated in a couple of outings They have people who come in to volunteer, some residents play the piano and music instruments, and so on. Also, during the holidays they had lots of gatherings for the residents, like a Christmas party and New Year's Eve celebration. The quantity of the food is something we had to adjust since my brother mentioned that he wasn't given large enough portions. He also indicated that he would like more variety. I saw the menu, so I was a little surprised when he mentioned that, but he's someone who doesn't like to eat the same thing more than once a week.

IIf you have a parent or loved one with Dementia or Alzheimers DO NOT consider this place! I moved my parents here and my father was treated horribly . After 2 hospital visits that should have never happened he was discharged with my mother still there and they have been married for 67 years. We were appalled at their lack of empathy or experience for our father who was highly functioning and within 6 weeks went straight down hill. Management is the worst I have ever heard about and they should not be allowed to advertise as a memory care facility when they couldn’t even handle my father who never hurt himself or others . Several staff were great with my mother but I would never have left her there after the way they treated my father. He is doing fantastic at another facility that knows what they are doing. Writing this to prevent the nightmare that we have experienced at this facility. Not enough room to tell the complete story but I assure you it is unbelievable !!

Provider Response

As the Regional Director of Operations, I want to reassure you that at Forest Heights -- as in all of our Five Star communities -- resident care, safety, and happiness is our top priority. We provide the team members in our memory care neighborhood with training according to state regulations as well as in our organizations' unique Bridge to Rediscovery standards. Our goal is to provide a safe and nurturing environment where seniors living with Alzheimer's disease and other forms of dementia can flourish and share a positive life experience. That said, I would welcome the opportunity to speak with you regarding your observations as we are always looking for ways to listen and improve. Please contact me at escales@5ssl.com. Kind regards -- Erin.

Terrible experience...our father fell several times when the staff took him to the bathroom. There were orders in the chart that 2 people were to assist but they continued to disregard those orders. The med tech didn't even know he had congestive heart failure. The power went out during a snow storm and the facility didn't even notify us and didn't even have a generator. The person In charge (LPN) was extremely rude and unprofessional.

Forest Heights was great, clean, and intentional about the plan for each person. The food was great. The personnel made eye contact and related well to each individual. However, they were too expensive.

I am a friend or relative of resident

My 90 years young mother has been in the bed for over 2 weeks and had basically given up. She is in a weakened condition with depression & anxiety & takes numerous medications. She was told a couple of weeks ago that she would be getting a roommate as she is in a 'shared room '. When she originally moved in over 3 years ago her roommate was bed bound & could not communicate. After several weeks the roommate was moved out to a room by herself due to an 'infection'. She died shortly thereafter. So we always knew that a 'new' room mate could happen. After communication last week to the business manager that I wanted to be notified prior to the new room mate moving in so we could have time to move Mom's things, I was not notified that today the new roommate's grandson decided to stop in & wanted to see the room & meet his grandmother's new roommate. I was not called. The director Mr Weaver & the sales/marketing staff went to my mom's room with the grandson. My mom verbalized to Mr Weaver that she was not having a good day. She was still in her pajamas as she has been x 3 weeks. Mr. Weaver & sales/marketing proceeded anyway with the grandson into the room to meet my mom. I am APALLED at the lack of respect given to my mother today. A strangers' request, male at that, superseded my mom's not feeling good today. Mr Weaver stated he was unaware of my request to the business manager. It appears he also did not know Mom's condition for the last 3 weeks. Mr. Weaver reluctantly admitted that they should have turned around and left the room and told the grandson that this was NOT A GOOD TIME to meet his grandmothers new roommate. I cannot believe that they did this to my mother.

Forest Heights provided compassionate, caring, and skillful care for my father. Could not have asked for more accommodating administrators and caring staff: from the social events he loved, the meals he deemed "very good", and the staff that looked after him as if he were their own family, and treated his visiting family well also. Also, everything was spit spot clean, with beautifully kept grounds. Loved the views of nature from his room. Thank you so much.

Forest Heights Senior Living wasn’t the right type of facility for my mother. I believe that they deal with Alzheimer's patients and memory care and she doesn’t have that. My mom has kidney failure so dialysis is what she is looking for. She needs a special diet, but they do not provide that for her there. The staff was very nice and very helpful. They were friendly and knowledgeable but I was blindsided by the fact that that’s all they specialized with.

I think it is a good facility. We are pleased with Forest Heights. The people working there are fine. The staff that gave me the tour was fine. Everything is fine right now. The rooms are nice. The price range of the facility impressed me the most. They do have some activities for the residents. I think it is good that the residents have a choice of food, and they try to work with the residents on what they need. My experience with this place is good. When we first got there though, the room was not ready. They knew we were coming, but there were no linens in the room. We didn't have much communication with the nursing staff. Those are the two reasons why we didn?t give it a perfect score.

My mom was only at Forest Heights for respite care. It was very nice, she liked the staff, and the food was good. The staff was skillfully trained, and as far as assistant living, I thought that they had a pretty extensive menu for her. You could have breakfast all day if you wanted, which was a good option. Mom had a suite, when you are in respite you get to choose, so it was pretty spacious. They had some social activities with the piano player, I know they had bingo. but she didn’t really do too much.

It was very difficult for me to accept that caing for my mom at home was going to be very trying. However, with prayer, God led me to this facility. I visited 5 facilities. One was just as nice but the sales person at Forest Heights was excellent and caring. She has walked in our shoes and personally knows the trials. The tour, move-in process, as well as admission went well. It is a beautiful, well decorated, no order, well kept facility. Caring.com assisted me all the way and being a former social worker, the shoe was now on the other foot and their help was critical since my mom's discharge from short term rehab was fastly approaching. Now the real test is coming, hope the care and attention to the residents is as beautiful as the facility, as so far the staff is very attentive. When I moved my mom's belongings in the day before, I observed very good care of the residents.

The building and rooms in Forest Heights were nice and clean, but it was more institutional. It does not have as fresh a smell. The nurse was actually rude to the person taking us on a tour. If the nurse was rude in front of a potential client, I can’t imagine how they could be behind your back. It wasn’t the kind of place I would want my mom to live in.

Provider Response

Thank you very much for your review. I recall our conversation in a follow up call placed after your tour when we learned that we fell short of your expectations. I appreciated your willingness to share why you were not choosing our community and when I shared with our team, everyone jumped into action to focus on the areas mentioned, using your feedback to improve. I am familiar with the incident that you described with our two teammates as I circled back around just as soon as our call was over to learn more of the circumstance. The “nurse” mentioned was actually a Medication Technician and although she was dealing with a situation that needed her immediate and undivided attention, during our counseling, she readily admits that she should have responded to her co-workers’ request differently. It was a great coaching opportunity, and we thank you for providing the feedback to help us address. As I shared during our conversation, we were aware that although not bad, our community could improve in the category of “freshness”. We have implemented several new best practices to ensure that any situations that might cause our community to be less than fresh, be addressed immediatley. We have a wonderful and caring team (I remember your very positive comments about the individual that toured with you), and they are passionate about providing a great senior living option for those we serve. If you have not found just the right community for you Mom, and even if you have, we encourage you to visit us again. We would appreciate your perspective.

Sincerely, Amanda Moore, Regional Director of Sales & Marketing, Five Star Senior Living

I visited Forest Heights for myself. They took me around and discussed the whole thing with me. The facility was just perfect. It was like living in a hotel. The staff was wonderful and accommodating during my visit. I also had lunch there and the food was excellent. They told me that I could even go and spend two days and one night so that I would get the idea of just how things operate.

I am/was a resident of this facility

I am a resident of Forest Heights Senior Living, beginning my seventh year here. Obviously, I love it or I would have moved elsewhere. I have seen a multitude of changes here, making each year of my residency better than the one before. Of course I would rather be living in my own home--who wouldn't?--but since I cannot, there is no place I would rather live. The staff turn themselves into pretzels trying to make us happy and comfortable. No request goes unconsidered and is acted on if at all possible. I am encouraging my father to join me here as he is beginning to see the need for more assistance and I certainly would NEVER want him anywhere but the best possible place and FOREST HEIGHTS IS THE BEST!!! In fact, the state of North Carolina has just declared my home DEFICIENCY FREE!! I find little to criticize and much to love in my assisted living home. Come visit and talk to the residents as well as the staff. We will tell you how great our home is!

Forest Heights gives a great tour, but what really happens there is very different from the way that things appear! In the year that my parents were living there, we encountered MANY problems! Medications were improperly administered, doctor's orders were overlooked, there was little security after hours, policies were continually changed from our resident agreement, food services declined considerably, staff did not fulfill their responsibilities for assistance with bathing, laundry was returned wet and not folded. There was little continuity with staff and many staff members were quite unprofessionable! We went through 4 different RN's and 3 Executive Directors during that time. Needless to say, my parents were moved to a different facility.

I was generally very pleased with the facility, and I remember thinking that I would be satisfied staying there myself if necessary one day. The staff members I encountered were professional and friendly. The amenities were very nice; clean, new, and in good condition. Generally, the I would describe the mood of the residents as upbeat. Many of the residents were engaged in various activities in the common areas during both of my visits, including sitting together talking on rocking chairs on the front porch, playing billiards, and watching television in the common areas. This was my first experience with an assisted living facility, and I was pleasantly surprised with how nice it seemed.
